603 Blocking of fibroblast growth factor receptor improves liver fibrosis in mice via inhibiting hepatic stellate cell activation

Objective: To study the effect of fibroblast growth factor receptor (FGFR) inhibition on carbon tetrachloride (CCl4)-induced liver fibrosis in mice and explore its mechanism. Methods: CCl4-induced liver fibrosis mouse model was treated with FGFR inhibitor AZD4547 and the changes in liver weight, serum alanine
transaminase (ALT) and serum aspertate aminotransferase (AST) concentrations were compared between groups. Histological staining was used to analyze changes in liver collagen deposition, fibrosis, liver inflammation and activation of hepatic stellate cells (HSCs). Quantitative PCR (qPCR) was used to detect the expression of fibrosis related genes (α-SMA, Col1a1 mRNA) in liver. Tumor necrosis factor α (TNFα)-induced human hepatic stellate cell line LX-2 was used as the inflammation model to investigate the effect of AZD4547. The expression of Vimentin was observed by immunofluorescence staining and the ultrastructure were checked by transmission electron microscope. JC-1 staining was applied to detect the mitochondrial membrane potential. The expression of adiponectin mRNA and protein in cells was analyzed, and the phosphorylation levels of JNK and AMPK were detected by Western blot. Results: FGFR inhibitor significantly reduced liver fibrosis, inflammation response and hepatic stellate cell activation in CCl4-induced mouse model without affecting ALT and AST (P<0.05). Interestingly, inhibition of FGFR could increase adiponectin expression at the damaged site, which plays the role of compensatory protection (P<0.05). In addition, FGFR inhibitors not only markedly reversed morphology and ultrastructure of TNFα-induced hepatic stellate cells, but also downregulated JNK activation (P<0.05) and alleviated mitochondrial dysfunction in HSCs (P<0.05), where adiponectin and p-AMPK were increased, suggesting a negative feedback control by FGF signaling inhibition (P<0.05). Conclusion: Inhibition of FGFR signaling indicates therapeutic effect on liver fibrosis via inhibiting the inflammation and activation of HSCs.

613 Preparation and functional exploration of polydopamine modified microspheres ALG@PDA

Objective: To prepare polydopamine (PDA)-modified Calcium alginate microspheres (ALG@PDA microspheres) by microfluidic electrospray technology and dopamine (DA) self-polymerization, and to explore their properties. Methods: Calcium alginate microspheres (ALG microspheres) were prepared by microfluidic electrospray technology, and were modified by DA oxidation auto-polymerization. The particle size of microspheres was observed by the light microscope. Fourier transform infrared spectroscopy (FTIR) was used to detect the absorption peak of microspheres. The surface structure of the microspheres was observed by scanning electron microscopy. Near-infrared laser and thermal imager were used to explore the photothermal effect of microspheres. The viability of 3T3 cells treated with microsphere leaching solution was detected by the CCK-8 method and cell live/dead staining. Mouse tissues were used to test the adhesion ability of microspheres. Results: Within a certain range, the particle size of microcarriers was proportional to the concentration of precursor solution, the flow rate of precursor solution, and the inner diameter of capillary glass tube tip, but inversely proportional to the applied voltage. Based on the microfluidic electrospray technology and DA oxidation self-polymerization reaction, the injectable ALG@PDA with a particle size of about 200 μm and ALG@PDA has good biocompatibility. Thanks to the photothermal conversion ability of PDA, ALG@PDA had good photothermal effect compared with ALG alone, and ALG@PDA can be heated up to 21.5 ℃ in 3 min under NIR irradiation. Based on the plentiful catechol groups on the surface of PDA, compared with ALG alone, ALG@PDA had good adhesion ability, which could adhere to each other and to the tissue surface, thus increasing the residence time of ALG@PDA in the lesion. Conclusion: The prepared ALG@PDA microspheres have good biocompatibility, photothermal effect, and adhesion.

