603 The relationship between placental lncRNA H19/miR-675/PPARα and non-gestational diabetes mellitus macrosomia

Objective: To investigate the relationship between placental lncRNA H19/miR-675/PPARα and non-gestational diabetes mellitus (GDM) macrosomia. Methods: A nest-case control study was performed. Full term neonates with birth weight ≥4 000 g in Obstetrics Department of the Second Affiliated Hospital of Wenzhou Medical University from 2014 to 2016 were collected continuously as the macrosomia group, and normal birth weight neonates between 2 500 and 3 999 g were recruited within 3 days from the birth of macrosomia as the control group. Demographic data and placental tissue samples of the two groups were collected. The mRNA expression
level of placental lncRNA H19, miR-675 and PPARα was detected by qPCR. Multivariable logistic regression was used to analyze the relationship between placental lncRNA H19, miR-675, PPARα expression and non-GDM macrosomia. The regulatory effect of lncRNA H19/miR-675 on PPARα was further verified in human chorionic trophoblast cells (HTR-8/SVneo). Results: Non-GDM macrosomia group had lower expression level of placental lncRNA H19 and miR-675 but higher mRNA expression level of PPARα than the control group (P<0.05). Logistic regression analysis showed that low lncRNA H19 and miR-675 and high PPARα expression in placenta increased the risk of non-GDM macrosomia (lncRNA H19: OR=3.69, 95%CI=1.28-10.65; miR-675: OR=3.32, 95%CI=1.20-9.19; PPARα: OR=4.71, 95%CI=1.63-13.57). After the knowckdown of lncRNA H19 in HTR-8/SVneo cells, miR-675 was decreased and the mRNA and protein expression of PPARα was upregulated. However, when lncRNA H19 was knocked down and miR-675 was overexpressed at the same time, the upregulation of mRNA and protein expression of PPARα was suppressed. Conclusion: Low expression of placental lncRNA H19/miR-675 can increase the risk of non-GDM macrosomia and high expression of placental PPARα, which may be partly regulated by lncRNA H19/miR-675, can increase the risk of non-GDM macrosomia.

609 Lipid emulsion rescues bupivacaine-induced myocardial toxicity by activating autophagic flux

Objective: To explore the role of autophagy in lipid emulsion to rescue myocardial toxicity of bupivacaine. Methods: The logarithmic growth of H9C2 cardiomyocytes were randomly divided into four groups: control group (DMSO group), bupivacaine group (Bup group), lipid emulsion group (Lip group), lipid emulsion+bupivacaine group (BLp group). After 24 hours of treatment with corresponding drugs, CCK8 was used to detect cell proliferation inhibition rate, cell morphology was observed under light microscope, cell autophagy was observed under electron microscope, and autophagy flow was detected under laser scanning confocal microscope. And we detected the expression of LC3II/I protein and p62 protein by Western blot and the expression of MAP1LC3 mRNA and p62 mRNA by RT-PCR in H9C2 cardiomyocytes. Results: Compared with the DMSO group, the Bup group significantly increased the myocardial cell proliferation inhibition rate (P<0.05),showed a large number of cell death and shrinkage, inhibited of autophagic flow, obviously increased of LC3II/I and p62 protein and mRNA expression (P<0.05). Compared with the Bup group, the BLp group and Lip group significantly reduced the myocardial cell proliferation inhibition rate (P<0.05), improved cardiomyocyte damage,promoted autophagic flow and accelerated the removal of autophagosomes, down regulated the expression of LC3II/I and p62 protein and mRNA (P<0.05). Conclusion: Lipid emulsion rescues bupivacaine-induced myocardial toxicity by activation autophagic flow, which accelerates the removal of autophagosomes.

615 Design and synthesis of 4-phenylbutyric acid analogues and their inhibition effect on the endoplasmic reticulum stress and over-autophagy in PC12 cells

Objective: To design and synthesize 4-phenylbutyric acid (PBA) analogues and explore their dual inhibition effect on the endoplasmic reticulum stress (ERS) and excessive autophagy in PC12 cells.Methods: Compounds (A1-A11) were designed and synthesized based on the structure of PBA whose chemical structures were confirmed by 1H NMR, 13C NMR and MS. MTT was used to analyze the protective effect of A1-A11 on endoplasmic reticulum injury model of PC12 cells induced by TG and injury model of PC12 cells induced by H2O2; Western blot was used to measure the levels of glucose regulated protein 78 (GRP78),LC3 II/I and Beclin-1. Results: Compounds A3, A5, A7 and A9-A11 were new compounds. MTT assay showed that A1 displayed good protective effect on endoplasmic reticulum stress injury model (P<0.05). A1, A2, A4 and A6-A10 indicated great protective effect on autophagy injury model (P<0.05). Western blot showed that A1 could significantly inhibit the expression of GRP78, LC3 II/I and Beclin-1 (P<0.05). Conclusion: Compared with PBA,A1 performs excellent dual protective effect on ERS and autophagy injured model.

