Influence of preoperative short-term parenteral nutrition support on clinical prognosis of radical gastrectomy for patients with gastric cancer: A single-center propensity score matching analysis

Objective: To investigate the effect of short-term (1-6 days) preoperative parenteral nutrition (PN) support on postoperative complications and overall survival (OS) in patients undergoing radical surgery for gastric cancer. Methods: The research retrospectively collected clinical data from 862 patients who underwent radical surgery for gastric cancer at the First Affiliated Hospital of Wenzhou Medical University from July 2014 to February 2019. According to whether or not they received preoperative short-term PN support, the patients were categorized into the non-PN group and the PN group. Following 1:1 propensity score matching (PSM),the two groups of patients were compared in baseline characters, postoperative complications, and OS before and after PSM using inter-group comparisons, multivariate analysis and subgroup analysis. Results: A total of 358 pairs of patients were successfully matched. The hospitalization cost of the PN group was significantly higher than that of the non-PN group (P<0.001), whereas the length of postoperative hospital stay (LOS) and the readmission rate within 30 days were similar between the two groups (both P>0.05). There was no significant difference in the rates of overall postoperative complications, infectious and noninfectious complications between the two groups (all P>0.05). The results of logistic regression analysis showed that Charlson comorbidity index (CCI) 2-6 (OR=2.206, 95%CI=1.391-3.499, P=0.003) and Roux-en-Y digestive tract reconstruction modality (OR=1.810, 95%CI=1.189-2.756, P=0.005) were independent risk factors for postoperative complications,and laparoscope surgery (OR=0.637, 95%CI=0.424-0.955, P=0.029) was an independent protective factor for postoperative complications. The results of the subgroup analysis showed that none of the short-term preoperative PN support was beneficial for postoperative complications (all P>0.05). The survival analysis results showed that the difference in postoperative OS between the two groups was not statistically significant (P>0.05). The results of Cox regression analysis showed that age≥70 years (HR=1.412, 95%CI=1.060-1.881, P=0.018), Rouxen-Y digestive tract reconstruction mode (HR=1.776, 95%CI=1.281-2.462, P<0.001), combined organ resection (HR=1.784, 95%CI=1.157-2.750, P=0.009), and TNM stages II (HR=2.176, 95%CI=1.321-3.587, P<0.001) and III (HR=4.575, 95%CI=3.015-6.942, P<0.001) were independent risk factors for postoperative OS. Subgroup analysis showed that no short-term preoperative PN support benefited postoperative OS (all P>0.05).Conclusion: Preoperative short-term PN support cannot significantly improve the clinical prognosis of gastric cancer patients and thus PN is not to be recommended for patients able to tolerate oral feeding.

Effect of endometrial CD138 positive on pregnancy outcome in vitro fertilization-embryo transfer in patients with repeated implantation failure

Objective: To study the effect of endometrial CD138 positive test on the clinical pregnancy outcome of in vitro fertilization-embryo transfer (IVF-ET) in patients with unexplained repeated implantation failure (RIF). Methods: A total of 332 patients (352 cycles) with unexplained RIF were selected for endometrial biopsy, immunohistochemical detection of CD138 positive cells in the First Affiliated Hospital of Wenzhou Medical University from July 2019 to May 2023. According to the detection results, they were divided as CD138-positive group and negative group. And according to the number of CD138-positive cell under 20 high-power field (20 HPFs), they were further divided into 1-5 /20 HPFs group, 6-20/20 HPFs group and >20/20 HPFs group.Some of the CD138 positive patients were treated with antibiotics, and the pregnancy outcome was followed up.Results: In 352 cycles, 181 cycles were positive for CD138, with the positive rate of 51.42%. Among them, there were 72 cycles in 1-5/20 HPFs group (39.78%), 63 cycles in 6-20/20 HPFs group (34.81%), and 46 cycles in >20/20 HPFs group (25.41%). Multivariate Logistic regression analysis showed that CD138 positive was one of the factors reducing clinical pregnancy rate (OR=-0.474, 95%CI=0.395-0.98, P=0.041). The clinical pregnancy rate and embryo implantation rate in CD138 positive group were significantly lower than those in negative group (P<0.05). When the number of CD138 positive cells was more than 20 for per 20HPFs, the clinical pregnancy rate and embryo implantation rate were significantly lower than those in the negative group (P<0.05). When the number of C138 was less than 5 for per 20 HPFs, there was no significant difference in pregnancy outcomes between antibiotic treatment and untreated groups. Patients with persistent CD138 cell positive after antibiotic treatment had significantly lower rate of clinical pregnancy embryo implantation and live birth than patients with initial negative of CD138 cell (P<0.05). Patients who turned negative of CD138 cell after treatment had similar clinical pregnancy rate, embryo implantation rate and live birth rate compared with those who had the initial negative of CD138 cells. Conclusion: The number of CD138-positive cells in endometrium more than 20 for per 20 HPFs may reduce the embryo implantation rate and clinical pregnancy rate. The clinical pregnancy outcomes can be improved if CD138 turns negative after antibiotic treatment, but persistently positive CD138 can affect the pregnancy outcome. A small amount of CD138 positive (<5/20 HPFs) at the initial test had no significant effect on the pregnancy outcome.