621 Clinical features of Escherichia coli meningitis in infants and risk factors for its complications

Objective: To analyze the clinical characteristics and risk factors for complications of Escherichia coli meningitis in infants, and to provide evidence-based reference for its diagnosis, treatment and improvement of prognosis. Methods: The clinical data of 918 children with community-acquired bacterial meningitis admitted into 52 hospitals in Zhejiang Province from January 1, 2019 to April 30, 2021 were collected. A total of 62 children with bacterial meningitis whose cerebrospinal fluid and (or) blood culture was positive for Escherichia coli were selected. The clinical characteristics, drug sensitivity of isolated strains and risk factors for complications were retrospectively analyzed. Results: Of 62 patients who were aged 1-228 days, 43 cases≤28 days (69.4%), 16 cases >28 days≤3 months (25.8%), and 3 cases >3 months (4.8%); 23 cases (37.1%) had underlying diseases. All patients had fever, with 15 cases (24.2%) having fever for more than 1 week. The initial extracranial infectious diseases were sepsis (47 cases, 75.81%), gastrointestinal infection (20 cases, 32.3%), and respiratory tract infection (14 cases, 22.6%). Complications occurred in 40 cases (64.5%), with subdural fluid/pus accumulation being the most common (19 cases, 30.6%). Univariate analysis showed that the age, first cerebrospinal fluid white blood cell count and micro total protein content of children with complications were higher than those without complications, with significant difference (P<0.05). Multivariate Logistic regression analysis showed that age >28 days (OR=5.730, 95%CI=1.216-27.001, P<0.05) and first cerebrospinal fluid micro total protein >1 271.5 mg/L (OR=5.473, 95%CI=1.473-20.329, P<0.05) were risk factors for complications in children with Escherichia coli meningitis. Escherichia coli was highly resistant to ampicillin (55.0%) and amoxicillin (55.6%), moderately resistant to cefuroxime, ceftriaxone and cefotaxime, and completely sensitive to meropenem, imipenem and ertapenem. Conclusion: Escherichia coli meningitis mainly occurs in infants younger than 3 months old, with a high incidence of complications. Age >28 days and first cerebrospinal fluid micro total protein >1 271.5 mg/L are important risk factors for complications. Clinicians should use effective antibacterial drugs and detect complications as early as possible, and implement early intervention to improve prognosis.

628 Differential value of cardiac magnetic resonance segpsychiatric Native T1 and tissue tracing in hypertrophic cardiomyopathy and hypertensive heart disease

Objective: To investigate the differential diagnostic value of segmental Native T1 and tissue tracking in hypertrophic cardiomyopathy (HCM) and hypertensive heart disease cardiomyopathy (HHD). Methods: From October 2018 to July 2022, 80 patients with HCM and 25 patients with HHD from the Fifth Affiliated Hospital of Wenzhou Medical University were included. The films and T1 mapping images of all subjects were processed by CVI42 software to obtain the conventional left ventricular function parameters, left ventricular myocardial strain parameters, Native T1 values of the whole and all segments. The cardiac magnetic resonance parameters of HCM and HHD patients were compared between groups, Univariate and multivariate Logistic regression analysis was used to screen the predictors and modeling analysis was carried out. ROC curve and area under curve (AUC) were used to evaluate the identification efficacy of parameters. Results: Clinical data showed that HHD patients were more male (P=0.048) and more smokers (P=0.032) compared with HCM patients; Compared with HHD patients, patients with HCM had higher left ventricular ejection fraction (LVEF) and more pronounced thickening of left ventricular wall thickness (LVMWT) at end-diastole, higher positive rate of late gadolinium enhancement (LGE) (P<0.001), whereas the left ventricular end diastolic volume index (LVEDVI) and the left ventricular end systolic volume index (LVESVI) were lower in HHD patients (P<0.01); The global radial strain (GRS) and global circumferential strain (GCS) of left ventricle in HCM patients were higher than those in HHD patients (P<0.05). Univariate and multivariate logistic regression analysis showed that 3D GCS (OR=0.68, 95%CI=0.48-0.95, P=0.025) and segment 8 Native T1 (OR=1.04, 95%CI=1.00-1.07, P=0.029) were predictors for HHD in HCM. The results of ROC curve showed that the AUC value of 3D GCS, segmental 8 native T1 and combined model was 0.751, 0.779 and 0.925, respectively. The sensitivity was 86.67%, 71.62% and 84.06%, and the specificity was 63.16%, 82.61% and 88.89%, respectively, with the AUC of the joint parameter model being the highest (P<0.05). Conclusion: Segmental Native T1 and tissue tracing techniques are of important value in the differential diagnosis of HCM and HHD, in which segmental 8 Native T1 and 3D GCS are predictors for the differential diagnosis of HHD in HCM.The combined model is most valuable in differential diagnosis.