623 The stress and strain of the second stage vaginal delivery: an analysis based on three-dimensional model of pelvic floor muscle

Objective: To explore the possible degree and location of pelvic floor muscle injury caused by different fetal biparietal diameter during the second stage of vaginal delivery. Methods: Data of female pelvic floor magnetic resonance imaging (MRI) were collected and the pelvic floor muscle model was re-established by 3D modeling. Then the responding mechanism of pelvic floor muscle in the second labor process was investigated by finite element method. Stress analysis, strain analysis and maximum displacement analysis were made based on the simulation of three different fetal head models. Results: Three kinds of fetal biparietal diameter models were successfully established through the three-dimensional model of pelvic floor muscle, which was respectively 80 mm (D80), 90 mm (D90) and 100 mm (D100). The stress analysis showed that the maximum equivalent stress of the D80, D90 and D100 were 0.0782, 0.0804 and 0.2509 MPa respectively at 0.15 s. The larger the biparietal diameter of fetus, the greater the maximum equivalent stress, with the maximum principal stress of pelvic floor muscle and the peak value at the attachment of the pubococcygeus to the skeleton. The strain analysis showed that the principal strain value of D80, D90 and D100 was 0.3215, 0.3336 and 0.2710 MPa at 0.15 s, respectively.The larger the biparietal diameter of the fetus, the greater the maximum principal strain of the pelvic floor muscle,with the peak value in the pubic coccygeus muscles, or at the place where the pelvic floor muscle attached to the skeleton. The maximum displacement analysis showed that the maximum displacement difference in Z direction of D80, D90 and D100 models was 50.36, 50.68 and 52.51 mm respectively, and the maximum displacement in X direction was 127.11, 140.21 and 129.38 mm respectively. The displacement nephogram showed that the whole pelvic floor muscle had a huge deformation in the second stage of labor, in which levator ani muscle was stretched obviously, which was 128.90%-131.27% in the vertical direction (mainly the pubococcygeal muscle) and 181.59%-200.30% in the horizontal direction. Conclusion: In the second stage of vaginal delivery, the larger the biparietal diameter of the fetus, the greater the maximum equivalent stress and the maximum principal stress of the pelvic floor muscle, the greater the maximum principal strain, with the peak value all appearing at the attachment ofthe pubococcygeus to the skeleton. In the second stage of labor, the whole pelvic floor muscle had a huge deformation, and the levator ani muscle, especially the pubococcygeus part, was the most stretched muscle.

632 The effect of lncRNA TTN-AS1 targeted inhibition of miR-1271 expression on the proliferation, invasion and apoptosis of lung cancer cells and its mechanism

Objective: To investigate the effect of lncRNA TTN-AS1 on the proliferation, invasion and apoptosis of lung cancer cells by regulating miR-1271 expression. Methods: Human lung cancer A549 cells were transfected with TTN-AS1 siRNA or miR-1271 mimics. QRT-PCR was used to detect the expression of TTNAS1 and miR-1271, CCK-8 test to detect cell proliferation, Transwell test to detect cell invasion, flow cytometry to detect cell apoptosis and Western blot to detect the expression of PI3K, p-AKT, PCNA, E-cadherin and cleaved caspase 3. miR-1271 inhibitors and TTN-AS1 siRNA were co-transfected and the effect of miR-1271 inhibition on the proliferation, invasion and apoptosis of A549 cells induced by TTN-AS1 was observed. The dual luciferase reporter gene experiment was performed to detect the targeting effect of TTN-AS1/miR-1271 and miR-1271/PDK1. The effect of over-expression or inhibition of TTN-AS1/miR-1271 on PDK1 expression was analyzed by Western blot. Results: Inhibition of TTN-AS1 and over-expression of miR-1271 could significantly inhibit the proliferation and invasion of A549 cells, promote apoptosis, down-regulate the expression of PI3K, p-AKT and PCNA, and up-regulate the expression of E-cadherin and cleaved caspase 3 (P<0.05). Inhibition of miR-1271 could reverse the inhibitory effect of TTN-AS1 on the proliferation, invasion of A549 cells and the promotion of apoptosis as well. Both TTN-AS1 and PDK1 had a targeted regulatory relationship with miR-1271. Conclusion:lncRNA TTN-AS1 can inhibit the proliferation and invasion of A549 cells and promote apoptosis by targeting miR-1271/PDK1 and inhibiting PI3K/AKT signaling pathway.

