517 Proanthocyanidins combined with piperine improved depression and anxiety-like behavior of chronic stress mice through serotonin receptor pathway

Objective: To investigate the anti-depressant and anti-anxiety effect of OPC combined with PIP on chronic unpredictable mild stress (CUMS) mice and to study the mechanism by analyzing the receptor of 5-HT1A in hippocampus, pre-frontal cortex and hypothalamus. Methods: The mice were divided into eight groups: Control group, CUMS group, CUMS+PIP 5 mg/kg+OPC (12.5, 25, 50 mg/kg) group, CUMS+PIP (5 mg/kg)+ OPC (50 mg/kg)+NAN-190 (0.1 mg/kg) group, CUMS+NAN-190 (0.1 mg/kg) group and CUMS+fluoxetine (10 mg/kg). After 21 days of chronic unpredicted mild stress, all mice were subjected to the forced swimming test, tail suspension test, novel food inhibition test and marble bury test to evaluate the depression- and anxiety-like behaviors. Then, we used high performance liquid chromatography method to detect the concentration of monoamine (5-HT, NE and DA) in the hippocampus, pre-frontal cortex and hypothalamus. Results: Compared with control group, the immobility time of forced swimming test and tail suspension test was significantly increased in CUMS group (P<0.01) and the latency to feed of CUMS in the novelty suppressed feeding also significantly increased (P<0.05). Meanwhile, the number of marbles buried of CUMS in the marble bury test obviously elevated (P<0.05, P<0.01). Given of OPC (25, 50 mg/kg) combined with PIP (5 mg/kg) improved the depression and anxiety behavior induced by CUMS (P<0.05, P<0.01). Using the HPLC method we found that OPC (25, 50 mg/kg) combined with PIP (5 mg/kg) significantly reversed the 5-HT level caused by CUMS in hippocampus, pre-frontal cortex and hypothalamus (P<0.05, P<0.01, P<0.001), however, there was no significant effect on NE and DA in the hippocampus, pre-frontal cortex and hypothalamus. Pretreatment with the 5-HT1A receptor antagonist NAN-190 prevented the improvement (P<0.05, P<0.01, P<0.001). Conclusion: OPC combined with PIP has improved the antidepressant- and anxiety-like behavior in CUMS mice probably by activating 5-HT1A receptor in the central nervous system.

524 Effect of Wenyangxiaozheng decoction on renal fibrosis in 5/6 nephrectomy mice and its possible mechanism

Objective: To observe the effect of Wenyangxiaozheng decoction on renal function and renal fibrosis in 5/6 nephrectomy (5/6N) mice and explore its mechanism. Methods: Out of 32 C57 mice 24 were randomly chosen to establish 5/6N model which closely simulated the process of human chronic renal failure and renal fibrosis. Two weeks after the operation, 24 mice were divided as 5/6N group, diovan group and wenyangxiaozheng decoction group according to the value of serum creatinine. The other 8 mice were treated as sham operation group. Eight weeks after the treatment, the protein expression of JAK2, STAT3, p-JAK2, p-STAT3, transforming growth factor-β1 (TGF-β1), Smad3 was determined by Western blot; the protein expression of Collagen-I (Col-I), Collgaen-III (Col-III) and α-SMA was determined by both Western blot and immunohistochemistry, TGF-β1, Smad3 mRNA were determined by RT-qPCR. Results: Eight weeks after the treatment, the protein expression of JAK2, STAT3, p-JAK2, p-STAT3, TGF-β1, Smad3, Col-I, Col-III, α-SMA and TGF-β1, Smad3 mRNA expression in 5/6N group was significantly upregulated when compared with that of sham group (P<0.01); compared with 5/6N group, the protein expression of JAK2, STAT3, p-JAK2, p-STAT3, TGF-β1, Col-I, Col-III, α-SMA and TGF-β1, Smad3 mRNA expression was obviously downregulated in diovan group and wenyangxiaozheng decoction group (P<0.01); compared with diovan group, indexes were all downregulated in wenyangxiaozheng decoction group (P<0.01) except TGF-β1 and α-SMA. Conclusion: Wenyangxiaozheng decoction could protect renal function and reduce the degree of renal fibrosis in 5/6N mice, and the mechanism may be related to the inhibition of TGF-β1/Smad3 and JAK2/STAT3 signaling pathway.

