775 Prediction and prognostic value of host immune response markers in sepsis

Objective: To explore the value of host immune response markers in predicting the risk and prognosis of sepsis in patients with infection, and provide reference for the prevention and treatment of sepsis. Methods: From January 2019 to December 2021, adult patients who visited and were hospitalized due to infectious diseases were collected in the Second Affiliated Hospital of Wenzhou Medical University. The demographic data, comorbidities, national early warning score (NEWS), host immune response markers and other data of infected patients were collected. According to whether the infected patients progressed to sepsis, they were divided into infection group and sepsis group. Univariate and multivariate Logistic regression analysis were used to screen host immune response markers that independently predict the progression of infection to sepsis, and the area under the ROC curve (area under curve, AUC), sensitivity, specificity, positive predictive value and negative prediction value to evaluate the predictive performance of markers. Results: A total of 294 infected patients were finally included, among which 138 (46.9%) developed sepsis and 32 died in hospital (10.9%). CD3, CD4, C3, CD19, IL-6, IL-10 and IgE were predictive factors for infection progression to sepsis (OR=0.971, 0.965, 0.973, 1.068, 1.028, 1.569, P<0.05). Among them, IL-10 had the best ability to predict the progression of infection to sepsis, with an AUC of 0.803 (0.753-0.854). After combined with the NEWS, the AUC reached 0.844 (0.797-0.890). IL-6 had the best ability to predict in-hospital death, with an AUC is 0.799 (0.714-0.884). Conclusion: The host immune response markers are dysregulated in the early stage of infection, among which IL-6 and IL-10 have the best ability to predict the progression of infection to sepsis and prognosis,IL-10 combined with NEWS can further improve early sepsis risk identification, which provides reference for sepsis blocking.

782 Effect of hyperuricemia on hemodynamics and left ventricular diastolic dysfunction in primary hypertension

Objective: To analyze the effect of hyperuricemia (HU) on hemodynamic, and to explore the relationship between HU and left ventricular diastolic dysfunction in patients with uncontrolled primary hypertension. Methods: An observational study was conducted in 189 patients with uncontrolled hypertension who visited the First Affiliated Hospital of Wenzhou Medical University from August to December 2022. They were divided into hypertension with HU group (81 cases) and hypertension with non-HU group (108 cases). The demography, history of concomitant diseases, biochemical indicators, cardiac ultrasound indicators and noninvasive hemodynamic indicators were compared between the two groups. The relationship between HU and left ventricular diastolic dysfunction in hypertensive patients was investigated by multivariate logistic regression analysis. Results: The arterial stiffness index (ASI) in HU group was significantly lower than that in non-HU group (0.70±0.27 vs. 0.81±0.32, P=0.01). The hemodynamic phenotyping showed that the proportion of arterial vascular phenotype (high AS) and systemic vascular phenotype (high SVRI) in HU group and non-HU group was 60.49% vs. 65.74% and 80.56% vs. 85.18%, respectively, indicating no significant difference between the two groups (P>0.05). After adjusting the confounding covariates, multivariate Logistic regression analysis showed that HU was independently associated with left ventricular diastolic dysfunction in patients with hypertension (adjusted OR=5.05, 95%CI=1.87-13.58, P=0.001). Conclusion: HU affects the hemodynamic characteristics of primary hypertensive patients; HU is an independent risk factor for left ventricular diastolic dysfunction in hypertension.

789 The impact of obstructive sleep apnea hypopnea syndrome on cognitive processing in children

Objective: To investigate the effects of obstructive sleep apnea hypopnea syndrome (OSAHS) on cognitive processing in children by using Das-Naglieri Cognitive assessment system (DN:CAS). Methods: Forty children aged 8-13, who were diagnosed with OSAHS by polysomnography (PSG), were selected as the OSAHS group, and 30 healthy children as the control group. The general characteristics, sleep monitoring parameters,and DN:CAS cognitive assessment results of both groups were statistically analyzed. Results: Compared with the healthy controls, the OSAHS group had a higher proportion of stage-1 sleep and obstructive apnea hypopnea index (OAHI), but lower proportion of stage-3 sleep, REM sleep, and the minimum blood oxygen saturation (LSaO2), which showed statistically significant differences (P<0.05). The cognitive assessment using DN:CAS revealed that the OSAHS group had significantly lower scores in Full Scale, Planning, Matching Numbers, Planned Connections, Attention, Receptive Attention, Number Detection, and Expressive Attention, compared with the control group with statistically significant differences (P<0.05). The scores of Full Scale and Attention of DN:CAS in the OSAHS group were positively correlated with LSaO2 (r=0.331, 0.365, P<0.05). Conclusion:Children with OSAHS exhibit deficits in cognitive processing, primarily in the areas of planning and attention.Nocturnal hypoxia is associated with attention deficits in children with OSAHS. The DN:CAS assessment system has demonstrated potential value in evaluating the cognitive processing of children with OSAHS.

