Effects of long-term high salt diet on blood glucose and fecal metabolites in mice

Objective: To explore the mechanism of long-term high salt diet on blood glucose in male C57BL/6J mice by fecal metabonomics. Methods: Eight 8-week-old mice were divided into two groups, four in each. The control group was given ordinary diet and the high salt group was given 8% high salt diet for 22 weeks.After intervention, fasting plasma glucose (FPG), glucose tolerance test (GTT) and insulin tolerance test (ITT) were performed, and tail cuff method was used to monitor the systolic blood pressure and diastolic blood pressure of tail artery in each group. The fecal samples of mice were collected and analyzed by liquid chromatographymass spectrometry (LC-MS). The data pattern was identified by principal component analysis (PCA). Results:Compared with control group, the high salt group significantly decreased body weight (P<0.01), so did the proportion of brown fat (P<0.01). The level of FPG was significantly higher than that in the control group (P<0.05), so was the area under the curve (AUC) of GTT and the systolic blood pressure (P<0.05), but there was no significant difference in the AUC of diastolic blood pressure and ITT (P>0.05). Metabonomic analysis showed differences in the principal components of fecal metabolites between the two groups, which was distinguishable. A total of 12 metabolites including 3-hydroxyoctanoyl carnitine, 2-otenedioic acid, 2-octenoate, drithromycin,NBD-stearoyl-2-arachidonoyl-sn-glycerol, 20-ethyl-PGE2, stevioside, biperiden, lysyl-Methionine, norcholic acid, lansiol, 7, 8, 7’, 8’-tetradehydroastaxanthin and PGP-Me were found and identified. These metabolites were mainly involved in amino acid metabolism, prostaglandin metabolism in islets and oxidative stress inflammation.Conclusion: Long-term high salt diet not only causes abnormal blood glucose metabolism in mice, but also has imbalanced regulation of amino acid metabolism, prostaglandin metabolism in islets and oxidative stress inflammation.

Fat mass- and obesity-associated gene promotes ectopic endometriotic stromal cells fibrosis via TLR2/p38 axis

Objective: To investigate the effect of FTO on fibrosis of ectopic endometrial mesenchymal cells (eESCs). Methods: m6A-related genes in endometriosis (EMs) were screened and detected, using quantitative real-time polymerase chain reaction (qRT-PCR). Detection of fibrosis-related gene expression in eESCs by overexpression of FTO in a lentiviral vector. Relationship between FTO and TLR2 in eESCs predicted and validated by m6A2 Target site and MeRIP-qPCR. Protein changes in p38 were detected by Western blot. Results:FTO expression was down-regulated in EMs (P<0.05). FTO overexpression promoted fibrosis and inhibited proliferation of eESCs (P<0.05). FTO upregulated its protein level through m6A modification of TLR2 in eESCs (P<0.05). FTO promoted cell fibrosis through TLR2/p38 signaling pathway in eESCs (P<0.05). Conclusion:FTO is low expressed in EMs and upregulation of FTO promotes fibrosis of eESCs through TLR2/p38 signaling pathway.

Molecular mechanism of dexamethasone in regulating aquaporin 2 through PKA signal pathway

Guide::
Objective: To observe the direct regulation effect of dexamethasone (Dex) on aquaporin 2 (AQP2) in vitro, and to clarify whether this effect depends on PKA signal pathway, using protein kinase A (PKA) as a reference. Methods: PKA, AQP2 wild-type and mutant plasmids were prepared. The mutant plasmids blocked the PKA signaling pathway by mutating the 4 PKA phosphorylation sites (S256, S261, S264, and S269) at the C-terminal of AQP2. AQP2 wild-type and mutant plasmids were transfected into HEK293 cells and
Xenopus oocytes by microinjection and then co-transfected with PKA plasmid or Dex 0.1 μmol/L intervention.Western blot was used to detect the total protein expression of AQP2, and cell surface biotin was used to detect AQP2 total protein expression. The AQP2 membrane protein expression was evaluated chemically, and water
permeability difference in oocytes was compared between each group. Results: Both Dex and PKA significantly up-regulated the expression of AQP2 membrane protein and total protein in HEK 293 cells (both P<0.01).Compared with the wild type, the mutation of PKA phosphorylation sites significantly inhibited the phosphorylation of AQP2, which was direct induced by PKA in vitro; AQP2 membrane protein and total protein expression were significantly down-regulated after mutation (all P<0.01). After the PKA phosphorylation sites mutated, the upregulating
effect on the expression of AQP2 membrane protein and total protein by Dex and PKA was significantly inhibited (all P<0.01). Both Dex and PKA significantly increased the water permeability of oocytes; after mutation of the PKA phosphorylation sites, the effect was significantly inhibited (all P<0.01). Conclusion: Dex and PKAact in the same direction and have significant up-regulation effect on the total protein and membrane protein expression of AQP2. This effect depends on the normal PKA signaling pathway.