Ultrasound-guided injection of botulinum toxin type A into the subscapularis and pectoralis major muscles alleviates hemiplegic shoulder pain

Objective: To investigate the impact of ultrasound-guided injection of botulinum toxin type A (BoNT-A) into the subscapularis and pectoralis major muscles on shoulder pain in patients with hemiplegia. Methods: Patients diagnosed with cerebral infarction and cerebral hemorrhage admitted to the Rehabilitation Department of Wenzhou TCM Hospital from February 2023 to January 2024 were selected as the research subjects. Using the random control principle, 36 eligible patients were divided into the intervention group and the control group, with 18 patients in each. In the intervention group, 100 U (2 mL) of BoNT-A was injected into the subscapularis and pectoralis major muscles under ultrasound guidance, totaling 200 U (4 mL). The control group received an equal amount of normal saline injection. The changes in VAS, painless joint range of motion (pROM),muscle tone (MAS), upper limb function score (FMA-UE), and activities of daily living score (MBI) were observed at 2 weeks, 1 month, 3 months, and 6 months after treatment, along with the occurrence of related adverse reactions. Results: All 36 patients completed the treatment successfully. There was no statistically significant difference in baseline values between the two groups (P>0.05). The VAS and MAS in the intervention group were lower than those in the control group starting from 2 weeks after treatment (P<0.05). The pROM was higher in the intervention group than in the control group starting from 1 month after treatment (P<0.05). There was no significant difference in FMA-UE and MBI between the two groups at each time point after treatment (P>0.05).Conclusion: Ultrasound-guided injection of BoNT-A into the subscapularis and pectoralis major muscles can alleviate shoulder pain, reduce spasticity, and improve shoulder joint mobility in patients with hemiplegia.

Clinical efficacy of immersive virtual reality technology combined with early rehabilitation training for postoperative patients with rotator cuff injuries

Objective: To observe the effect of immersive virtual reality technology (VR) combined with early rehabilitation training on the early and mid-term rehabilitation treatment of postoperative patients with rotator cuff injury. Methods: Forty-five patients who underwent the initial rotator cuff repair in the Second Affiliated Hospital of Wenzhou Medical University between April 2023 and November 2023 and met the inclusion criteria were selected as subjects. They were randomly divided as the experimental group (n=22) cases and the control group (n=23). From 0 to 2 weeks after the operation, routine passive rehabilitation training was performed in both groups; from 2 to 6 weeks post-operation, the control group adopted early rehabilitation training, and the experimental group, on the basis of early rehabilitation, was given immersive virtual reality system for upper limb motor function training. The VAS pain visual analog scale score, UCLA shoulder score, shoulder joint active and passive forward flexion and external rotation mobility, and cross-sectional area of the supraspinatus muscle on the affected side (CSA) were evaluated before and after 4 weeks of treatment, respectively, in both groups.Results: During the study 1 case in the experimental group and 2 cases in the control group who failed to finish were thus excluded. There were 21 cases who completed in either group and were included in the final efficacy analysis. After treatment, VAS score decreased, UCLA score increased, active and passive forward flexion activity and CSA of the affected supraspinatus muscle increased compared with the pre-treatment period, and active and passive external rotation of the arm increased compared with the pre-treatment period in the experimental group, and all the indexes of the experimental group improved significantly compared with the control group,with significant difference (P<0.05). In the control group, there was no significant increase in the active-passive posting arm external rotation angle compared with the pre-treatment period (P>0.05), and both groups showed a significant correlation between the supraspinatus CSA and the UCLA shoulder joint score (P<0.05). Conclusion:Immersive VR combined with early rehabilitation can effectively reduce patients’ pain, increase shoulder joint mobility, improve patients' functional activities, and promote the repair of supraspinatus tendon.