795 Efficacy and safety analysis of different thrombectomy techniques for the treatment of acute M2 segment occlusive cerebral infarction

Objective: To investigate the efficacy and safety of different embolization techniques in the treatment of acute middle cerebral artery M2 segment occlusive cerebral infarction. Methods: A retrospective analysis was conducted on the clinical data of 45 patients with occlusive M2 segment cerebral infarction who received mechanical thrombectomy treatment at Lishui Central Hospital from August 2018 to October 2022. The patients were divided into a direct aspiration thrombectomy group and an aspiration thrombectomy combined with stent implantation group (referred to as the combined group), and the differences in the number of thrombectomies, vascular recanalization time, treatment effectiveness, and good prognosis rate between the two groups were analyzed. Results: The average arterial puncture-recanalization time was shorter in the combined group than in the direct aspiration group [(61.7±17.4)min vs. (71.3±24.2)min, P=0.037], and the difference was statistically significant ; the number of embolizations in the combined group was less than that in the direct aspiration group [1.0(1.0, 2.0) vs. 2.0(1.0, 4.0), P=0.034], and the difference was significant difference; the efficiency of the combined group was better than that of the aspiration group (77.8% vs. 55.6%); the good prognosis rate of the combined group was better than that of the aspiration group (70.4% vs. 44.4%). Conclusion: Compared with direct aspiration thrombectomy, the combined technique of aspiration and stent implantation has a shorter time to revascularization and perhaps higher efficacy and favorable prognosis for acute M2 segment occlusion in the middle cerebral artery.

801 The effect of porcine acellular dermal matrix on fistula model repairing in rats and on TGF-β1/NF-κB p65 signaling pathway

Objective: To observe the effect of acellular dermal matrix on the repair of fistula model in rats and on TGF-β1/NF-κB p65 signaling pathway, and to explore its possible repairing mechanism. Methods: After adaptive feeding for one week, 40 healthy SD rats were randomly divided into four groups with 10 rats in each:sham operation group, model group, porcine acellular dermal matrix (pADM) group (dermal group), and positive control group (fistula plug group). After treatment for consecutive 2 weeks, the serum levels of IL-1β, IL-6,VEGF and prostaglandin E2 (PGE2) were measured by ELISA. The wound tissues were collected to observe the pathological changes. Western blot was used to detect the expression levels of TGF-β1, NF-κB p65 and VEGF protein. Results: Compared with the model group, the dermal group and fistula plug group increased significantly (P<0.05) in the body weight, and the wound pus decreased significantly. The levels of serum IL-1β, IL-6 and PGE2 in the dermal group were lower and the serum level of VEGF was higher (P<0.05). Meanwhile, HE staining showed that inflammatory infiltration in the dermal group reduced obviously. Moreover, the collagen fibers increased significantly and became denser. In both dermal group and fistula plug group, the expression levels of TGF-β1 and VEGF proteins increased while the expression of NF-κB p65 protein decreased in wound tissues (P<0.05). Conclusion: Porcine acellular dermal matrix could inhibit the inflammatory reaction in fistula wound tissue in rats, promoting angiogenesis and wound healing. Its promoting effect on collagen generation is much more obvious in the dermal group than in the anal fistula plug group. The underlying mechanism might be related to the regulation of the TGF-β1/NF-κB p65 signaling pathway.

807 The diagnostic value of multimodal ultrasound and contrast-enhanced computed tomography in the discrimination of clear cell and non-clear cell renal cell carcinoma: a comparative study