532 The mechanism of circRNA_000809 in regulating endometriosis stromal cell proliferation, migration and epithelial-mesenchymal transition by targeting miR-200c-3p

Objective: To explore the role of circRNA_000809 in regulating proliferation, migration and epithelial-mesenchymal transition (EMT) of endometriosis stromal cells by targeting miR-200-3p. Methods: From January 2020 to January 2022, twenty-five cases of chocolate ovarian cyst tissues from patients undergoing ovarian cyst excision in the Second Affiliated Hospital & Yuying Children’s Hospital of Wenzhou Medical University were selected as the endometriosis group. Twenty-five normal endometrial tissue samples were collected from non-endometriosis patients of childbearing age who underwent hysterectomy or hysteroscopy for early cervical cancer and were assigned to the normal endometrial group. qRT-PCR analysis was used to detect the expression of circRNA_000809 and miR-200c-3p in ectopic endometrium tissues of endometriosis patients and normal endometrium. The 5-ethynyl-2’-deoxyuridine (EdU) assays were used to detect cell proliferation ability. Transwell migration assays were employed to monitor migration capabilities. Western blot was performed to detect protein expression. Results: The circRNA_000809 expression in ectopic endometrium tissues was increased (P<0.05), and the miR-200c-3p expression was decreased compared with normal endometrium tissues (P<0.05). Pearson’s test showed that the expression of circRNA_000809 and miR-200c-3p was negatively correlated in ectopic endometrium tissues. We co-transfected ectopic endometrial stromal cells with siNC+inhibitor NC, si-circRNA_000809+inhibitor NC and si-circRNA_000809+miR-200c-3p inhibitor, respectively. The results showed that the proliferation and migration of ectopic endometrial stromal cells were attenuated and the protein levels of ZEB1/ZEB2 were also decreased in the si-circRNA_000809+inhibitor NC group compared with the siNC+inhibitor NC group; while the proliferation and migration of ectopic endometrial stromal cells and the expression of ZEB1/ZEB2 protein were reversed to some extent in the si-circRNA_000809+miR-200c-3p inhibitor group (P<0.05). Conclusion: Silencing of circRNA_000809 inhibited EESCs proliferation, migration, and EMT through miR-200c-3p/EMT signaling.

539 Trauma finite element modeling and analysis of ulna coronoid process fracture with posterior dislocation of elbow joint

Objective: To establish the ulna coronary finite element model of the fracture after the combined elbow dislocation and to study the influence of posterior elbow dislocation on joint stability. Methods: The intact elbow joint and forearm (IEJF) finite element model and fractured ulnar coronoid-process and forearm (FUCF) finite element model were established, and two groups of physiological loads with compression and flexion (three amplitudes for each group) were used for the calculation, verification and stress analysis. Results: The IEJF and FUCF finite element models with complete structure were established and verified. The results were consistent with clinical practice. Compared with the IEJF finite element model, the FUCF finite element model presented serious stress concentration in the radial head, and the longitudinal compression displacement of the elbow joint under different compressive force and the flexion angle caused by different flexion bending moments were both increased. Conclusion: The fracture of the coronoid process of the ulna and the posterior dislocation of the elbow might affect the elbow joint and forearm longitudinal instability to some extent. The elbow joint and forearm finite element model constructed in this experiment can be used to evaluate the clinical stability of ulna coronoid process fracture with posterior dislocation of elbow and the formulation of surgical treatment plan.

