431 The effect of Qingxin Kaiqiao Formula on neuroinflammation in APP/PS1 double transgenic mice based on NLRP3 inflammasome

Objective: To investigate the effect of the Qingxin Kaiqiao Formula on neuroinflammation and cognitive performance in mice with Alzheimer’s disease (AD) model and to explore its association with NLRP3 inflammasome-related pathways. Methods: APP/PS1 double transgenic mice were randomly divided as model group, Aricept treatment group (1.67 mg·kg-1·d-1), and Qingxin Kaiqiao Formula treatment group (low dose group: 4.75 mg·kg-1·d-1; middle dose group: 9.5 mg·kg-1·d-1; high dose group: 19 mg·kg-1·d-1), with ten mice in each group. SPF-grade male C57BL/6J mice served as the control group. The Morris water maze experiment was performed to test the ability of directional navigation and spatial perception; HE and Nissl staining were performed to determine changes in the number of cells and Nissl bodies in the hippocampal region; Western blot and real-time fluorescence quantitative polymerase chain reaction (RT-qPCR) were used to determine the expression of NLRP3, Caspase-1, Aβ, IL-1β, and IL-6. Results: Compared with the control group, mice in the model group showed significantly increased mean escape latency (P<0.01), significantly decreased number of passage through the plateau shorter residence time in the target quadrant where the plateau was located (P<0.01),decreased number of neurons in the hippocampal region, irregularity, decreased number of Nissl bodies , lighter staining color, and obviously increased expression of NLRP3, caspase-1, Aβ, IL-1β and IL-6 (P<0.01). Compared with the model group, both the Aricept treatment group and the Qingxin Kaiqiao Fang-treated group showed visibly shorter escape latency, significantly greater number of crossing platforms longer residence time in the quadrant where the plateau was located (P<0.05 or P<0.01); and the neuronal cells in the treated group were more closely arranged and more intact morphologically, apparently higher number of Nissl bodies deeper color,and significantly reduced NLRP3, Caspase-1, Aβ, IL-1β and IL-6 expression (P<0.05 or P<0.01). Conclusion:Qingxin Kaiqiao Formula may improve Aβ pathological changes and nimpairment in learning and memory in APP/PS1 mice by reducing neuroinflammation through inhibition of the NLRP3 inflammatory pathway.

440 The effect of metformin on glioma cell metabolism under normoxic and hypoxic conditions

Objective: To explore the effect of metformin on the metabolism of glioma U251 cells under normoxic and hypoxic conditions. Methods: Under the conditions of normox (21% O2) and hypoxia (1% O2),metformin at 25 mmol/L was used to detect the cell viability of U251 cells by MTT method; the ability of cells to form clones by plate cloning method; the DNA proliferation changes of cells were detected by EdU method;the cycle changes of cells were detected by cell cycle kits; the metabolic changes of cells were detected by ultra-high performance liquid chroma-high-resolution mass spectrometry untargeted metabolism detection. The content of ATP in cells and the content of extracellular lactic acid were detected by ATP and lactate kits, and the relative expression of glucose transporter (GLUT1) and proto-oncogene (c-Myc) proteins were detected by Western blot method, respectively. Results: Compared with the control group, the viability, cell formation cloning rate, cell proliferation rate and cell metabolite content were reduced (P<0.05) after 25 mmol/L metformin treated with glioma U251 cells for 48 h under normal oxygen and hypoxic conditions. Under normal oxygen conditions,metformin increased the ATP content and extracellular lactate content of brain glioma U251 cells (P<0.05).The relative expression of GLUT1 and c-Myc proteins in U251 cells after metformin treatment under hypoxic conditions decreased significantly (P<0.05). Conclusion: Metformin inhibits the proliferation of glioma U251 cells and the ability to form clones by downregulating the metabolism of glioma U251 cells.

450 Water treadmill training promotes the polar expression of aquaporin 4 and reduces tissue edema after spinal cord injury in rats

