861 The intervention effect of baicalein on colitis in mice through TLR4/NF-κB signaling pathway

Objective: To investigate the effect of baicalein on experimental colitis induced by dextran sodium sulfate (DSS) through TLR4/NF-κB signaling pathway. Methods: Male mice of C57BL/6 strain were randomly divided into 5 groups: control group, colitis group, baicalein low-dose (BA-L) treatment group [10 mg/(kg·d)], baicalein high-dose (BA-H) treatment group [25 mg/(kg·d)] and 5-amino-salicylic acid (5-ASA) treatment group, with 12 mice in each group. Except for the control group, the other mice were given 3.5% DSS solution for 7 days to construct the experimental colitis model. Different doses of baicalein or 5-ASA were administered 7 days before DSS modeling and 7 days after DSS modeling, and the treatment lasted for 14 days. The average body weight, disease activity index (DAI) and colon histological activity degree score (HAI) were compared among all groups. The mRNA levels of IL-1β, TNF-α, IFN-γ, TLR4, NF-κB P65 and MyD88 in colon tissues of mice in each group were detected by real-time fluorescence quantitative PCR. The nucleation rate of NF-κB P65 in intestinal epithelial cells was determined by immunofluorescence. Results: Compared with the control group, mice in the colitis group developed severe colitis, which was manifested by decreased body weight, shortened total colon length, and significantly increased DAI and HAI (all P<0.01). And compared with the colitis group, the colitis of mice was significantly improved in BA-L group, BA-H group and 5-ASA group (all P<0.05). Besides, the mRNA expression levels of IFN-γ, TNF-α, IL-1β, MyD88, NF-κB p65 and TLR4 in colitis group were significantly higher than those in control group (all P<0.01). While the mRNA expression levels above in Ba-L group, BA-H group and 5-ASA group were significantly lower than those in the colitis group (all P<0.05). In addition, the nucleation rate of NF-κB p65 in intestinal epithelial cells of the colitis group was significantly higher than that of the control group (all P<0.001). While the nucleation rate of NF-κB P65 in Ba-L group, Ba-H group and 5-ASA group was significantly lower than that in the colitis group (all P<0.001).Conclusion: Baicalein may inhibit DSS-induced colon inflammation through TLR4/NF-κB signaling pathway in a dose-dependent manner.

868 Downregulation of acetyl-CoA synthase 2 inhibits non-small cell lung cancer cell proliferation by inducing autophagy

Objective: To investigate the expression of acetyl-CoA synthase 2 (ACSS2) in non-small cell lung cancer (NSCLC) cells and its regulatory mechanism of cell proliferation and invasion. Methods: Western blot was used to detect and analyze the difference of ACSS2 expression in NSCLC cell lines A549 and H1299 and human fibroblast cell HFF-1; ACSS2 knockdown (ACSS2 KD group) and control cell lines (NC group) were constructed in NSCLC cell lines using lentivirus. The effect of ACSS2 on the proliferation of NSCLC cells was analyzed by MTT assay and cell clone formation assay. The effect of ACSS2 on the migration and invasion ability of NSCLC cells were analyzed by scratch assay and transwell assay. The effect of ACSS2 knockdown on autophagy was analyzed. Western blot and immunofluorescence experiments. Results: The protein level of ACSS2 in NSCLC cells was significantly increased compared with human fibroblast HFF-1 (P<0.05). Knockdown of ACSS2 significantly reduced cell proliferation and cell viability. ACSS2 KD group also significantly reduced the migratory and invasive abilities of NSCLC cells (both P<0.01). Western blot and immunofluorescence experiments showed that knockdown of ACSS2 significantly increased the autophagy activity and the number of autophagosomes (P<0.01). However, knockdown of ACSS2 hardly affected the protein expression of caspase3 and caspase8 (P>0.05). A549 cells were co-transfected with ACSS2-shRNA+Beclin1/Atg7-shRNA and the results showed that the cell proliferation ability of the ACSS2-shRNA+Beclin1/Atg7-shRNA group was enhanced and the number of clones increased significantly compared with the ACSS2 KD group. Conclusion: ACSS2 is highly expressed in NSCLC cells, which promotes cell proliferation and invasion. Down-regulation of ACSS2 can induce autophagic cell death in NSCLC cells and inhibit cell proliferation.