545 Clinical analysis of transcanal endoscopic ear surgery for single congenital middle ear malformations

Objective: To describe the operative findings and analyze the surgical effect of endoscopic ear surgery for congenital middle ear malformations. Methods: From January 2017 to June 2021, a retrospective study was performed on 27 patients with congenital middle ear malformations who underwent endoscopic surgery. Demographic data, high-resolution computed tomography findings, intraoperative findings, surgical management and audiometric data were recorded. Results: Altogether 27 patients with congenital middle ear malformations were included, consisting of 3 cases unilateral mixed hearing loss with significant conductive hearing loss and 24 cases unilateral conductive hearing loss. Anomalies were categorized according to the Teunissen and Cremers classification system: 5 ears were categorized as class I, 6 ears as class II, 15 ears as class III and 1 ear as class IV. Prostheses used in surgery included 8 PORPs, 7 TORPs and 11 Pistons. One patient was type IV, with vestibular window atresia, facial nerve malformation, and malleostapedotomy was performed with lengthened 7mm Piston to reconstruct ossicular chain. The air conduction pure tone average was (62.1±8.3) dB HL preoperatively and (30.6±7.4) dB HL postoperatively (t=19.47, P<0.001). Fifteen cases (55.6%) acquired an air conduction hearing gain exceeding 30 dB HL. Mean air-bone gap (ABG) was (43.1±9.3) dB HL preoperatively and (13.4±5.2) dB HL postoperatively (t=16.75, P<0.001). Twenty-five cases (93%) showed a postoperative ABG of less than 20 dB after operation. Conclusion: Simple congenital middle ear deformity is mainly manifested as a listening bone chain deformity. The ear endoscopy alone for the diagnosis and treatment of congenital middle ear malformation has the advantages of a clear field of vision, accurate manipulation, no need for external ear canal or posterior ear incision, small wound, few complications, good postoperative hearing recovery.

550 Expression of SMARCA5 and its role in non-small cell lung cancer

Objective: To study the relationship between SMARCA5 expression and prognosis of patients with non-small cell lung cancer (NSCLC) and also to investigate the effect of silencing SMARCA5 expression on the biological behavior of NSCLC. Methods: The immunohistochemistry method was adopted to detect the expression of SMARCA5 proteins in NSCLC and normal lung tissues. The correlation between the expression of SMARCA5 and clinicopathological factors and survival prognosis of patients with NSCLC was statistically analyzed. Western blot and reverse transcription polymerase chain reaction were used to detect the expression of SMARCA5 in normal human bronchial epithelial cell line and human lung cancer cell lines (A549, Lu165, SK-MES-1, and NCI-H520). SMARCA5 siRNA was employed to silence the expression of SMARCA5. MTT and Transwell methods were used to detect the proliferation and invasion of lung cancer cells. Results: SMARCA5 protein was highly expressed in 78(59.5%) NSCLC tissues. SMARCA5 expression was significantly associated with poor differentiation (χ2=16.666, P<0.001), TNM stage (χ2=24.462, P<0.001), and pleural invasion (χ2=4.601, P=0.032). Additionally, high expression of SMARCA5 was associated with poor prognosis of patients with NSCLC (Log-rank test, P=0.001). The knockdown of SMARCA5 expression inhibited cell growth (P<0.05) and invasion (P<0.05) in NSCLC. Conclusion: The high expression of SMARCA5 is associated with tumor TNM stage, pleural invasion, poor differentiation and poor prognosis in NSCLC patients, and the knockdown of SMARCA5 expression can reduce the proliferation and invasion activity of NSCLC cells. SMARCA5 may play a decisive role in tumorigenesis of NSCLC.

557 Comparison of preoperative immune-inflammation index and carcinoembryonic antigen in different pathological types of ground-glass opacity