The effect of erythropoietin on the lung ischemia/reperfusion injury in rats

Objective: To explore the role and mechanism of erythropoietin (EPO) on lung ischemia/reperfusion injury (LIRI). Methods: Adult male Sprague-Dawley rats were randomly divided into 5 groups (n=8): Control group, EPO single administration group (3 000 U/kg), LIRI Model group, LIRI model+EPO pretreatment group (3 000 U/kg) (EPO pretreatment group), LIRI model+EPO treatment group (3 000 U/kg) (EPO treatment group). The mRNA expression of Bcl-2, Bax and caspase-3 was measured by RT-qPCR. The pneumocyte apoptosis was achieved by TUNEL method. The level of FGF23 in lung tissues was determined by immunohistochemistry. The protein levels of FGF23, FGFR4 and p-ERK1/2 was detected by Western blot.The morphology of lung tissue was observed under the ordinary light microscope. Results: Compared with the control group, the expression of Bax and caspase-3 of LIRI model group had a significant increase (P<0.01),while the Bcl-2, FGF23, FGFR4 and p-ERK1/2 expression levels significantly decreased (P<0.01), and the lung tissue injury and cell apoptosis were aggravated. Compared with LIRI model group, the expression of Bax and caspase-3 of EPO pretreatment group and EPO treatment group was significantly decreased (P<0.01), while the Bcl-2, FGF23, FGFR4 and p-ERK1/2 expression levels significantly increased (P<0.01), and the lung tissue injury and cell apoptosis were reduced. Conclusion: Erythropoietin can effectively improve the lung ischemia/reperfusion injury, which may be related to the activation of the FGF23/FGFR4/p-ERK1/2 signaling pathway,up-regulating the expression of apoptosis inhibitor Bcl-2 while down-regulating the expression of pro-apoptotic genes Bax and caspase-3.

Network analysis of depression among mothers at the critical parenting period of 0-3 years

Objective: To investigate the network manifestations of symptoms in maternal depression during critical parenting periods. Methods: Mothers of infants and toddlers aged 0-3 years were recruited from 10 community immunization centers in Wenzhou, Zhejiang Province, China, between March and September 2021. The Edinburgh postpartum depression questionnaire was used to assess maternal depression symptoms.Results: The primary symptom of maternal depression during childcare was identified as “feeling sad or miserable,” showing the highest mediation, proximity, intensity, and expected impact centrality (2.51, 1.66, 1.67,0.77), followed by “being so upset as to cry” (-0.44, 1.05, 1.10, 0.76). Conclusion: The findings suggest that emotional symptoms such as sadness, misery and crying are key manifestations of maternal depression in critical child-care periods, which could be addressed in future interventions for depression in critical period baby care mothers.

Effect of bovine basic fibroblast growth factor combined with chlortetracycline hydrochloride eye ointment on wound healing of oral-mucosal pressure injury after orthognathic surgery

Objective: To explore the clinical effect of bovine basic fibroblast growth factor for external use combined with chlortetracycline hydrochloride eye ointment on wound healing of oral-mucosal pressure injury after orthognathic surgery. Methods: Patients with oral-mucosal pressure injury after orthognathic surgery in the Affiliated Hospital of Stomatology, Wenzhou Medical University from June 2018 to September 2023 were selected as subjects, who were divided into three groups according to the medication methods as the group of chlortetracycline hydrochloride eye ointment, the group of bovine basic fibroblast growth factor for external use and the group of bovine basic fibroblast growth factor for external use combined with chlortetracycline hydrochloride eye ointment (denoted as combination group), with 30 cases in each group. The pain degree,mouth opening, wound healing time and satisfaction degree were compared between the three groups. Results:After 5 days of medication, the visual analogue scale (VAS) score of the three groups were all significantly decreased (P<0.05) and the mouth opening of the three group were distinctly enlarged (P<0.05), compared with before medication. After 5 days of medication, the differences of VAS score and the mouth opening among the three group were statistically significant (H=13.921, P=0.001; F=8.569, P<0.001). The combination group had the lowest VAS score and the biggest mouth opening compared with the other two groups (P<0.05). There was statistical difference in the values of VAS score and the mouth opening before and after medication among the three group (H=18.292, P<0.001; F=6.720, P=0.002), and the difference value of the combination group was the biggest. The difference in the wound healing time and satisfaction degree for the three group were statistically significant (F=9.510, P<0.001; χ2=8.940, P=0.011), in which the combination group had the shortest healing time and greatest satisfaction degree (P<0.05). Conclusion: The combination of bovine basic fibroblast growth factor for external use with chlortetracycline hydrochloride eye ointment can improve patients’ satisfaction degree in that it promotes the wound healing of oral-mucosal pressure injury after orthognathic surgery, reduces postoperative pain and enlarges the mouth opening while shortens the wound healing time.