876 Risk factors of lymphovascular invasion in T1 breast cancer

Objective: To investigate the risk factors for lymphovascular invasion (LVI) in patients with breast invasive ductal carcinoma (IDC) stage T1. Methods: The clinic-pathological data and preoperative magnetic resonance images of T1 IDC patients were retrospectively collected from January 2017 to June 2022 in the First Affiliated Hospital of Wenzhou Medical University. According to the LVI results, they were divided into two groups. Tumor-related breast edema was classified into 4 grades according to the Breast Edema Score (BES) on T2-weighted images (T2WI). To simplify it, BES2 was classified as slight edema, and BES3 and BES4 were combined as moderate to severe edema. Univariate analysis and binary logistic regression model were used for statistical analysis to determine whether the characteristics of IDC were associated with LVI. Results: A total of 307 patients were included in this study, of whom 262 were LVI negative and 45 were LVI positive. Compared with low histological grade and negative lymph node metastasis, high histological grade (OR=2.59, 95%CI=1.19-5.61, P=0.016) and positive lymph node metastasis (OR=4.74, 95%CI=2.39-9.43, P<0.001) was more likely to be LVI positive. Compared with no edema, slight edema (OR=3.80, 95%CI=1.78-8.14, P=0.001) and moderate to severe edema (OR=5.54, 95%CI=2.25-13.60, P<0.001) had an increased probability of LVI positive, but there was no significant difference between groups (P>0.05). Conclusion: Breast edema, lymph node status and histological grade are independent risk factors for LVI in T1 breast IDC patients.

882 Nomogram for prediction of lymphovascular invasion in breast cancer based on DCE-MRI radiomics and conventional MRI features

Objective: To develop a nomogram based on arterial contrast-enhanced magnetic resonance imaging (DCE-MRI) radiomics and conventional MRI features for preoperative prediction of lymphovascular invasion (LVI) status in patients with invasive breast cancer (IBC). Methods: A retrospective analysis was made on 300 patients with IBC confirmed by postoperative pathology between July 2016 and May 2021, who were randomly divided into the training (n=238) and validation group (n=62) according to ratio of 8:2. Lesion areas in second-stage DCE-MRI images of all patients were manually segmented and radiomics features were extracted.
Variance threshold, select k best, and LASSO regression were used to screen radiomics features and calculate the radiomics score (rad-score). Logistic regression analysis was used to screen conventional MRI features to establish conventional feature model, while a joint prediction model was built based on radiomics and independent risk factors in conventional MRI features, and a corresponding nomogram was drawn. The performance of the model was evaluated using receiver operating characteristic (ROC) and calibration curves, and the clinical value of the model was assessed using decision curve analysis. Results: A total of 1409 radiomics features were extracted,and 15 radiomics features were screened to correlate with the LVI status of breast cancer, and were involved in the calculation of rad-score values. Among all conventional MRI features, the largest tumor diameter (OR=1.743,P<0.001) and burr sign (OR=6.304, P<0.001) were independent risk factors for LVI positive. In the training and validation group, the area under the ROC curves (AUCs) of the radiomics model was 0.831 and 0.811,respectively; the AUCs of the conventional feature model was 0.779 and 0.770, respectively; and the AUCs of the joint prediction model was improved to 0.889 (95%CI=0.844-0.934) and 0.856 (95%CI=0.759-0.952). The calibration curve showed that the predicted value of the nomogram was in good agreement with the actual value, and the decision curve showed that the nomogram had a higher clinical value. Conclusion: The constructed nomogram based on DCE-MRI radiomics and conventional MRI features has good application value for preoperative prediction of LVI status in IBC patients, and can provide reference for clinical treatment.

889 Benefit finding and its influencing factors in caregivers of hematological tumor patients

Objective: To explore the current status of benefit finding and its influencing factors among caregivers of patients with hematologic malignancies. Methods: A total of 100 caregivers of patients suffering from hematologic malignancies were selected as the research subjects using the convenience sampling method.A general information questionnaire, benefit finding scale, and perceived social support scale were performed to analyze the benefit finding and its influencing factors. Results: The total score of benefit finding for caregivers of patients with hematologic malignancies was (71.80±16.22), and the disease benefit of caregivers was positively correlated with social support and three dimensions (P<0.001). The results of multiple linear regression analysis showed that physical and mental status and family support were the main influencing factors of disease benefit of caregivers of hematologic malignancies patients (P<0.05), which explained 61.8% of the total variation of
disease benefit of caregivers of hematologic malignancies patients. Conclusion: The benefit finding in caregivers of patients with hematologic malignancies is at a moderate level, and enhancing the social support of caregivers could improve their benefit finding.