639 The value of circulating exosomes-mediated lncRNA HOXA11-AS in the proliferation of vascular endothelial cell by up-regulating SOX4

Objective: To explore the effect of circulating exosomes on the proliferation ability of endothelialm cells and its possible mechanism. Methods: The plasma of patients with in-stent restenosis (ISR, n=199) and Non-ISR (n=32) was collected and the exosomes in the plasma were extracted. The exosomes were used to intervene the endothelial cell proliferation inhibition model induced by rapamycin. The cell proliferation ability of each group was measured by MTT method. The expression of PCNA and Ki67 in cells was measured by Western blot. The gene chip was used to detect the difference in non-coding RNA of the two groups of exosomes, and qRT-PCR was used to detect the expression of lnc HOXA11-AS. Results: Compared with the control group, the cell proliferation ability of the rapamycin group was weakened, and the expression of PCNA and Ki67 decreased (P<0.05); compared with the rapamycin group, the endothelial cell proliferation ability of the control group increased after exosomal intervention, and the expression of PCNA and Ki67 increased (P<0.05). The microarray results showed that the expression of lnc HOXA11-AS was reduced in ISR exosomes. pHOXA11-AS increased the proliferation of endothelial cells as well as the expression of PCNA, Ki67 and SOX4. Conclusion: Lnc HOXA11-AS in plasma exosomes may increase the proliferation ability of endothelial cells by up-regulating the expression of SOX4.

646 Comparison of two palatoplasty with intravelar veloplasty technique in children with cleft palate

Objective: To compare the clinical effect of two palatopalasty techniques with traditional and radical intravelar veloplasty for children older than 2 years old with cleft palate. Methods: The clinical data of 220 patients after primary palatoplasty in West China Stomatology Hospital of Sichuan University from 1985 to 2013 were retrospectively analyzed. The incidence of postoperative velopharyngeal insufficiency and palatal fistula were compared between the two groups. Meanwhile, the factors associated with velopharyngeal insufficiency were analyzed by hierarchical analysis and multiple logistic regression analysis. Results: There was no significant difference between the two study groups in the rate of velopharyngeal insufficiency and oronasal fistula (P>0.05). Nevertheless, in the RIVV group, the rate of velopharyngeal insufficiency of 2-to-5-year group was significantly lower than the group ≥5 years old (P<0.05). Logistic regression analysis showed that age at palatoplasty was the only most important risk factor of velopharyngeal insufficiency (OR=1.107, 95%CI=1.037-1.181, P=0.002). Logistic regression analysis of TIVV subgroup showed no risk factor of velopharyngeal insufficiency. Furthermore, logistic regression analysis of RIVV subgroup showed that age at palatoplasty was the risk of VP insufficiency (OR=1.102, 95%CI=1.025-1.185, P=0.009). Conclusion: The radical intravelar veloplasty was not significantly superior to the traditional intravelar veloplasty for cleft palate children older than 2 years. Moreover, the RIVV technique may be more suitable for those aged 2 to 5 years old.

652 Current status and influencing factors of fear of hypoglycemia in empty-nesters with type 2 diabetes mellitus

Objective: To investigate the current status and influencing factors of fear of hypoglycemia (FOH) in empty-nesters with type 2 diabetes mellitus. Methods: A total of 193 empty-nesters with type 2 diabetes mellitus selected from the First Affiliated Hospital of Wenzhou Medical University from August to December 2020 were investigated by convenience sampling. The demographic questionnaire, Chinese version of the fear of hypoglycemia scale 15, social support rating scale and self-rating anxiety scale were used for investigation.Results: The mean score of FOH for the empty-nesters with type 2 diabetes mellitus was (21.82±10.41), and the incidence of FOH was 19.2%. Binary logistic regression analysis showed risk factors of FOH included female(OR=3.453, 95%CI=1.250-9.537, P=0.017), primary school education (OR=3.972, 95%CI=1.170-13.483, P=0.027), oral therapy combined with insulin therapy (OR=4.701, 95%CI=1.339-16.507, P=0.016), the frequency of hypoglycemia (OR=4.336, 95%CI=1.582-11.883, P=0.004), and anxiety (OR=1.095, 95%CI=1.029-1.164,P=0.004). Conclusion: Attention should be paid to the assessment of FOH with relevant intervention for the empty-nesters with type 2 diabetes mellitus to improve their quality of life.