Objective: To compare the preoperative neutrophil to lymphocyte ratio (NLR), platelet to lymphocyte ratio (PLR), lymphocyte to monocyte ratio (LMR) and serum carcinoembryonic antigen (CEA) in different pathological types of ground-glass opacity (GGO) in order to explore whether there were differences
and their significance. Methods: The NLR, PLR and LMR of 494 GGO patients who underwent surgical treatment were collected, including 176 cases of infiltrating adenocarcinomas, 150 cases of minimally invasive adenocarcinoma, 117 cases of precursor glandular lesions and 51 cases of benign lesions. Serum CEA was collected from only 150 infiltrating adenocarcinomas, 127 minimally invasive adenocarcinomas, 95 precursor glandular lesions, and 45 benign lesions. The NLR, PLR, LMR, and serum CEA of each pathological group were compared. Results: The NLR of the precursor glandular lesion group (2.32±1.59) was higher than that of the benign lesion group (1.82±0.64) (P<0.05). The PLR in lung adenocarcinoma group (141.19±53.14) and in precursor glandular lesion group (145.94±51.92) was both higher than that of the benign lesion group (124.90± 37.04) (all P<0.05). The CEA of lung adenocarcinoma group [1.83 (1.18, 2.73) ng/mL] was higher than that of the control group (precursor glandular lesions+benign lesions) [1.49 (1.03, 2.08) ng/mL] (P<0.01). The CEA in infiltrating adenocarcinoma group [2.14 (1.29, 2.92) ng/mL] was higher than that of minimally invasive adenocarcinoma group [1.67 (1.09, 2.43) ng/mL], precursor glandular lesion group [1.46 (1.03, 2.06) ng/mL], and benign lesion group [1.53 (1.00, 2.10) ng/mL] (P<0.05). Conclusion: Systemic immune inflammatory index begins to change in early-stage lung adenocarcinoma, even in the stage of precursor glandular lesions, and serum CEA begins to change in early-stage lung adenocarcinoma. Dynamic observation of the changes in the above indicators is conducive to the early detection and early intervention of malignant lesions.

562 Investigation on related symptoms of 2 965 perimenopausal women in Wenzhou and analysis of the influencing factors

Objective: To understand the menopause related symptoms and their influencing factors of perimenopausal women, and to provide reference basis for strengthening perimenopausal health management. Methods: An investigation was made on 2 965 women aged 40-65 in Wenzhou by cluster random sampling. The incidence of menopause related symptoms was evaluated by the modified Kupperman score scale. Results: The detection rate of perimenopausal syndrome in women aged 40-65 was 68.20%. The top five reporting rates of main symptoms of perimenopausal syndrome were fatigue in 1 948 cases (65.70%), anxiety in 1 785 cases (60.20%), insomnia in 1 638 cases of (55.24%), bone and joint pain in 1 506 cases (50.79%) and decreased sexual desire in 1 466 cases of (49.44%). Multivariate Logistic regression analysis showed the incidence of perimenopausal syndrome was relatively low among those who were younger than 45 years old, married, medical and educational workers, non-postmenopausal, and those who did not expect menopause, and 70.22% of the surveyed women said they had a demand for perimenopausal health management services. Conclusion: The detection rate of perimenopausal syndrome among women aged 40-65 in Wenzhou is high, and so is the demand for perimenopausal women health management. Therefore, it is necessary to popularize perimenopausal health care knowledge and improve perimenopausal health management so as to promote women’s health level.

567 Reliability and validity of the 3-meter backward walk test in evaluating dynamic balance function in the stroke population

Objective: To test the reliability, validity and responsiveness of 3-meter backward walk test (3MBWT) in the evaluation of dynamic balance function in stroke patients. Methods: From December 2020 to June 2021, 40 stroke patients from the Second Affiliated Hospital of Wenzhou Medical University completed 3MBWT, Berg Balance Scale (BBS), the timed up and go test (TUGT) and modified Barthel Index (MBI) for Spearman correlation analysis. Two researchers completed the 3MBWT of the same patient separately. One of the researchers retested the 3MBWT of the same patient 2 hours later and calculated the intraclass correlation coefficient (ICC). After one week of treatment, 3MBWT, BBS and TUGT were retested in 40 patients to compare the reactivity of the three tools. Results: The retest ICC of 3MBWT was 0.889 and the inter-rater ICC was 0.998; 3MBWT was strongly correlated with BBS (r=-0.854, P<0.001), TUGT (r=0.872, P<0.001), and 3MBWT was moderately correlated with MBI (r=-0.519, P<0.001). The order of |ES| and |ERM| values is 3MBWT>TUGT>BBS. Conclusion: 3MBWT has good reliability, validity and reactivity in evaluating dynamic balance function in stroke patients.