Comparison of two screening tools in GLIM for the diagnosis of malnutrition in hospitalized elderly

Objective: To analyze the consistency of the results of different nutrition screening tools and different combinations of criteria in diagnosing malnutrition of the global leadership initiative on malnutrition (GLIM). Methods: A total of 130 elderly patients (≥65 years old) who were hospitalized in the Geriatrics Department of Zhejiang Hospital from June 2019 to June 2020 were retrospectively selected. The NRS2002 and other questionnaires were evaluated, and the patients with positive nutrition screening were diagnosed as malnutrition according to the second step of GLIM who met at least one etiological criterion and one clinical criterion. Kappa value analysis of the consistency of NRS2002 and MNA-SF as the first step of GLIM nutritional screening tool for diagnosing malnutrition and the consistency of the two etiological criteria combined with various clinical criteria respectively for diagnosing malnutrition. At the same time, the independent influencing factors of malnutrition were determined by Logistic regression analysis. Results: Using NRS2002 and MNA-SF as screening tools, 23(17.69%) and 25(19.23%) malnourished patients were finally diagnosed, with high consistency (Kappa=0.71, P<0.001). In malnutrition patients screened positive for both NRS2002 and MNA-SF, consistency in diagnosing malnutrition with the combination of involuntary weight loss and the two etiological criteria wasmoderate (Kappa=0.59, P=0.003), the diagnostic agreement between muscle loss and the respective variety of the two etiological criteria was also moderate (Kappa=0.51, P=0.007). However, low body mass index (BMI) was less consistent with the combination of the two etiological criteria (Kappa=0.26, P=0.188). In addition, logistic regression analysis showed that depression, insomnia were independent risk factor of malnutrition in elderly hospitalized patients (P<0.05). Conclusion: When NRS2002 and MNA-SF are used as the first screening tools of GLIM, the consistency of diagnosis of malnutrition is high, and they can be substituted for each other. In the standard combination of the second step of GLIM, meeting involuntary weight loss or muscle mass loss with each combination of the two etiological criteria was moderately consistent.

Support vector machine based on clinical risk factors and CT radiomics for diagnosing of axial spondyloarthritis

Objective: To construct a prediction model of axial spondyloarthritis (axSpA) based on CT imaging and clinical features by support vector machine (SVM). Methods: A group of 568 patients diagnosed as axSpA (n=319) or non-axSpA (n=249) between October 2012 and February 2019 were included retrospectively,who were randomly divided into training and validation cohorts at a ratio of 7:3. The volume of interest (VOI) was manually sketched on the sacroiliac joint CT and the radiomics were extracted. The mRMR and LASSO algorithms were used to select the optimal parameters. Univariate and multivariate Logistic regression analysis was made to find the clinical risk factors of axSpA. Finally, SVM was used to build clinical, radiomics and clinicalradiomics combined models, and their performance was evaluated by the receiver operating characteristic curve (ROC) and Delong test. Results: The clinical-radiomics model showed the best diagnostic efficacy in the validation cohort and higher area under curve (AUC) values (AUC=0.91) than radiomics model (AUC=0.83) or clinicalmodel (AUC=0.81) (P<0.05), with high accuracy (0.83), sensitivity (85.2%) and specificity (79.7%). Conclusion:Support vector machine based on CT radiomics and clinical risk factors has high value in the diagnosis of axSpA.