Objective: To compare and analyze the diagnostic performance of multimodal ultrasound and contrast-enhanced computed tomography (CECT) in differentiating clear cell renal cell carcinoma (ccRCC) from non-clear cell renal cell carcinoma (non-ccRCC). Methods: This study included 121 patients with renal cell carcinoma (RCC) and 125 renal lesions, including 88 ccRCCs and 37 non-ccRCCs. Preoperative multimodal ultrasonography and CECT were performed in all patients, whose postoperative pathological results were obtained. Clinical data and imaging variables of all tumors were analyzed to compare the diagnostic performance of multimodal ultrasound and CECT in ccRCC and non-ccRCC. Weighted kappa statistics was used to estimate the interobserver agreement. Results: Univariate and multivariate analyses showed that tumor cystic changes (OR=2.744, 95%CI=1.022-7.369, P=0.045), parenchymal phase enhancement (OR=4.102, 95%CI=1.154-14.573,P=0.029) and the wash-out pattern (OR=0.226, 95%CI=0.083-0.613, P=0.004) in multimodal ultrasound were independent predictors for distinguishing ccRCC from non-ccRCC. CECT parameters including tumor necrosis (OR=3.163, 95%CI=1.181-8.468, P=0.022), arterial phase enhancement (OR=4.103, 95%CI=1.192-14.117,P=0.025) and peak enhancement (OR=4.942, 95%CI=1.587-15.382, P=0.006)were independent predictors for ccRCC and non-ccRCC. Multimodal ultrasound and CECT both showed good diagnostic properties in distinguishing ccRCC from non-ccRCC, with sensitivity being 81.8% and 80.7%, specificity being 75.7% and 81.1%, and areas under the receiver operating curve being 0.832 and 0.838, respectively. There was no significant difference between the two methods. In addition, the diagnostic accuracy of multimodal ultrasound and CECT was 82.4% and 76.8%, respectively, with no statistical difference (P=0.620). The weighted kappa values of the interobserver agreement for multimodal ultrasound and CECT both ranged from 0.706-1.000. Conclusion: Both multimodal ultrasound and CECT have excellent diagnostic performance in distinguishing between ccRCC and non-ccRCC. Multimodal ultrasound can be used as an important supplement to CECT in the evaluation of renal cell carcinoma, especially in patients with iodinated contrast agent allergy and renal insufficiency.

814 An analysis of the association between the depth of periodontal exploration, the level of attachment loss,and the occurrence and severity of obstructive sleep apnea

Objective: To investigate the correlation between the depth of periodontal exploration, the level of attachment loss, and the severity of obstructive sleep apnea (OSA). Methods: Totally 157 subjects suspected of OSA due to snoring and undergoing polysomnography monitoring between June 2020 and August 2023 were examined for periodontal probing depth and attachment loss level. According to the severity of the condition, they were divided as healthy group (control group) (AHI<5 times/h, 61 cases), mild OSA group (5≤AHI<15 times/h,30 cases), moderate OSA group (15≤AHI<30 times/h, 33 cases), and severe OSA group (AHI≥30 times/h, 33 cases). The first part was divided into two groups: the control group (AHI<5 times/h) with 61 cases and the OSA group (AHI≥5 times/h) with 96 cases; The second part was divided into two groups: 30 cases in the mild OSA group (5≤AHI<15 times/h) and 66 cases in the moderate to severe OSA group (AHI≥15 times/h). General information and differences in periodontal indicators were compared among each group, and binary Logistic regression was used to analyze the associated factors of OSA occurrence and the factors that affected the severity of OSA. The receiver operating characteristic (ROC) curve was established for evaluation and analysis of the depth of periodontal exploration, the level of attachment loss, and the occurrence and severity of OSA. Results:There were statistically significant difference in periodontal exploration depth, attachment loss level, and diastolic blood pressure between the OSA group and the control group (P<0.05). Binary Logistic regression showed that an increase in attachment loss level (OR=1.225, 95%CI=1.084-1.385, P=0.001) and an increase in periodontal exploration depth (OR=1.545, 95%CI=1.336-1.787, P<0.001) were associated factors affecting the occurrence of OSA. The ROC curve showed that the level of attachment loss, the depth of exploration, and the area under the ROC curve for joint assessment of OSA occurrence were 0.752, 0.905, and 0.927, respectively. There was significant difference in the level of attachment loss between the mild and moderate to severe groups (P<0.01),while there was no statistical difference in the depth of periodontal exploration between the mild and moderate to severe groups (P>0.05). Binary Logistic regression showed that the level of attachment loss (OR=1.256,95%CI=1.108-1.423, P<0.001) was an associated factor for moderate to severe OSA. The ROC curve showed that the area under the ROC curve for assessing the level of attachment loss in moderate to severe OSA was 0.770,which was of evaluation value for moderate to severe OSA. Conclusion: There is a correlation between the level of attachment loss, depth of exploration, and the occurrence of OSA, and the level of attachment loss is correlated to the severity of OSA.