637 E20 mediates the protective effect against cisplatin-induced damage to human ovarian granulosa cells by upregulating Sirt1

Objective: To investigate the protective effect of a novel spirocyclic chalcone-like compound E20 on cisplatin (CDDP)-induced damage in human ovarian granulosa cells (KGN) and to explore its preliminary molecular mechanism. Methods: In the in vitro KGN cell culture system, different concentrations of E20 were added, and the safe concentration of E20 was screened by MTT assay. KGN cells were treated with CDDP alone or in combination with different concentrations of E20 (5 μg/mL), and then changes in cell viability were detected by MTT assay. Different concentrations of CDDP alone or in combination with E20 were used to treat ovarian cancer cells ES-2, and cell viability was detected by MTT assay to calculate the half-maximal inhibitory concentration (IC50). Changes in KGN cell morphology were observed by inverted microscopy, and changes in apoptosis were detected by flow cytometry. Changes in estradiol (E2) levels in KGN cell supernatant were measured by ELISA. Changes in the expression of Sirt1 and apoptosis-related proteins Bax and Bcl-2 in KGN cells were detected by Western blotting. The silencing effect of Sirt1-siRNA transfection on KGN cells was verified by Western blot, and the effect of E20 on KGN cell viability after Sirt1 downregulation was detected by MTT assay. Results: The safe concentration of E20 for KGN cells was below 20 μmol/L. CDDP decreased the viability of KGN and ES-2 cells, while E20 could alleviate CDDP-induced damage to KGN cells without reducing the anti-proliferative ability of CDDP in ES-2 cells. CDDP changed the morphology of KGN cells and increased cell apoptosis (P<0.05), while E20 treatment restored the normal morphology of KGN cells and reduced cell apoptosis (P<0.05). After CDDP stimulation, the protein expression of Sirt1 and Bcl-2 in KGN cells decreased significantly (P<0.05), while the protein expression of Bax increased significantly (P<0.05). E20 treatment could significantly upregulate the expression of Sirt1 and Bcl-2 proteins in cells (P<0.05) and downregulate the expression of Bax protein (P<0.05). Compared with the control (NC-siRNA) group, the Sirt1 protein expression in cells decreased significantly after Sirt1-siRNA transfection (P<0.05). Moreover, after the expression of Sirt1 was silenced, the protective activity of E20 on cells almost disappeared compared with the normal control group. Conclusion: The chalcone derivative E20 can effectively improve the viability and morphology of KGN cells damaged by CDDP, enhance their anti-apoptotic ability, and may play a protective effect by activating the Sirt1 pathway. This study provides a promising new small molecule compound for chemotherapy-induced premature ovarian failure.

643 The predictive value of dual-energy CT combined with tumor markers in molecular subtypes of breast cancer

Objective: To explore the clinical value of dual-energy CT parameters combined with tumor markers in predicting molecular typing of breast cancer. Methods: A retrospective analysis was performed on 119 breast cancer patients confirmed by postoperative pathology in the Fifth Affiliated Hospital of Wenzhou Medical University from July 2019 to March 2020. All patients underwent dual-energy CT examination and tumor marker detection two weeks before surgery. Dual-energy CT parameters, including normalized iodine concentration (NIC), energy spectrum curve slope (λHU), and normalized adequate atomic number (nZeff), were measured at the post-processing workstation; and the expression levels of preoperative tumor markers, including CEA, CA125 and CA15-3 were recorded. The differences between different indicators among the four molecular subtypes were evaluated. The diagnostic efficacy of different single and combined indicators was analyzed by the ROC curve, and the area under the curve (AUC), sensitivity, and specificity were calculated. Results: The expression level of CA125 in patients with Her-2 overexpression was significantly higher than that in the other three groups (P<0.05), while the expression level of CEA in triple-negative patients was significantly lower than that in the other three groups (P<0.05). The arterial phase NIC and arterial phase λHU values of the patients were higher than those of the other three groups (P<0.05); the nZeff of triple-negative patients was higher than those of the other three groups (P<0.05). Arterial phase NIC and arterial phase λHU value were independent risk factors for the diagnosis of Her-2 overexpression type. Among them, arterial phase λHU value had the highest efficacy, with AUC, sensitivity, and specificity of 0.762, a sensitivity of 81.3%, and a specificity of 62.1%. In addition, CEA and venous phase nZeff were independent risk factors for the diagnosis of triple-negative; the venous phase nZeff had the highest efficacy, with AUC, sensitivity, and specificity of 0.709, the sensitivity of 85.7%, and specificity of 52.0%. In further analysis, the combination of arterial phase NIC, arterial phase λHU value, and CA15-3 can increase the AUC of Her-2 overexpression type to 0.843; the combination of CEA and venous phase nZeff can increase the AUC to 0.784. Conclusion: Dual-energy CT parameters combined with tumor markers have good application value in evaluating the molecular typing of breast cancer and can guide individualized treatment to a certain extent.