Objective: To explore the mechanism of water treadmill training (TT) in the spinal cord edema after spinal cord injury (SCI) in rats. Methods: Rats were randomly divided into Sham operation no treatment group (Sham-Con), Sham operation exercise group (Sham-TT), spinal cord injury group (SCI-Con) and spinal cord injury exercise group (SCI-TT). The rats in the exercise group were treated with TT intervention, and the rats in each group were evaluated with Bssso Beattie Bresnahan (BBB) Test on the 1st, 3rd, 7th and 14th day after operation. The moisture content of the spinal cord was detected by wet and dry weight method, the bloodspinal barrier permeability was detected by Evans bule (EB) staining, and the expressions of aquaporin 4 (AQP4) and glial fibrillary acid protein (GFAP) in the surrounding tissues of the spinal cord were detected by Western blot. The co-labeling of platelet-endothelial cell adhesion molecule (CD31)/AQP4/GFAP/DAPI was detected by immunofluorescence three-label staining, and the presence of AQP4 on endothelial cells (ECs) in each group was detected by immunoelectron microscopy. Results: There was no statistical significance (P>0.05) between Sham-Con group and Sham-TT group in water content in spinal cord, EB content in tissue, polar expression of AQP4, total protein expression and BBB scores. After SCI, the water content of spinal cord in SCI-Con group was significantly increased (P<0.05), the content of EB in tissue increased (P<0.05), the expression of GFAP protein up-regulated (P<0.05), and the polarity expression of AQP4 increased. However, there was no statistical
significance in the expression level of total AQP4 protein compared with Sham-Con (P>0.05). TT could reduce tissue edema after SCI (P<0.05), decrease the permeability of blood-spinal barrier (P<0.05), and further increase the polarity expression of AQP4 (P<0.05), and promote the recovery of motor function in rats at 7 d and 14 d (P<0.05). But there was no significant effect on the total protein expression of AQP4 (P>0.05). Conclusion: TT can reduce tissue edema after SCI in rats, and its protective mechanism may be related to the promotion of the polarity expression of AQP4.

458 The effect of basic fibroblast growth factor on liver injury in diabetic mice: A metabonomics analysis

Objective: To observe the effect of basic fibroblast growth factor (bFGF) on diabetic liver injury through remodeling metabolism by 1H nuclear magnetic resonance (1H NMR)-based metabonomics. Methods:C57BL/6J mice acted as control group (wt group). The db/db mice were randomly divided into diabetic liver injury group (db/db group) and bFGF-treated group (bFGF group). The liver tissues were collected from mice after 10 weeks of bFGF treatment. Morphology of liver tissue was examined by hematoxylin-eosin (HE) staining.The characteristic alterations in liver metabolites were explored by the projection to latent structure discriminant analysis (PLS-DA) based on the 1H NMR spectra. Results: HE staining results showed that the structure of hepatic lobules and hepatic sinuses in wt group was clear, and the steatosis of hepatic cells in db/db group was obvious. After bFGF intervention, lipid droplets and inflammatory cells in bFGF group were significantly reduced, and cell necrosis damage was relieved. PLS-DA results showed that there was significant difference in the metabolic profile of liver among the three groups of mice. Quantitative analysis of metabolites showed that,compared with age-matched wt group mice, the levels of leucine, isoleucine, valine, lactate, succinate, creatinine,glycine, guanosine triphosphate and phenylalanine were significantly lower in db/db group mice but they were significantly increased after bFGF treatment (P<0.05). Conclusion: The effect of bFGF on diabetic liver injury could be mediated by remodeling metabolic phenotype, mainly involving energy metabolism, amino acid metabolism and nucleotide metabolism.

465 A nomogram model for short-term mortality risk in elderly patients with sepsis

Objective: To explore the risk factors affecting the prognosis of death in elderly patients with sepsis, and to construct a nomogram to predict the risk of short-term in-hospital death in elderly patients with sepsis. Methods: The clinical data of elderly sepsis patients from the Second Affiliated Hospital of Wenzhou Medical University and a large multi-center intensive care database in the United States were collected, and the relevant clinical data of patients, such as vital signs, laboratory tests and clinical outcomes, were retrospectively analyzed. Univariate and multivariate Logistic regression analysis was used to screen out risk factors and a nomogram model was constructed to predict death of elderly patients with sepsis within 30 days. Results: The lowest blood oxygen saturation on the first day of admission, the lowest mean arterial pressure, the highest heart rate, the lowest body temperature, the highest respiratory rate, the lowest blood albumin, the highest blood bilirubin, lactate, and brain natriuretic peptide (BNP), mechanical Ventilation, age, liver failure and history of metastatic malignant tumors were independent risk factors for elderly patients with sepsis. The area under the ROC curve (AUC) of nomogram was 0.777 (95%CI=0.743-0.810), and the AUC for predicting the risk of death by external validation was 0.747 (95%CI=0.733-0.762), with both the internal and external calibration curves close to the standard curve. Conclusion: This nomogram has good differentiation and calibration and can accurately predict the death prognosis of elderly patients with sepsis, which provides a basis for clinicians to evaluate the prognosis of elderly patients with sepsis.