Expression of AIM2 inflammasome signaling pathway in primary EBV infection in children and its correlation with liver injury

Objective: To explore the expression of absent in melanoma 2(AIM2) inflammatory signaling pathway in children with primary Epstein-Bark virus (EBV) infection and its correlation with liver injury. Methods: A total of 82 children with positive EB-VCA IgM and EBV-DNA from January 2020 to June 2021 were enrolled as primary EBV infection group, among whom 52 had liver injury and 30 had no liver injury; 50 healthy children with positive EBV-IgG (EB-VCA-IgG, EB-NA-IgG) and negative EB-VCA-IgM as previous EBV infection group and 50 healthy children with negative EBV-IgG/EBV-VCA IgM as the healthy control group.Real-time PCR was used to measure the mRNA expression of AIM2 inflammasome (AIM2, ASC and caspase-1) in peripheral blood mononuclear cells. The serum interleuk in (IL)-1β, IL-18 level was detected by ELISA.Results: Compared with previous EBV infection group and the control group, primary EBV infection group had significantly higher relative mRNA expression level of AIM2, ASC, and caspase-1 in PBMCs and serum level of IL-1β and IL-18 (P<0.05). The level of AIM2, ASC, caspase-1 mRNA in PBMCs and serum level of IL-1β, IL-18 were positively correlated with the virus load in serum (r=0.712, 0.494, 0.479, 0.384, 0.300, P<0.05). Compared with the control group, both liver injury group and no liver injury group had significantly higher relative mRNA expression level of AIM2, ASC, and caspase-1 in PBMCs and serum level of IL-1β and IL-18 (P<0.05). The liver injury group had significantly higher level of above-mentioned parameters than the no liver injury group (P<0.05).The relative expression level of AIM2 was positively correlated with the relative mRNA expression level of ASC and caspase-1 and the serum level of IL-1β and IL-18 (r=0.718, 0.729, 0.548, 0.529, P<0.05). Conclusion:The AIM2 inflammasome signaling pathway might be involved in the pathogenesis of primary EBV infection in children and is associated with the EBV hepatitis.

Performance of MRI LI-RADS v2018 vs. CEUS LI-RADS v2017 in the diagnosis of hepatocellular carcinoma: a comparative analysis

Objective: To compare the difference between magnetic resonance imaging (MRI) Liver Imaging Reporting and Data System (LI-RADS) version 2018 and contrast-enhanced ultrasound (CEUS) LI-RADS version 2017 in diagnostic efficacy for hepatocellular carcinoma (HCC). Methods: We prospectively collected the data of 153 high-risk patients (173 lesions) with HCC from August 2019 to July 2021 in Lishui Central Hospital of Zhejiang Province, and evaluated the imaging characteristics of all intrahepatic lesions. Moreover, the classifications of all intrahepatic lesions by MRI LI-RADS v2018 and CEUS LI-RADS v2017 were obtained.Based on the pathological or clinical diagnosis, all lesions were divided as HCC group (119 cases), other malignancy (OM) group (20 cases) and benign lesion group (34 cases). The sensitivity, specificity and accuracy of MRI LI-RADS v2018 and CEUS LI-RADS v2017 for the diagnosis of HCC or OM were calculated. The Mann-whitney U test was used to compare the classification results between MRI LI-RADS v2018 and CEUS LI-RADS v2017. Results: Regarding the classification results of HCC or OM, there was no statistical differencebetween MRI LI-RADS v2018 and CEUS LI-RADS v2017 (Z=-0.363, -1.705; P=0.717, 0.088). In applications of MRI LI-RADS v2018 and CEUS LI-RADS v2017, the LR-5 criterion for the diagnosis of HCC had respective sensitivity of 63.9% and 63.9%, specificity of 98.1% and 92.6%, accuracy of 74.6% and 72.8%. The joint criterion of LR-4 and LR-5 for the diagnosis of HCC showed respective sensitivity of 74.8% and 84.9%, specificity of 94.4% and 70.4%, accuracy of 80.9% and 80.3%. Besides, the LR-M criterion for the diagnosis of OM showed respective sensitivity of 90.0% and 70.0%, specificity of 87.6% and 90.8%, accuracy of 87.9% and 88.4%.Among all, the MR v2018 result was significantly higher than the CEUS v2017 in regarding the specificity of LR-4/5 criteria for the diagnosis of HCC, showing statistical difference (P=0.001). Conclusion: Both the LR-5 and LR-M criteria of MRI LI-RADS v2018 and CEUS LI-RADS v2017 have similar diagnostic efficacy for HCC
and OM, with MR v2018 significantly higher than that of the CEUS v2017 in comparison of the specificity by LR-4/5 criteria for the diagnosis of HCC.