651 Clinical observation and nursing care of sensitized kidney transplant recipients treated with combined desensitization based on double-column protein A immunoadsorption for clearing HLA antibodies

Objective: To investigate the clearance effects of desensitization therapy on human leukocyte antigen (HLA) antibodies so as to provide a reference formulation of nursing practice on immunoadsorption therapy with double-column. Methods: Totally 48 kidney transplant recipients who received preoperative/postoperative desensitization therapy based on Protein A Immunoadsorption (PA-IA) therapy in combination with drug therapy in Shulan (Hangzhou) Hospital from July 2017 to July 2022 were enrolled, of whom 27 treated with double-column and 21 with single-column. For the patients with double-column treatment the pre-treatment mean fluorescence intensity (MFI) of 1 324 HLA antibody specificities (MFI > 2 000) and the post-treatment MFI of the corresponding antibody specificities (after 1, 4, 7, and 10 sessions) were recorded for analysis of the changes in antibody level reduction for the different antibody classes and MFI ranges. The operation procedure of double column immunoadsorption and nursing measures were recorded and summarized. The age, weight, regenerated plasma volume, treatment time and anticoagulant dosage of the two groups were compared. Results: There was no statistically significant difference in the baseline data between the two groups (P>0.05), the regenerated plasma volume of two-column group is larger than that of single-column (P<0.05). Compared with single-column therapy, double-column therapy significantly reduced extracorporeal circulation time (P<0.001) and anticoagulant dosage (P<0.001). After 10 sessions of two-column therapy, the MFI of class I antibodies decreased from 8 298.6 to 3 196.2 (reduction of 66.80%), while the MFI of class II antibodies decreased from 13 521.1 to 2 773.3 (reduction of 71.14%). The clearance effect of PA-IA therapy were was better on strongly positive (MFI > 10 000) class II antibodies than on strongly positive class I antibodies, showing a respective reduction (P=0.015) of 62.59% (25.17%, 91.04%) and 45.13% (32.70%, 73.94%). Conclusion: Double-column immunoadsorption system can remove effectively HLA antibody, significantly reduced extracorporeal circulation time and anticoagulant dosage and thus improve medical security.

657 The effect of temporomandibular joint disc injury on masticatory muscle structure in adolescents: An MRI-based study

Objective: To study the effect of temporomandibular joint disc injury on masticatory muscle structure in adolescents, and to explore the diagnostic value of MRI in temporomandibular disorder (TMD). Methods: A total of 42 patients who received MRI examination of temporomandibular joint in the Second
Affiliated Hospital ﹠ Yuying Children’s Hospital of Wenzhou Medical University from April 2019 to December 2020 were collected as research subjects. All patients underwent MRI examination of temporomandibular joint at 3.0T, and were diagnosed as unilateral disc displacement without contralateral displacement through routine sequence images. T2-mapping and 3D-CUBE T2 sequences were scanned. The joint disk T2 values were measured on the T2-mapping sequence. The cross-sectional area and the height of 3 pairs of masseters, i.e. internal pterygoid muscles, external pterygoid muscles and masseter muscles were measured by multiplane recombination on the 3D-CUBE T2 sequence images, and their relative volume was calculated. Paired t test was used to compare the cross-sectional area, height and relative volume of masticatory muscles as well as the T2 value between the injured side and the control side. Pearson correlation analysis was used to study the correlation among the cross-sectional area difference, height difference, and relative volume difference of masticatory muscles with differences between the injured and control sides. Pearson correlation was also used to analyze the correlation between the difference of the cross-sectional area, height and relative volume of the masticatory muscle on the injured side and the control side and the difference of the articular disk T2 value. Results: Compared with the control side [(435.60± 99.28)mm2], the cross section of masseter muscle on the injured side [(412.66±106.40)mm2] decreased (P<0.05), and compared with the control side [(31.77±8.83)cm3], the relative volume of masseter muscle on the injured side [(29.79±9.91)cm3] decreased (P<0.05). The T2 value on the injured side [(32.06±3.86)ms] was lower than that on the control side [(36.45±5.48)ms] (P<0.001). There was a high positive correlation between the difference in cross-sectional area and relative volume of the masseter muscle on the injured and control sides (r=0.930, P<0.001). Conclusion: The masseter muscle of the healthy side is more developed than that of the affected side in patients with TMD. It is inferred that the masseter muscle of the affected side may be an indirect imaging feature of TMD before symptoms appear. The enlarged masseter muscle on the affected side may be an indirect imaging feature of TMD.