575 Lymphoepithelioma-like carcinoma in the liver: clinicopathological features and MRI findings of 7 cases

Objective: To investigate the clinicopathological features and MRI findings of 7 cases with lymphoepithelioma-like carcinoma of liver. Methods: A retrospective analysis was carried out focusing on the clinicopathological features and MRI findings of 7 cases with lymphoepithelioma-like carcinoma of liver, who were confirmed by surgery and pathological examination in our hospital from December 2009 to December 2021. Results: Of 7 patients, there were 2 males and 5 females, with a median age of 54 years old. There were 4 cases of HBV (+), and all cases showed HCV (-), among which 1 case was accompanied by liver cirrhosis and elevated serum AFP level (389.7 ng/mL). Further pathological diagnosis reported 1 case of lymphoepithelioma like
hepatocellular carcinoma, and 6 cases of lymphoepithelioma like cholangiocarcinoma. Microscopically, there was infiltration of a large number of lymphocytes in the tumor tissue. EBER in situ hybridization was positive in 6 cases. MRI showed that the tumors with nodular appearance in 5 cases, regular morphology in 6 cases and clear boundary in 6 cases. T1WI showed low signal, while T2WI showed high signal. Meanwhile, 2 cases showed slightly high signal and 5 cases displayed significantly high signal on DWI. Furthermore, there were various enhancement patterns, including 3 cases of type I (wash-in and wash-out enhancement); 2 cases of type II (continuous annular enhancement); and 2 cases of type III (marginal filling enhancement). Capsular-like enhancement was visible in type I and II lesions. There was no abnormal enhancement around the tumor, no bile duct dilatation or hepatic capsule retraction in all lesions. Conclusion: Clinically rare, lymphoepithelioma-like carcinoma of liver has relatively good prognosis with certain pathological characteristics. MRI manifestations are mainly rounded nodules with various enhancement, mostly capsular-like enhancement.

577 Analysis of related factors in clinical staging of nasopharyngeal carcinoma patients

Objective: To study the correlation between clinical stage and related factors of nasopharyngeal carcinoma patients, and to provide clinical data for early diagnosis and treatment of nasopharyngeal carcinoma. Methods: From June 2016 to September 2021, 63 patients with nasopharyngeal carcinoma hospitalized for initial diagnosis in the Second Hospital of Yinzhou District, Ningbo (Patients with nasopharyngeal carcinoma were excluded from radiotherapy and chemotherapy) were retrospectively analyzed, and their clinical stages, symptoms of first diagnosis, time of first diagnosis, concomitant symptoms, lymphatic metastasis, tumor growth pattern, pathological type and other characteristics were analyzed. Results: Of 63 patients with nasopharyngeal carcinoma, there were 49 males and 14 females (male: female=3.5:1), aged from 22 to 82 years, with an average of (53.8±1.7) years. There were 17 cases (27.0%) in stage I, 14 cases (22.2%) in stage II, 29 cases (46.0%) in stage III and 3 cases (4.8%) in stage IV, respectively. There were 31 cases in early stage and 32 cases in middle and late stage, with the early diagnosis rate being 49.2%. Symptoms at first diagnosis in the order of occurrence rate were upper neck lymph node enlargement in 24 cases (38.1%), bloodsucking nose in 18 cases (28.5%), nasal obstruction in 10 cases (15.9%), tinnitus in 8 cases (12.7%), nosebleed, headache and no symptoms in 1 case (1.6%) respectively. The time of first diagnosis was (2.4±0.3) months for early-stage patients and (5.4±1.1) months for middle and late stage patients. The proportion of patients with concomitant symptoms was 15/31 (48.4%) and 24/32 (75%), respectively. Logistic regression analysis showed that initial symptoms (lymphadenopathy, nasal blood aspiration), longer initial diagnosis time, concomitant symptoms, and lymph node metastasis were independent risk factors for middle and late clinical stages of nasopharyngeal carcinoma (P<0.05). Conclusion: The clinical stage of nasopharyngeal carcinoma was correlated with first diagnosis symptoms, time of first diagnosis, concomitant symptoms and lymph node metastasis. Shortening the time of first diagnosis is an important means to find early nasopharyngeal carcinoma. Nasopharyngeal examination must be done once within 2 months in cases of reflux, nasal obstruction, tinnitus and stuffy ears, nasal bleeding or lymph node enlargement in upper neck.