819 Influence of high-speed power system on the air quality during nasal endoscopic transethmoidal surgery

Objective: To analyze the impact of high-speed power systems on the air quality during endoscopic transluminal surgery, and to provide recommendations for intraoperative prevention and control for medical staff in the operating room. Methods: The air during nasal endoscopic transethmoid operation in Eye Hospital Affiliated to Wenzhou Medical University from September to November 2021 was selected as the study object. An air quality detector and an air microbiological sampler were used respectively to measure in real-time at the patient’s nostril level, the level at 50 cm from the patient’s nostril, the level at 1.5 m from the patient’s nostril,and the surrounding areas of the operating room during surgical incision, when the diamond grinding drill was used in the nose, when the micro power planer head was used in the nose, when contaminated high-speed power system was activated in vitro, and 10 minutes after discontinuation of high-speed power systems. The changes in PM2.5, PM10, particle count, and planktonic bacterial concentration generated at the above sampling points were analyzed. Results: With the use of intraoperative high-speed power systems, PM2.5, PM10, and particle count showed a fluctuating upward trend, reaching their peak during in vitro activation, and the concentration of
particulate matter ranked in order from high to low as follows: when the contaminated high-speed power system activated outside the body, when the micro dynamic force cutting blade used inside the nose, and when the diamond grinding drill used inside the nose (P<0.01). The PM2.5 concentration, PM10 concentration and particle
number at different sampling locations were statistically different (P<0.01), with the concentration ranking in order from high to low as follows: patient’s nostril>surgeon’s mouth and nose>instrument nurse’s mouth and nose>operating room’s surrounding area, showing significant difference (P<0.01). The concentration of planktonic bacteria from different sampling locations during surgery was higher than the baseline, with statistical difference (P<0.05). Conclusion: The use of high-speed power system during endoscopic transethmoidal approach will increase the number of particles and bacteria in the air of the operating room, and produce different effect with
different using modes and from difference working distance. Nursing staff should take corresponding prevention and control measures to reduce occupational exposure and prevent from surgical infection.

825 The clinical utility of expanded non-invasive prenatal testing in chromosomal microdeletion and microduplication syndromes

Objective: To investigate the clinical utility of expanded non-invasive prenatal testing (NIPTplus) in screening for chromosomal microdeletion and microduplication syndromes (MMS) in fetuses. Methods: A total of 5 027 pregnant women who underwent NIPT-plus testing at Wenzhou Central Hospital from January 2020 to June 2023 were included in this study. Among them, 54 pregnant women with NIPT-plus results indicating high risk for MMS were recommended to undergo amniocentesis for chromosome microarray analysis (CMA) to assess the detection performance in high-risk MMS pregnancy. Results: Of all the pregnant women, those aged >35 years accounted for 42.59% (2 141/5 027). The NIPT-plus results revealed 54 cases with high risk for MMS yielding a positive rate of 1.07% (54/5 027), of which pregnant women aged >35 years accounting for 44.45% (24/54). Altogether 35 pregnant women underwent invasive prenatal diagnosis, of which 14 cases were confirmed as MMS, with a positive predictive value of 40% (14/35). Additionally, two cases showed discordance between CMA results and NIPT-plus results, and seven pregnant women chose to terminate the pregnancy after genetic counseling. Conclusion: NIPT-plus demonstrated a relatively high positive predictive value in screening for high-risk MMS pregnancies.

830 The application of ciprofol combined with sufentanil for deep sedation in fiberoptic bronchoscopy

Objective: To investigate the efficacy and safety of intravenous anesthesia of ciprofol combined with sufentanil for deep sedation in fiberoptic bronchoscopy. Methods: A total of 60 patients who received fiberoptic bronchoscopy in Lishui Hospital of Traditional Chinese Medicine between January 2021 and March 2023 were selected and divided into ciprofol group and propofol group by random number table method (n=30).The ciprofol group (Group H) was given 0.1 μg/kg of sufentanil and 0.4 mg/kg of ciprofol successively, and the propofol group (Group B) was given 0.1 μg/kg of sufentanil and 1-2 mg/kg of propofol successively. The heart rate (HR), mean arterial pressure (MAP), and finger pulse oxygen saturation (SpO2) before examination (T0),as the fiberoptic bronchoscope entering the glottis (T1) and immediately after bronchoscopy (T3), as well as the onset time of anesthesia, recovery time, exit time, bronchoscopy time, number of assisted ventilation times, and incidence of adverse events, were monitored and recorded respectively. Results: As compared with T0, there was statistical significance (P<0.05) between Group H and Group B in decrease of MAP at T1 and T2. There was statistical significance (P<0.05) between Group H and Group B in degree of SpO2 decline and incidence of respiratory at T1 and T2, as compared with T0 and the decline degree of MAP and SpO2 at T1 and T2 in Group H was significantly lower than in group B, with statistical significance (P<0.05). However, there was no statistical significance (P>0.05) between the two groups in exit time and examination time. The incidence of respiratory depression, incidence of adverse events, and the number of assisted ventilation times in Group H were lower than in Group B, with statistical significance (P<0.05) between the two groups. Conclusion: Under the same titer,ciprofol is equivalent to propofol combined with sufentanil in anesthetic effect for fiberoptic bronchoscopy.Ciprofol combined with sufentanil is safe when used for anesthesia in fiberoptic bronchoscopy in that it has slight effect on the patient’s breathing and blood pressure, stable hemodynamics and less adverse reaction.