662 Efficacy and safety of sofosbuvir/velpatasvir in the treatment of patients with genotype 3 and 6 chronic hepatitis C

Objective: To analyze the efficacy and safety of sofosbuvir/velpatasvir in the treatment of patients with genotype 3 and 6 chronic hepatitis C. Methods: All 172 patients with genotype 3 and 6 chronic hepatitis C were selected from outpatient and inpatient departments of the Third Hospital of Wenzhou Medical
University from April 2020 to December 2021. Baseline characteristics of the patients were analyzed, and indicators before and after treatment were compared, including blood routine (white blood cell count, hemoglobin, platelets), liver and kidney function, HCV RNA, and abdominal imaging. The antiviral efficacy and safety of the patients were assessed by SVR12 (sustained viral response rate). Results: A total of 172 patients with confirmed chronic hepatitis C were enrolled, predominantly male (83%); genotype 3 accounted for 84% (46% of 3a and 38% of 3b) and genotype 6 accounted for 16%. There were 133 patients complicated with intravenous drug addiction. Out of 172 patients with confirmed chronic hepatitis C, 140 patients were treated with sofosbuvir/velpatasvir, with an SVR of 98.6% (138/140) at 12 weeks. ALT and AST were significantly reduced after treatment, and the APRI score improved after treatment compared with the pre-treatment, but there was no significant difference in blood routine (white blood cell count, hemoglobin, platelets) and creatinine of patients before and after treatment. No patients discontinued or died due to adverse drug reactions. Conclusion: Of patients with chronic viral hepatitis C genotype 3 and 6 in Wenzhou, Zhejiang Province, genotype 3 has a relatively high proportion, predominantly male and a high proportion of drug-users. Treatment with sofosbuvir/velpatasvir can achieve high virological response rate with good safety and tolerability.

677 Research progress of platelet-rich plasma in orthopaedics based on CiteSpace analysis

Objective: To map and analyze, on the basis of CiteSpace software, the use of platelet-rich plasma in the field of orthopedics at home and abroad, and to summarize its research progress and predict the development trend in this field. Methods: Using CNKI, VIP, Wanfang, PubMed and Web of Science databases as data sources, the relevant literature on platelet-rich plasma in the field of orthopedics from the date of database construction to November 1, 2022 was screened, visualized and analyzed by CiteSpace software and discussed in terms of annual publication volume, authors and keywords. Results: A collection of 1 089 papers related to platelet-rich plasma and orthopedics (core journals) from three major domestic databases were screened, and 2 568 and 1 645 papers were obtained from PubMed database and Web of Science core collection, respectively. The results suggested that the number of domestic and international publications had been increasing year by year, with the research focus mainly on osteoarthritis, bone tissue engineering, tendinopathy, and bone defects. Conclusion: The further application of platelet-rich plasma in the future should focus on the direction of “discussion on the biological molecular mechanism of treatment of diseases”, “establishment of injection norms and standards”, “improvement of clinical efficacy of osteoarthritis” and “combination with bone tissue engineering”.

684 Research development on the prognostic value of oncotype DX and Magee equation in breast cancer

Objective: The latest global cancer statistics showed that breast cancer has become the top malignancy in female worldwide. With the rapid development of molecular biology, the standard medication strategy for breast cancer is the major component of promoting individualized precision therapy in Western countries. Oncotype DX is a well-established multigene genomic assay, approved by the Food and Drug Administration (FDA) to predict the risk of cancer recurrence and metastasis, and guide proper chemotherapy in patients with estrogen receptor (ER)-positive/human epidermal growth factor receptor 2 (HER2)-negative early invasive breast cancers. Subsequently, based on currently available pathologic parameters and breast cancer biomarkers, Magee equation as a novel and cost-effective algorithm, which is proposed to simulate Oncotype DX recurrence score, has been commonly applied in clinical administration of breast cancer. This review summarizes the predictive value of Oncotype DX method and Magee equation for the breast cancer prognosis in patients, which might provide a suitable reference for clinical management.