582 Application of closed-loop management in the examination procedure of magnetic resonance imaging for critically ill patients

Objective: To explore the effect of closed-loop management in the procedure of magnetic resonance imaging (MRI) examination for critically ill patients. Methods: Fifty-four critically ill patients who underwent MRI in the First Affiliated Hospital of Wenzhou Medical University from August 2020 to November 2020 after the process reformation were selected from picture archiving and communication systems (PACS) as the experimental group, and 28 critically ill patients who underwent MRI before the process reformation in June 2020 were allocated to the control group. We analyzed the time data and the problems in the MRI process of critically ill patients. Through multi-department collaboration, closed-loop management measures were applied to transform the process, a closed-loop path was formed for key stages, such as MRI appointments, waiting before examination, examination, and report sharing. The total length of MRI examination (including appointment duration, waiting duration before examinations, examination duration, and report sharing duration), the satisfaction degree of medical staff, and other indicators were compared and evaluated between the two groups. Results: Compared with the control group, the experimental group had significantly shorter appointment time, waiting time, examination time, report sharing time, and the overall MRI examination time (P<0.05). Compared with before process transformation, the satisfaction degree of doctors, nurses and patients after process transformation was improved (P<0.001). Conclusion: The application of closed-loop management could make all aspects of the examination seamless, effectively shorten the whole duration of the MRI examination for critically ill patients, improve the satisfaction degree of medical staff and patients, and guarantee the medical safety of patients.

587 Preventive effect of pressure-redistribution air pad for intraoperative acquired pressure injury in patients with aortic dissection surgery

Objective: To evaluate the clinical effect of pressure-redistribution air pad for intraoperative acquired pressure injury in aortic dissection surgery. Methods: Tolly 119 aortic dissection patients who needed aortic arch replacement were selected as research subjects from March 2021 to February 2022 in the First
Affiliated Hospital of Wenzhou Medical University. They were randomly divided into two groups, 59 in the test group and 60 in the control group. The test group adopted pressure-redistribution device to prevent intraoperative acquired pressure injury, while the control group was given pressure protection nursing care with conventional gel pad and cotton pad. The difference in incidence of intraoperative acquired pressure injury, the relative temperature
difference in the sacrococcyx skin and the job satisfaction of nurse between the two groups were compared. Results: Compared with the control group, the test group had lower incidence of intraoperative acquired pressure injury (0 vs. 36.7%), higher job satisfaction of nurse (94.44% vs. 77.14%) and smaller relative temperature difference between sacrococcyx skin and circumference normal skin [0 (-0.1,0.1) vs. 0.3 (-0.3,0.5)], showing statistical difference between the two groups (P<0.05). Conclusion: The use of pressure-redistribution air pad during operation can better reduce the incidence of intraoperative acquired pressure injury in patients with aortic dissection, and produce less temperature difference between the caudal sacrum and surrounding normal tissue, which results in more job satisfaction of nurse. It is a preferable protective measure for intraoperative pressure injury.

To meet the social demand for health inspection and quarantine professionals under Healthy China strategy, Wenzhou Medical University, taking advantages of its medical laboratory technology and the experience in professional education, has explored the interdisciplinary educational mode. The author introduces the train of thought, the implementation method and the preliminary results from the aspects of training objectives, curriculum, teaching staff and teaching resources, so as to share experience in the higher education reform of health inspection and quarantine specialty against the background of New Medical Science.