658 Clinical observation of metformin combined with low-dose aspirin in the treatment of patients with recurrent abortion and insulin resistance

Objective: To observe the clinical efficacy of metformin combined with low-dose aspirin in the treatment of patients with recurrent abortion and insulin resistance. Methods: In this study, 90 patients with recurrent abortion and insulin resistance were randomly divided into three groups: basic intervention group,metformin group (basic intervention & metformin group), and observation group (basic intervention & metformin &low-dose aspirin group). After 3-month treatment, body mass index (BMI), homeostasis model insulin resistance index (HOMA-IR), plasminogen activator inhibitor (PAI-1), homocysteine (Hcy), fasting insulin (FINS) level and pregnancy rate within 6 months after treatment were evaluated and compared among the three groups. Results:Compared with indexes before treatment, BMI, HOMA-IR and FINS were decreased in the three groups after treatment, with statistical difference (P<0.05). There was significant difference between the observation group and the basic intervention group (P<0.05), but there was no statistical difference between the observation group and the metformin group (P>0.05). After treatment, serum PAI-1 level and Hcy level decreased in three groups,especially significant in the observation group, and compared with the metformin and basic intervention group,the difference was significant (P<0.05). The pregnancy rate of the observation group and the metformin group was significantly different from that of the basic intervention group within 6 months after treatment (P<0.05),but there was no significant difference between the two groups (P>0.05). Conclusion: Metformin combined with low-dose aspirin can improve insulin resistance, reduce serum PAI-1 and Hcy levels and improve clinical pregnancy rate in patients with recurrent abortion and insulin resistance, with fewer adverse reactions.

662 Correlation analysis of T2 value of articular cartilage and cartilage injury in gouty knee arthritis

Objective: To explore the correlation between T2 value of the knee joint cartilage and the degree of articular cartilage damage through a comparative study of patients with gouty arthritis and healthy volunteers.Methods: A total of 47 patients who were clinically diagnosed with gouty knee arthritis (56 cases) were selected as the case group, and 15 healthy volunteers (30 cases) without knee disease were assigned to the control group.According to the ICRs grading standard, two radiologists evaluated the performance of gout and the grading of articular cartilage injury on the conventional sequence of the knee joint. The ICRs I-II grade was classified as
mild and the ICRs III-IV grade as severe cartilage injury. The cartilage T2 values of patella (patella), femoral trochlear (FT), medial femoral condyle (MFC), lateral femoral condyle (LFC), medial tibial plateau (MTP) and lateral tibial plateau (LTP) were measured respectively. The difference in T2 values between gouty knee arthritis patients and normal control group was compared, and the consistency between the two groups was verified.Results: The average value of cartilage T2 measured in all parts of the knee joint was significantly higher than that in the control group (P<0.05); the consistency between groups and within groups was good. Conclusion:The T2 value of articular cartilage in patients with gouty knee arthritis was significantly higher than that in the control group.