894 Role of elavl1a in regulating growth and development in zebrafish

Objective: To establish the generation of elavl1a-/- mutant zebrafish with an aim to explore the effect of elavl1a gene on zebrafish survival, growth and development. Methods: We generated elavl1a F0 generation mutant zebrafish by CRISPR-Cas9 technology, then obtained F1 generation zebrafish by crossing with wild-type zebrafish. After genotyping, heterozygous F1 zebrafish was self-bred to produce elavl1a-/- homozygous mutant zebrafish. Finally, gene sequencing, Western blot and the relative quantitative statistics and morphological analysis were performed. Results: Using gene sequencing, we found that the elavl1a-/- mutant zebrafish had five
nucleotides missing in the exon 2, which produced frame-shift mutation, leading to appearance of early stop codon.The elavl1a-/- mutant zebrafish could only encode partial peptide, which lacked elval1a protein expression by WB. Moreover, the survival rate of elavl1a-/- mutant zebrafish was greatly reduced, which did not abide by
Mendelian inheritance, and the zebrafish growth and development was delayed. Conclusion: The elavl1a-/- mutant zebrafish was successfully generated, which provides an insight that the elavl1a gene plays an important role in zebrafish survival and development.

899 Analysis of phenotypic drug sensitivity of rifampicin-resistant Mycobacterium tuberculosis detected by Xpert MTB/RIF

Objective: To analyze the results of the phenotypic drug sensitivity test of rifampicin-resistant tuberculosis patients diagnosed by Xpert MTB/RIF (Xpert) assay to provide a scientific basis for the treatment of drug-resistant tuberculosis. Methods: Tuberculosis patients who were treated in the Infection Department of Wenzhou Central Hospital between January 2015 and December 2020 and showed rifampicin resistance in sputum or alveolar lavage fluid Xpert test were selected, and the results of phenotypic drug sensitivity of Mycobacterium tuberculosis were retrospectively analyzed. Results: The result of the first-line anti-tuberculosis drug susceptibility test was obtained from 570 rifampicin-resistant pulmonary tuberculosis diagnosed by Xpert,and the order of the drug resistance rate was rifampicin (90.18%) > isoniazid (88.60%) > streptomycin (56.32%)> ethambutol (29.47%); the resistance rates to isoniazid, rifampicin, and ethambutol in re-treated patients
(94.50%, 94.50%, and 35.78%, respectively) were significantly higher than those in the new patients (84.94%,87.50%, and 25.57%, respectively). There were 24 strains sensitive to first-line anti-tuberculosis drugs, and 46,156, 226 and 118 strains resistant to 1, 2, 3, and 4 drugs, respectively. There were 476 patients (278 new patients
and 198 re-treated patients) diagnosed with multidrug-resistant tuberculosis (MDR-TB), with the proportion of MDR-TB significantly higher in re-treated patients that in the new patients (90.83% vs. 78.98%, χ2=13.723,P<0.001). The results of second-line antituberculosis drug sensitivity test obtained from 302 patients showed
the order of resistance rate was ofloxacin (35.76%) = amikacin (35.76%) > kanamycin > kanamycin (13.25%) =capreomycin (13.25%), with the resistance rate to ofloxacin in new patients significantly lower than in re-treated patients (26.46% vs. 51.33%, χ2=19.044, P<0.05); 128 strains were fully susceptible to second-line antituberculosis drugs, and 111, 23, 21 and 19 strains were resistant to 1, 2, 3 and 4 drugs, respectively. There were 50 patients (25 new patients and 25 re-treated patients) diagnosed with extensively drug-resistant tuberculosis (XDR-TB), with a higher proportion of re-treated patients with XDR-TB (22.12% vs. 13.23%, χ2=4.051, P=0.044). Conclusion:Resistance to isoniazid, ofloxacin, and amikacin is common in rifampicin-resistant patients diagnosed by Xpert. Re-treated patients have more severe drug-resistance than new patients. The drug sensitivity results for first- and second-line anti-tuberculosis drugs should be obtained as early as possible to guide clinical use.

903 Intervention effect of written expression on reproductive stress and negative emotions in female patients with recurrent implantation failure

Objective: To explore the effect of written expression on reproductive stress and negative emotions in female patients with recurrent embryo transfer failure (RIF). Methods: A total of 213 female patients with RIF who were for treatment in Reproductive Medicine Center of the First Affiliated Hospital of Wenzhou Medical University from January 2021 to April 2022 were included. They were allocated into the observation group (n=105) and the control group (n=108) according to the odd and even number of medical record number.The control group received routine assisted reproductive health education, while the observation group was given additional written expression intervention. The anxiety, reproductive stress and patient satisfaction were compared between the two groups before and after intervention. Assessment of the correlation between the observed scores was made and the factors associated with patient satisfaction were analyzed. Results: There were no significant differences in age, years of infertility, educational level, number of embryo transfer between the two groups, and the scores of self-rating anxiety scale (SAS) and fertility problem inventory (FPI) in the observation group were significantly lower than the control group (P<0.05) while the patient satisfaction was significantly higher in observation group (P<0.05). The degree of anxiety that changed before and after intervention was positively correlated with the change degree of reproductive stress in both groups (r=0.192, P=0.005). The SAS score changes (OR=0.895, 95%CI=0.821-0.975, P=0.011) and written expression intervention (OR=0.201,95%CI=0.041-0.983, P=0.048) had statistically significant impact on patient satisfaction. Conclusion: Written expression can reduce reproductive stress and improve patient satisfaction of female patients with RIF by releasing negative emotions such as anxiety and depression.