The role of extracellular HSP70 in regulating lung injury in acute pancreatitis

Objective: To investigate the regulating role of extracellular heat shock protein 70 (HSP70) in acute pancreatitis complicated with acute lung injury. Methods: The mouse models of acute pancreatitis with different expression of extracellular HSP70 were constructed. The role of extracellular HSP70 in acute pancreatitis and acute lung injury was observed by detecting pancreatic water content, HE staining of pancreas and lung tissue,enzyme linked immunosorbent assay (ELISA), transcriptome sequencing of lung tissue and RT-PCR. Results:In the experiments of pancreatic water content and HE staining of pancreatic and lung tissues, higher level of extracellular HSP70 showed higher pancreatic water content, and more obvious edema of pancreatic and lung tissues (P<0.05). In the ELISA experiment, higher level of extracellular HSP70 was also accompanied by higher level of tumor necrosis factor-α (TNF-α) and interleukin-6 (IL-6) (P<0.05). In the lung tissue transcriptional
sequencing experiment, the gene expression change caused by acute pancreatitis was the most significant, which mainly affected the signal pathways such as ECM-receptor interaction, Cytokine-cytokine receptor interaction,Focal adhesion and PI3K/Akt and thereby changed the function of cell adhesion. In the RT-PCR experiment, the expression of the most significantly changed related genes was consistent with the results of transcriptome sequencing. Conclusion: This study shows that extracellular HSP70 can aggravate lung injury in acute pancreatitis and may play a role through such signal pathways as ECM-receptor interaction, Cytokine-cytokine receptor interaction, Focal adhesion and PI3K/Akt.

Effect of small airway dysfunction on static lung function index in the elderly and its significance in the identification of labor ability

Objective: To study the influence of small airway dysfunction on static lung function index in the elderly. Methods: According to the inclusion and elimination criteria, 60 cases of normal lung function group (control group) and 94 cases of simple small airway dysfunction group (observation group) were selected. The various lung function indicators were compared between the two groups. Persion correlation analysis of the small airway and static lung function indexes was performed, and uniary linear regression was used for the RV/TLC and DLCO regression equations. Results: V50, V25 and MMEF, the three indicators of both groups were positively associated with TLC, DLCO, respectively (r=0.272-0.527, P<0.05), and none was associated with KCO (P>0.05);RV/TLC in the control group was negatively associated with V25, MMEF (r=-0.322, -0.376, P<0.05); RV/TLC in the observation group was negatively associated with V50, MMEF (r=-0.336, -0.250, P<0.05). Both groups showed no significant difference (P>0.05) in TLC, FRC, RV, the three static lung function indicators, but very significant difference in RV/TLC, DLCO, KCO (P<0.001); RV/TLC, DLCO established the corresponding regression equations with V50, V25 and MMEF respectively (r=0.322-0.544, F=6.719-63.755, P<0.05) and then DLCO,
RV/TLC correction formula was acquired, i.e. corrective value=actual value±α×ΔX (α=regression coefficient,ΔX=expected-Actual). Conclusion: The elderly simple airway dysfunction affect static lung function indicators to varying degrees. It is recommended that the RV/TLC, DLCO judgment standard should adopt the corresponding correction formula to reduce false positives when assessing lung impairment in occupational disease patients with small airway dysfunction known to be caused by non-occupational factors.

Expression of circRNA CDR1as in breast cancer and its clinical significance

Objective: To investigate the expression of circRNA CDR1as in breast cancer tissues and its relationship with clinicopathological characteristics. Methods: A total of 97 breast cancer patients who underwent surgical resection from June 2018 to September 2019 were collected from the Department of Thyroid and Breast Surgery of the Third Affiliated Hospital of Wenzhou Medical University. The expression of circRNA CDR1as in breast cancer tissues and adjacent normal breast tissues was detected by fluorescence quantitative PCR. Results:The expression of circRNA CDR1as was increased in breast cancer tissues compared with normal breast tissues (P<0.05). The expression of circRNA CDR1as was increased in invasive ductal carcinoma of the breast compared to ductal carcinoma in situ (P<0.05). The expression of circRNA CDR1as in invasive ductal carcinoma of the breast showed significant difference in size, lymph node metastasis and clinical stage (P<0.05). Conclusion:The expression of circRNA CDR1as in invasive ductal carcinoma of the breast is related to the size, lymph node metastasis and clinical stage.