473 Influencing factors of PACU hypoxemia after thoracoscopic radical resection of lung cancer: A study based on the decision tree model

Objective: To establish a decision tree model that can predict hypoxemia in postanesthesia care unit of lung cancer patients undergoing thoracoscopic radical resection, and to explore predictive value of decision tree model. Methods: A retrospective study was conducted. 5 560 patients who underwent thoracoscopic radical resection of lung cancer in the Anesthesiology Department of the First Affiliated Hospital of Wenzhou Medical University were selected as the study objects from January 2017 to June 2022. According to whether hypoxemia was complicated in the PACU, the patients were divided into two groups: non-hypoxemia group and hypoxemia group. A prediction model was established and validated based on the decision algorithm, and receiver operator characteristic curve, calibration curve, and decision curve were draw, respectively. Results: Among 5 560 patients, 1 653 patients developed hypoxemia in the PACU. The results of decision tree model analysis based on training sets showed that age, sex, intraoperative auxiliary use of dexmedetomidine, anesthesiologist’s shift, time of surgery, and intraoperative auxiliary use of diazosine were related to PACU hypoxemia after thoracoscopic radical resection of lung cancer (P<0.05). Among them, ≥64 years old, female, the shift of anesthesiologist, intraoperative auxiliary use of diazosine, dexmedetomidine, and non-morning surgery were the promoting factors for postoperative PACU hypoxemia, while male, intraoperative auxiliary use of flurbiprofen axetil, and morning surgery were the protective factors for postoperative PACU hypoxemia (P<0.05). Decision tree model was validated by validation set data. The results showed that the AUC value was 0.916, and the predicted value was in good agreement with the measured value. The model had good net benefits. Conclusion: The prediction model for PACU hypoxemia in patients undergoing thoracoscopic radical resection of lung cancer based
on decision tree algorithm has good predictive ability, high accuracy, good net benefit, and intuitive structure,which is helpful for PACU medical staff to identify and prevent hypoxemia in advance.

482 Partial coagulation and biochemical indexes in patients with Alzheimer’s disease: Charge analysis and its significance

Objective: To explore the useful blood indexes in the diagnosis and treatment of Alzheimer’s disease (AD) by analyzing the levels of some blood coagulation and biochemical indicators in AD. Methods: Of all 148 AD patients admitted to the Second Affiliated Hospital of Wenzhou Medical University from January 2019 to December 2022 were included as AD group, while 100 healthy subjects during the same period were assigned as control group. Blood levels of coagulation factor XII activity (FXII:C), prothrombin time (PT), activated partial thromboplastin time (APTT), thrombin time (TT), fibrinogen (FIB), D-dimer (DD), triglyceride (TG),total cholesterol (CHOL), HDL-cholesterol (HDL-C), LDL-cholesterol (LDL-C), glucose (GLU), glycated hemoglobin A1c (HbA1c), lipoprotein (LPA), alkaline phosphatase (ALP) and C-reacitve protein (CRP) were analyzed by STAGO and Siemens detection systems. The cognitive ability was assessed by mini-mental state
examination and Montreal cognitive assessment (MoCA), life ability was assessed by the Barthel index of daily living, and dementia degree was assessed by clinical dementia rating scale (CDR). Independent sample t test or Kruskal-Wallis test were used to compare the differences among all groups. Spearman correlation analysis was used for correlation analysis. Logistic regression analysis was used for regression analysis. Results: The levels of FXII:C and CRP in AD group were significantly higher than those in control group (P<0.01), while the levels of TT, CHOL, HDL, LDL and ALP in AD group were significantly lower than those in control group (P<0.05). Age,education level, ALP, CRP, FIB, DD were positively correlated with the degree of dementia (r=0.357, 0.289, 0.308,0.409, 0.416, 0.290, P<0.01). Regression analysis showed that FXII:C was a independent risk factor for AD (OR=1.102, P<0.01), and TT and CHOL were independent protective factors for AD (OR=0.296, 0.011, P<0.05).MoCA score+FXII:C+CHOL+TT combined screening had the largest AUC (95%CI=0.956-1.000), while MoCA score+FXII:C+CHOL combined screening the maximum sensitivity of 96.2% (AUC=0.963, 95%CI=0.918-1.000). Conclusion: Cognitive assessment combined with FXII:C, CHOL and TT can improve the sensitivity and specificity of AD screening. Reducing the activity of FXII and avoiding low cholesterol level can reduce the prevalence of AD. CRP and FIB can be used to monitor the severity of AD disease.