Exploring the mechanism of ursolic acid in the treatment of colon cancer based on the PI3K/Akt/mTOR signaling pathway

Objective: To investigate the effect of ursolic acid on the colon cancer cells and its mechanism.Methods: A blank control group and different concentrations of ursolic acid treatment groups were established.The proliferation activity of each group of cells was measured using the CCK-8 method. The migration ability of each group of cells was measured through cell scratch assay, and the effect of ursolic acid on the PI3K/Akt/mTOR signaling pathway was measured through WB assay. Results: Through CCK-8 experiments, it was found that ursolic acid could significantly reduce the survival rate of SW480 and LOVO colon cancer cells, and with the increase of drug concentration, the survival rate of cells significantly decrease (P<0.05). The cell scratch experiment showed that compared with the control group, the low concentration of ursolic acid treatment did not significantly reduce the cell healing rate (P>0.05), while the high concentration of ursolic acid treatment significantly reduced the cell healing rate (P<0.05). The WB experiment results showed that the expression levels of p-PI3K, p-Akt, and p-mTOR proteins in cells treated with ursolic acid were significantly reduced (P<0.05).Conclusion: Ursolic acid has clear anti-tumor properties by inhibiting the expression of the PI3K/Akt/mTOR signaling pathway, thereby inhibiting the proliferation and migration of colon cancer cells.

Perinatal outcomes of increased nuchal translucency in fetuses with normal karyotype and copy number

Objective: To investigate the perinatal outcomes of fetuses with normal karyotype and copy number with nuchal translucency (NT) ≥3.0 mm, so as to provide a scientific diagnostic strategy for pregnant women and their families. Methods: From January 1, 2019 to December 30, 2021, 57 pregnant women with normal karyotype and copy number with increased nuchal translucency were enrolled in the Second Hospital.Color ultrasound examination and postnatal follow-ups were performed at 22, 28, 32, and 36 weeks of gestation.Pregnancy complications, delivery mode, and neonatal outcomes were recorded. Results: A total of 120 cases of NT thickening were detected, of which 92 cases had normal karyotype and copy number, of which 35 cases were lost (38.0%), and 57 cases were followed up. Karyotype and copy number abnormalities were found in 28 cases (23.3%). Among the 57 pregnant women with NT thickening who were followed up with normal karyotype and copy number, the rate of spontaneous abortion was 3.5% and the rate of abnormal structural malformations 12.3%.Cardiac malformation was the most common structural malformation, accounting for 42.9% of the structural malformations. Preeclampsia occurred in 3.6%, insufficient amniotic fluid and fetal growth restriction in 1.8%,gestational diabetes in 14.5%. We stratified the degree of NT thickening and found that when NT >5.0 mm,the proportion of miscarriages (stillbirths) and live births with structural abnormalities increased even though the chromosomes and copy number were normal. Conclusion: With normal karyotype and copy number, NT thickening does not increase pregnancy complications and has a better perinatal outcome. However, when NT >5.0, the proportion of abortion (stillbirth) and the proportion of live births with structural anomalies increase.During pregnancy, ultrasound (including fetal echocardiography) should be performed to exclude structural malformations, especially cardiac malformations.

Current situation of nursing management of venous thromboembolism in Wenzhou: An investigation with analysis