909 The effect of high cycle dynamic loading on the bending strength and its reliability of dental zirconia ceramics

Objective: To investigate the effect of high cycle dynamic loading on the bending strength and its reliability of dental zirconia ceramics. Methods: A total of 30 dental zirconia ceramic specimens were randomly divided into 2 groups (n=15), with the experimental group subjected to dynamic cyclic loading for 3 million times and the control group not treated. The three-point bending strength of all the samples was tested and the results were studied by Weibull and statistical analysis. The fractured surface morphology of 2 specimens randomly selected from each group was observed under the scanning electron microscope. Results: The bending strength of the control and the experimental group was (1 051.3±98.8) MPa and (890.2±128.2) MPa respectively (P<0.05), with the control group was significantly higher than the experimental group. The Weibull modulus of bending strength was 12.29 and 7.77 respectively, and the characteristic strength was 1 095.4 MPa and 946.5 MPa
respectively. A large number of closed holes were found in the fracture surface of the experimental group, which confirmed the crack propagation in the specimens. Conclusion: Fatigue cracks can be produced by high cyclic dynamic loads in dental zirconia ceramics, which reduces the bending strength and reliability while increases
their fracture risk.

914 Application of personalized 10-second mass injection combined with the contrast agent dual-flow technique in synchronous imaging of axillary artery and venous CT angiography

Objective: To investigate the effect of personalized 10-second mass injection combined with contrast agent dual-flow technique on image quality and radiation dose of CT angiography (CTA) synchronous imaging in left axillary vein and artery. Methods: Eighty-three patients who underwent CTA synchronous imaging examination of left axillary artery and vein were included in this study. They were randomly divided into group A and group B according to different contrast agent injection methods. In group A, contrast agent was injected through the right elbow vein and diluent agent was injected into the left elbow vein, while in group B unilateral cubital vein multi-stage dual-flow injection was given, with personalized injection velocity and tube voltage were set according to their body weight. Contrast agent was injected through the left cubital vein for 10 s, and then the contrast agent was diluted by double-flow technique for 15 s. The dosage of contrast agent, radiation dose,objective evaluation indexes [including CT value of axillary artery and vein, signal to noise ratio (SNR), contrast noise ratio (CNR)] and subjective score were compared between the two groups. Results: The dosage of contrast agent in group B was (28.81±5.56) mL, lower than that in group A (60 mL) (t=29.620, P<0.05). The effective radiation dose of group B was significantly lower than that of group A [(2.39±1.12) mSv vs. (3.88±0.48) mSv,t=8.330, P<0.05). There was no significant difference in subjective scores between the two groups (P>0.05). The CT values of axillary artery in group A and group B were (376.35±72.49) Hu and (348.35±67.65) Hu, respectively (P>0.05). The SNR and CNR of axillary artery were statistically different between group A and group B (39.19±14.24 vs. 25.51±13.65, t=4.391, P<0.05 for SNR; and 29.59±11.73 vs. 19.34±10.95, t=4.056, P<0.05 for CNR).The CT value of axillary vein in group B [(690.70±105.24) Hu] was significantly higher than that in group A [(490.60±95.71) Hu], and the difference was statistically significant (t=-8.783, P<0.05). The SNR and CNR of axillary vein in group A and group B were not significantly different (P>0.05). Conclusion: Synchronous axillary artery and vein imaging can be achieved with high quality by using 10-second mass injection combined with dual-flow contrast agent technology. Personalized contrast agent flow rate and tube voltage settings not only effectively ensure the image quality, but also effectively reduce the amount of contrast agent and radiation dose in this examination.