488 Investigation on MNS blood group antigen and gene frequency of 122 cases of She ethnic minority population in Cangnan area of Wenzhou

Objective: To understand the distribution of antigen and gene frequency of MNS blood group system in She ethnic minority population in Cangnan area of Wenzhou. Methods: The MNS phenotype from 122 She people’s blood samples in Cangnan between October 2021 and April 2022 were identified by serological method, and MNS genotypes were detected by fluorescence PCR-SSP method. Results: The results of MNS blood group serotype and genotype were consistent in She population in Cangnan area of Wenzhou. The gene frequency of the MNS blood group system were M=0.566, N=0.434, S=0.037, s=0.963. The frequencies of genotypes MM, MN, and NN were 0.254, 0.123, and 0.623, and the frequencies of genotypes ss and Ss were 0.926 and 0.074,respectively. No SS genotype was detected. Conclusion: The MNS gene frequency of She population in Cangnan area of Wenzhou has its own distribution frequency characteristics, similar to yet different from the reported Tibetans or Han populations.

493 Clinical application of computer image navigation in renal duplication

Objective: To explore the clinical application of 3D reconstruction and 3D printing technology in renal and ureteral duplication. Methods: A total of 23 patients of congenital renal and ureteral duplication who were operated and had undergone computer tomography (CT) in the Second Affiliated Hospital & Yuying Children’s Hospital of Wenzhou Medical University from July 2015 to January 2022 were screened, and their clinical history and imaging data were collected. The target tissues were integrated and extracted by threshold segmentation method in Amira. Then 3D reconstruction and 3D model printing were completed, of which 15 cases were performed preoperatively and 8 cases postoperatively. Out of 15 preoperative reconstruction, 8 cases of heminephrectomy were selected as the research subjects, and the cases of heminephrectomy without 3D reconstruction at the same time were randomly selected as the control group. The operation time and bleeding volume were compared between the preoperative reconstruction group and the non-reconstruction group. Results:Relevant 3D reconstructed images of 23 cases were obtained using Amira software, and the 3D model was successfully printed. The 3D reconstructions performed preoperatively or postoperatively were consistent with the actual anatomy and did not affect the results. The comparative study showed that both the 3D reconstructed image and the 3D model could fully display the anatomical details of the kidney, ureter and the surrounding blood vessels, and more intuitively display the parts of ureteral duplication compared with the traditional two-dimensional imaging manifestation. The average operation time for heminephrectomy was 120.0(114.0,144.0)min in the reconstruction group, and 200.0(162.0, 221.3)min in the non-reconstruction group, showing significant difference between the two groups (P=0.002). The intraoperative blood loss in the reconstruction group was 6.0(5.0, 9.3)mL, with statistical difference compared with 15.0(12.5,16.8)mL in the non-reconstruction group (P=0.001). Conclusion: The 3D images obtained by Amira combined with the 3D printed model, as a convenient and feasible clinical aid, can help surgeons better evaluate renal duplication through the integration of computer image technology.

508 The current situation of the editorial board of medical journals: an analysis and optimization strategies

The editorial board is an important part of the core competitiveness of sci-tech journals, and the guardian of academic quality. The development of sci-tech journals highly depends on an efficient and responsible editorial board team. Medical journals are an important part of sci-tech journals. Take the journals of hepatobiliary and pancreatic surgery from Chinese S&T Journals Citation Reports as an example, this paper is to analyze the current situation of the editorial board of medical journals. We propose four optimization strategies,including optimizing the structure of the editorial board, clarifying the division of functions, dynamically managing the editorial board, and maintaining close contact between the editorial department and members, in order to enhance the role of the editorial board in journal management and the development of journals.

513 Collaborative governance of prevention and control of myopia in children and adolescents from the perspective of healthy China: A case study of Wenzhou

The prevention and control of myopia among children and adolescents is an important component of the implementation of the “Healthy China” strategy and a crucial livelihood project for numerous families. In recent years, the incidence of myopia among children and adolescents in China has been on the rise year by year,which has attracted great attention from the leaders of the Party and the country. They have emphasized that it is a major issue concerning the future of the nation and the people, and that it must be given due importance without being left unchecked. Myopia prevention and control is not just a medical issue but also a complex systemic project that requires exploration through diversified and multi-stakeholder collaborative governance. The “Wenzhou model” of myopia prevention and control has transitioned from a single entity to a collaborative governance involving multiple stakeholders, including the government, schools, families, medical research institutions, and children and adolescents. This model has adopted a series of scientific and effective prevention and control measures, forming a close-looped governance pattern with multiple stakeholders assuming their respective responsibilities and engaging in positive interactions. It has proven to be effective and has been replicated and promoted in many regions throughout the province and even nationwide, showcasing significant theoretical exploration and practical innovation.