Objective: To investigate and analyze the current situation of nursing management of venous thromboembolism (VTE) in Wenzhou so as to provide an effective reference for proposing accurate countermeasures. Methods: Based on the VTE nursing quality control data from Wenzhou Nursing Quality Control Center, the current status of VTE nursing management in hospitals of second-class or above in Wenzhou between 2021 and 2022 was investigated, including the bleeding risk assessment rate of VTE moderate-high risk patients, VTE high-risk patient testing rate, VTE related knowledge education rate, VTE education awareness rate and hospital-related VTE incidence rate. And the different years, administrative levels of hospitals, class and type of hospitals were compared and analyzed. Results: Among the 20 medical institutions involved in the survey, in 2021, compared with 2022, the bleeding risk assessment rate of VTE moderate-high risk patients increased from 72.56% to 81.91%, and the examination rate of high-risk patients increased from 77.25% to 85.52%; the education rate of VTE related knowledge increased from 76.54% to 80.71%, the awareness rate of education increased from 72.17% to 76.97%, and the incidence of hospital-associated VTE increased from 2.20% to 2.57%, with all the differences statistically significant (P<0.01). There was significant difference (P<0.01) in VTE nursing management related quality control indicators such as the bleeding risk assessment rate of VTE moderate-high risk patients, VTE high-risk patient testing rate, VTE related knowledge education rate, VTE education awareness rate, hospital related VTE incidence rate. Compared between general hospitals and specialized hospitals, between provincial hospitals and municipal or county-level hospitals, between tertiary hospitals and second-class hospitals.Conclusion: VTE nursing management in Wenzhou has been generally improved in recent years. Provincial hospitals have obvious advantages over prefecture-level or county-level hospitals and tertiary hospitals have obvious advantages over secondary hospitals in VTE nursing management,suggesting that great differences exist in VTE nursing management among various medical institutions. Greater attention and more management measures are needed.

Research progress of the evaluation and treatment of furcation involvement

Furcation involvement is the periodontal lesion involving the root furcation area of multi‑root teeth, where periodontal pockets, attachment loss and alveolar bone resorption occur. The special anatomical structure of the root furcation area makes it very difficult to do the thorough debridement. In addition, the complex pathogenic factors lead to rapid progression of furcation involvement, making the treatment difficult. The loss rate of teeth with furcation involvement is higher than that of ordinary teeth. Therefore, early detection, accurate evaluation, and standardized treatment of furcation involvement is crucial for the prevention and control of periodontitis. This review summarizes the classical and recent researches home and abroad on the evaluation and treatment of furcation involvement, which is helpful to improve the clinical diagnosis and treatment of furcation involvement.

The status quo and countermeasures of humanities and social sciences in independent medical colleges under the background of new medical sciences: Statistical analysis based on CSSCI papers and research projects from 2013 to 2022

To provide decision-making support for the development of the disciplines, based on the understanding of the current development status of humanities and social sciences in independent medical colleges,The article statistically analyzed the total number of CSSCI papers, discipline distribution, and key discipline papers from 25 independent medical colleges, as well as the number of humanities and social science projects funded by the National Social Science Foundation and the Ministry of Education. The results showed that there was a significant gap in the research level of humanities and social sciences among the 25 independent medical colleges, and there was an imbalance in scientific research strength among disciplines. It is recommended to strengthen investigation and enhance disciplinary integration and interoperability by keeping a foothold in medical colleges and universities, and to promote the development of humanities and social sciences in independent medical colleges and universities through policies, platform construction, and other measures.

Curriculum and teaching reform from the perspective of the national medical practitioner qualification examination

Objective: To provide reference for the teaching reform of medical colleges by taking the scores and influencing factors of one annual medical practitioner qualification exam at Wenzhou Medical University (WMU) as the entry point. Methods: In this study, 878 participants in the Clinical Practitioner Examination,78 participants in the Public Health Practitioner Examination, and 26 participants in the Oral Practitioner Examination at WMU in a certain year were selected as subjects. The scores of each subject in the practitioner examination were compared with analysis with the national average score and average mastery rate; the correlation between the score of each subject during undergraduate medical education and the corresponding result in the National Medical Practitioner Qualification Examination was retrospectively verified. Results: The Clinical Practitioner Examination showed that the scores in all subjects were higher than the national average (P<0.05).Except for Health Statistics and Maternal and Child Health Care in the Public Health Practitioner Examination,and Oral Internal Medicine, Oral Prevention, Internal Medicine, and Surgery in the Oral Practitioner Examination, which had no difference from the national average, scores in all other subjects were higher than the national average (P<0.05). The scores in basic knowledge and professional knowledge in the National Practitioner Qualification Examination for Clinical and Public Health were moderately or low correlated with the scores of majority relevant subjects during the undergraduate years (P<0.05). However, there was no significant correlation between the scores of majority relevant subjects in medical humanities and dental majors (P>0.05). Conclusion: Importance should be attached to the undergraduate education of medical humanities for medical students to improve their humanistic literacy, and emphasis should be placed on process-based assessment and evaluation, combining theory with practice as well as problem-based teaching to ensure an effective curriculum reform and desired teaching quality.