919 Effective dose of esketamine for inhibiting body movement in hysteroscopic patients

Objective: To investigate the effective dose of esketamine for inhibiting body movement in hysteroscopic patients when combined with 3 μg·mL-1 propofol of plasma target control infusion. Methods: A total of 26 patients who received hysteroscopy with general anesthesia from Haikou Maternity and Child Health Hospital during December 2021 to February 2022 were selected. 3 μg·mL-1 propofol of plasma target control infusion was given when esketamine was injected, with the initial dose of 0.5 mg·kg-1. Dose of esketamine was determined by up-and-down sequential method, with the common ratio of esketamine dose in adjacent patients being 0.9. Probit analysis was used to calculate the median effective dose (ED50) and 95% effective dose (ED95) and the corresponding 95% confidence interval (CI). The circulation, respiration, body movement, operation time,awakening time and VAS score 10 minutes after awakening in surgery were observed. Respiratory depression (SpO2<90%), hypotension, nausea and vomiting, recovery agitation, dizziness, and nightmare were recorded.Results: The ED50 of esketamine combined with 3 μg·mL-1 propofol for inhibiting body movement in patients with hysteroscopy was 0.391 mg·kg-1 (95%CI=0.340-0.431 mg·kg-1), ED95 was 0.476 mg·kg-1 (95%CI=0.434-0.778 mg·kg-1). The operation time was (24.84±8.45) min, the time to awaken was (7.16±3.23) min, the median VAS score 10 minutes after awakening was 1(1, 2) scores. No patients had respiratory depression, hypotension or nightmare. Three patients had dizziness, and two had vomiting and one patient had agitation. Conclusion: The ED50 and ED95 of esketamine for inhibiting body movement in patients with hysteroscopy was 0.391 mg·kg-1 and 0.476 mg·kg-1 respectively when combined with propofol 3 μg·mL-1.

933 The strategies of in vitro fertilization and embryo transfer in elderly women with polycystic ovary syndrome

Polycystic ovary syndrome (PCOS) is one of the most common endocrine and metabolic disorders in women of childbearing age and the most common cause of ovulation disorders. Elderly women with PCOS have many infertility factors such as endocrine and metabolic disorders, decreased oocyte and embryo quality, and poor endometrial receptivity. Some patients can improve their pregnancy outcomes through in vitro fertilization and embryo transfer (IVF-ET). In this process, how to find suitable strategies to obtain higher clinical pregnancy rate and live birth rate is still uncertain. In this paper, we reviewed the research progress and discussed the strategies of IVF-ET in elderly PCOS women in order to provide some ideas for clinical application.

938 Development and practice of five-dimension integrated Career Guidance course for medical students

In order to enhance the educational value of the “Career Guidance” course for medical students,five dimensions were integrated into the course and implemented into the teaching practice. By reconstructing teaching objectives and teaching mode, the course adopted an integrated teaching form such as theory & practice,online & offline, on-campus & off-campus, simulation & actual practice; the experiential, heuristic, immersive,interactive and other teaching methods were attempted; ideological education was permeated into the teaching practice from five dimensions concerning student whole-person development, namely, moral integrity,professional competence, academic ability, aesthetic taste and humanistic love. In this way, we enriched the educational connotation and optimized the curriculum evaluation methods by strengthening process evaluation,integrating value-added evaluation and attaching more importance to the comprehensive evaluation. The whole process enhanced all-round educational effect. A questionnaire was made to compare the five-dimension integrated teaching mode with the traditional one and analyze the teaching effect. The results demonstrated that the integration of five dimensions produced effective education value with more positive impact and higher acceptance than the traditional mode, thereby realizing the goal of education and employment.

943 The practice of curriculum-based moral education with OBE teaching mode for future orthopedic professionals

Objective: To explore the practice of outcome-based education (OBE) teaching mode and its effect on the future orthopedic professionals under the background of curriculum-based moral education. Methods:A total of 120 orthopedics interns from the Second Affiliated Hospital of Wenzhou Medical University were selected from February to May 2022, and assigned according to the random number table method into 3 groups with 40 students in each: the traditional teaching group, OBE group (with OBE-guided orthopedic curriculum) and OBE+curriculum-based moral education group (OBE teaching mode integrated with curriculum-based moral education and clinical orthopedic teaching). The teaching outcomes of the three groups were assessed and compared through the full-mark final evaluation together with the teaching quality feedback by the investigation of the students’ satisfaction. Results: Both assessment scores and student teaching satisfaction of OBE group were higher than traditional teaching group, and OBE+curriculum-based moral education group had significantly higher scores and satisfaction than either traditional teaching group or OBE group, with statistical significance (P<0.05). Conclusion: Under the background of curriculum-based moral education, the orthopedic teaching course combined with OBE concept can improve students’ morality and stimulate their enthusiasm for the course,deepen their understanding and memory of orthopedic theoretical knowledge, thereby improving their clinical professional skills.