Mutation of FUT1 and FUT2 of para-Bombay phenotypes and pedigree genetic analysis

Objective: To analyze the mutation of FUT1 and FUT2 genes of a rare para-Bombay phenotype and its genealogy. Methods: A patient with para-Bombay blood type treated in the People’s Hospital of Cangnan County on December 22, 2019 was selected. Tube agglutination method was used for ABO blood type and H antigen testing. ABO week antigens on red blood cells were determined by erythrocytes absorption-radiation testing. PCR-SBT method was used to sequence the gene of FUT1 and TUT2, and venous blood samples of his wife and daughter were extracted for examination. Results: The blood type of proband and his daughter was Amh
type, H antigen negative. Direct sequencing results showed FUT1 c.551-552delAG, FUT2 c.357C>T, c.385A>T.His wife’s blood type was A, H antigen positive; ABO blood group genotypes were all ABO*A1.02/O.01.02.Conclusion: FUT1 and FUT2 genes of proband were inherited to his daughter.

Intervention of bundle care in EICU patients with mechanical ventilation and its effect on prognosis

Objective: To investigate the efficacy of bundle care on prognosis of emergency intensive care unit (EICU) patients with mechanical ventilation. Methods: Totally 100 patients with mechanical ventilation were selected as research subjects from June 2020 to June 2021, who were randomly allocated into rehabilitation group and conventional group with 50 cases in each. The conventional group adopted the conventional treatment and nursing care, while the rehabilitation group adopted bundle care. The differences in prognostic effect between two groups were compared. Results: Before the intervention, there was no significant difference in blood gas indicators between two groups. After intervention, the mechanical ventilation time and EICU hospitalization time in the rehabilitation group were (5.6±1.0)days and (16.6±3.5)days respectively, compared with (7.9±1.3) days and (24.3±5.3) days in the conventional group (P<0.05). Adverse reactions such as ventilator-associated pneumonia (VAP), non-planning extractivity was significantly reduced, and the nursing compliance was increased (P<0.05).Conclusion: Bundle care nursing intervention in EICU mechanical ventilation patients can significantly reduce the mechanical ventilation time, prevent adverse reactions and promote patients’ recovery.

Hospital-family based health information satisfaction nursing for patients with polycystic ovary syndrome undergoing egg retrieval and transplantation and its effect on pregnancy outcomes

Objective: To analyze the value of hospital-family based health information satisfaction nursing for patients with polycystic ovary syndrome (PCOS) undergoing egg retrieval and transplantation and to explore its effect on pregnancy outcomes. Methods: Eighty PCOS patients who underwent egg retrieval and transplantation in Zhoushan Women and Children’s Hospital from January 2019 to January 2020 were enrolled as research subjects and they were randomly divided as study group (n=40) and control group (n=40). The study group was given hospital-family based health education information satisfaction nursing, while control group was given
routine nursing. The scores of State-Trait Anxiety Inventory (STAI) before and after nursing, cognition level of related diseases before and after nursing intervention, fertilization, abortionand total pregnancy rate after nursing intervention were compared between the two groups. Results: After nursing, STAI scores in both groups were
improved, with the study group significantly lower than the control group (P<0.05). After nursing intervention,cognition levels of ovulation induction, egg retrieval, PCOS and complications in study group were significantly higher than those in control group (P<0.05), and total pregnancy rate was significantly higher than that in control
group (P<0.05). Conclusion: The hospital-family based health education information satisfaction nursing can effectively improve disease cognition level of patients after egg retrieval and transplantation, reduce negative emotions and effectively increase pregnancy rate.

Objective: To provide reference for colleges and universities with language assessment policy and practice through positive discourse analysis. Methods: By adopting appraisal theory as the analysis framework and classroom observation and interview as major methods of data collection, this study attempted to investigate the process of students’ involvement in peer-evaluation and to discuss the characteristics and the role of teacher feedback in classroom activities for students’ oral competence enhancement. Results: Cognitive scaffolding and social-affective support embedded in teacher feedback can work positively on cultivating students’ oral competence in conducting useful peer evaluation, which in turn benefits learners from the assessment process. Gradual withdrawal of linguistic support and adjustment in assessment strategies are necessary for teachers to cater for different learning situations and to realize a new pedagogical paradigm. Conclusion: Discussions are made on effective building of feedback strategy and future study directions in relation to the application of appraisal theory in oral English classrooms.