666 Effect of inflatable insulation system on laminar flow and total bacterial count in air of operating room

Objective: To investigate the effect of inflatable insulation system on laminar wind speed, wind direction and total bacterial count in the air of the operating room. Methods: From March 2020 to December 2020, 76 simulated patients who were operated in vertical laminar flow operating room were selected as subjects and randomly divided as insulation laminar flow group and laminar flow group, with 38 cases in each. In the thermal insulation laminar flow group, inflatable thermal insulation blanket was used as a measure to simulate the patient’s thermal insulation during the whole perioperative period. Three dimensional ultrasonic anemometer was used to measure the wind speed and direction of laminar flow in the two groups at 64 points around the operating bed half an hour after laminar flow opening and after air sampling. Air samples were taken before the operation,30 min, 60 min, 90 min and 120 min after the operation, with an analysis of the changes in total bacterial count in the air of laminar flow of the two groups. Results: The wind direction of laminar flow was downward at different time points of the two groups, and there was no significant difference in the wind speed between the two groups (P>0.05) 30 min after laminar flow was opened. After air sampling, the insulation laminar flow group had lower wind speed than the laminar flow group (P<0.05). Further subgroup analysis showed no significant difference in the wind speed of the non head area between the two groups (P>0.05), but the wind speed of the head area of the insulation laminar flow group was lower than that of the laminar flow group (P<0.05). The total number of bacteria in the air of the two groups increased with the extension of operation time. There was no significant difference in the total number of bacterial colonies in the air of the operating room between the two groups before operation,30 min, 60 min, 90 min and 120 min after operation (P>0.05). Conclusion: Set at 38 ℃, the inflatable insulation system has no obvious influence on the wind direction of laminar flow or the total number of bacteria in the air,although it affects the laminar wind speed in the operating room to a certain degree.

670 Single-center clinical study of cryptococcal neoformans meningitis after renal transplantation: a retrospective analysis

Objective: To analyze the clinical characteristics and difficulties in the diagnosis and treatment of cryptococcal neoformans meningitis in renal transplant patients. Methods: Clinical data of patients with Cryptococcal meningitis after kidney transplantation in the First Affiliated Hospital of Wenzhou Medical University from June 2014 to September 2019 were collected and analyzed retrospectively. Results: Of 320 cases after kidney transplantation, 9 were infected with cryptococcal meningitis,with first symptoms including fever (n=7), headache (n=4), cough (n=2), blurred vision (n=2) and consciousness disorder (n=1). The average time of infection after kidney transplantation was (1.6-14) years and the time from onset to confirmed diagnosis was 8-42 D. CSF culture was positive in 8 cases (88.9%). On the basis of tacrolimus+mortymecofenate+meponyolone (TAC+MMF+PRED) immunosuppressive regimen, antifungal therapy such as 5 fluorocytosine (5FC) 5G/D+amphotericin B liposome (L-AmB) 3 mg·kg-1D-1 were given for (5-11.5) months. After 6-month follow-up, 5 cases had complete remission (n=5), 5 partial remission (n=5) and 1 case died (n=1). Conclusion: Cryptococcus infection after kidney transplantation is characterized by a variety of clinical manifestations, long incubation period and long course of disease. Therefore, early detection, early diagnosis, multi-department cooperation and adequate standardized treatment are the key to the successful treatment of cryptococcal meningitis.

674 An analysis of Johanson-Blizzard syndrome characterized by diabetes mellitus

Objective: To investigate the clinical phenotype and genotype of Johanson-Blizzard syndrome (JBS) with diabetes mellitus as the main manifestation. Methods: The clinical data, laboratory examination and imaging examination of JBS patients were collected. The genomic DNA of their family members was extracted and verified by whole-exome sequencing and then Sanger sequencing. Results: The main manifestation of the patient was diabetes mellitus, with enlarged eye space, flat nasal root, nasal alae aplasia, and low hairline.Sequencing result showed that the patient carried a homozygous missense variant of c.4463T>C (p. Ile1488Thr) of the UBR1 gene, verified as a novel variation, and analysis showed it was the pathogenic site. Conclusion: The finding that the patient carries on a new UBR1 gene c.4463T>C has improved the understanding of the clinical phenotype and the genetic spectrum of JBS.

Objective: To analyze the risk factors of leukoaraiosis. Methods: A total of 188 inpatients were selected from the Department of Neurology, Wenzhou People’s Hospital from July 2019 to December 2020.According to the presence or absence of leukoaraiosis (LA), the patients were divided as case group (78 cases) and control group (110 cases). Age, gender, hypertension, diabetes, blood biochemical, and microalbuminuria indexes of both groups were collected. The univariate comparison was conducted in the data of the two groups,and multivariate logistic regression analysis was made of the significant factors in the univariate comparison.Results:Univariate analysis showed statistical difference (P<0.05) between the two groups, with the case group significantly higher positive rate of age, homocysteine, hypertension and microalbuminuria than the control group.Multivariate logistic regression analysis showed that age (OR=1.163, 95%CI=1.107-1.221),microalbuminuria (OR=2.599, 95%CI=1.132-5.966), and hypertension (OR=2.767, 95%CI=1.223-6.261) remain to be independent risk factors for leukoaraiosis. Conclusion: Age, microalbuminuria and hypertension were independently related to leukoaraiosis.