834 Clinical diagnosis and literature review of Helsmoortel-Van der Aa syndrome

Objective: To summarize the clinical manifestations and genetic characteristics of Helsmoortel-Van der Aa syndrome (HVDAS) caused by ADNP gene mutation with a review of the related literatures. Methods:The clinical data of the patient with HVDAS confirmed in August 2020 at the Department of Pediatrics of Sun Yat-sen Memorial Hospital, Sun Yat-sen University were collected retrospectively. The data for analysis included clinical manifestations, biochemical tests and gene tests. Meanwhile, literatures on HVDAS caused by ADNP gene mutation were searched from domestic and foreign databases. Results: The patient, male, 5 years and 10
months old, was admitted to hospital due to motor and language development retarding for more than 5 years and social communication impairment for more than 2 years. The children presented with intellectual disability,autism spectrum, developmental delay, special facial features, hydrocele of testis, cryptorchidism and testicular microlithiasis. Genetic examination report shows that the child carries a heterozygous pathological mutation of ADNP gene: ADNP (NM_015333.9) Exon3: c.56_57delTG: p.(Val19fs), which is not found in either parent.And this gene mutation site has not been reported. A total of 102 children (including this case) were found in the literature, with an average age of 6.3 years and a male to female ratio being 3:2. All children were reported to have mild to severe intellectual impairment and severe language and motor retardation. Autism spectrum disorders and characteristic facial appearance are also common. Conclusion: HVDAS is a rare autosomal dominant neurodevelopment disorder. The possibility of HVDAS should be clinically considered in children with developmental retardation, autism spectrum disorder and specific features. Gene sequencing should be performed as early as possible, and the detection of ADNP gene variation in genetic testing can make a clear diagnosis.

839 The application of part-time nurse management system in human resources allocation at pharmacy intravenous admixture service center

Objective: To explore the effectiveness of part-time nurse management system on human resources allocation at pharmacy intravenous admixture service center. Methods: A management team of parttime nurses at the pharmacy intravenous admixture service center was established with its information system developed accordingly, and an implementation plan was formulated regarding access qualifications, training strategies, quality control, drug depletion management, performance management, human resources coordination and arrangement. The average time of drug dispensing, the incidence of approximate error of drug dispensing and the satisfaction from part-time nurses, pharmacy intravenous admixture services and clinical departments were compared between the two groups. Results: After the implementation of part-time nurse management system in pharmacy intravenous admixture service center, the average time of drug dispensing was shortened from
(122.20±4.95) min to (101.06±10.91) min (P<0.001). The incidence of drug approximate disposition error decreased from 0.125‰ to 0.058‰ (P<0.05). The satisfaction from part-time nurses, pharmacy intravenous admixture services and clinical departments were significantly improved (P<0.001). Conclusion: The part-time nurse management system can reasonably allocate human resources and improve work efficiency, which helps achieve the goal of scientific and dynamic management of human resources.

855 Investigation and analysis of college students’ sense of gain from the summer social practice

Objective: To investigate what college students’ may gain from the summer social practice of “going to the countryside to promote scientific knowledge and literacy and to render medical service to farmers”,with an analysis of the influencing factors, and relevant suggestions. Methods: A questionnaire was developed after an interview with 75 college students, and a survey was made in 383 randomly selected students to analyze their practical sense of gain and its influencing factors. Results: There were 21.41% of the surveyed students who did not participate in the summer social practice. Of those who did participate in the summer social practice,68.11% believed that they gained much or very much. The proportion of students who believed that they gained little or very little was 3.32%. Among them, 49.83% of students believed that the greatest benefit from the summer social practice was reflected in spiritual aspect; 42.52% of students believed that they gained most in their ability improvement, and 7.64% of students believed that they gained new knowledge. College students’ sense of gain was mainly influenced by organizational style, subjective cognition, and attitude towards practice.Conclusion: To ensure the effectiveness of practical education, it is necessary to attach great importance to the participants’ actual sense of achievement by improving the organizational level of the university, the cognitive level of students, and the collaboration level of society.
