689 The association of mitochondrial tRNATyrA5834G mutation with Leber’s hereditary optic neuropathy in two Chinese families

Objective: To investigate the possible relationship between mitochondrial tRNATyrA5834G mutation and Leber’s hereditary optic neuropathy (LHON). Methods: Clinical data were collected from two pedigrees, and detailed ophthalmological examination and mitochondrial genomes were conducted for the two proband members.Detailed eye examinations mitochondrial whole genome analysis phylogenetic analysis and haplotype analysis were performed on probands and other members of the pedigrees. Results: The clinical symptoms and ophthalmological examinations of the probands were in line with typical LHON manifestations,
and other members of the family were not abnormal. The two pedigrees did not carry the three primary mitochondrial mutation sites of ND1 G3460A and ND4 G11778A and ND6 T14484C. The polymorphic mutation sites belonged to the East Asian haplotype M7b. Both probands carried highly conserved A5834G mutation sitesamong which the conservative coefficient of A5834G in 17 species was 87.5%. Conclusion: The mitochondrial tRNATyrA5834G mutation may be the mtDNA mutation site associated with LHON and the low penetrance rate suggests that other factors (including nuclear modified genes environment etc.) may affect the phenotypicexpression of these two families.

695 Optimized preparation method for decellularized skin flap

Objective: To prepare decellularized skin flaps by auxiliary shaking method and test their biocompatibility. Methods: After taking the skin flap of the lower abdominal wall of adult rats to remove blood, a magnetic stirrer was used to perform auxiliary shaking to remove cells. The biological characteristics of the harvested scaffolds were obtained by quantitative analysis of DNA, detection of SDS residues, and scanning electron microscopy. Then take the decellularized skin flap specimens embedded subcutaneously on the back of adult SD rats, count the number of new blood vessels to detect its biocompatibility, and the proportion of regulatory T lymph in the peripheral blood. Results: Compared with the control group, the DNA content of the decellularized group decreased by more than 95%; no obvious nucleus was seen in DAPI staining; scanning electron microscope and HE staining showed that a large amount of extracellular matrix remained in the decellularized group; SDS residues were far below the biological safety level; angiography showed that the branches of blood vessels were complete and clear, and the number of new blood vessels was significantly higher than that in the control group (P<0.05). The results of animal experiments showed that in decellularized group the number of blood vessels was regenerated and the proportion of regulatory T lymphocytes increased (P<0.05). Conclusion: A protocol for preparing decellularized skin flaps with good biocompatibility and complete stent structure is provided.

700 The correlation of adipocyte fatty acid binding protein and leptin in umbilical cord blood with non-GDM macrosomia

Objective: To investigate the correlation of AFABP and leptin in umbilical cord blood with non-GDM macrosomia. Methods: A case-control study was performed. Newborns were selected, with 46 macrosomia  and 46 normal-birth-weight, in the Obstetric Department of the Second Affiliated Hospital & Yuying Children’s Hospital of Wenzhou Medical University. A self-designed questionnaire was used to collect maternal and fetal baseline information. The umbilical cord blood concentration of AFABP and leptin was measured by ELISA and RIA, respectively. Logistic regression analysis was used to analyze the influencing factors of non-GDM macrosomia. Results: There was no difference in cord blood AFABP concentration between the two groups, and the leptin concentration in macrosomia cord blood was significantly higher than that of control group (P<0.05). Stratified analysis found that in the higher gestational layer (gestational age>39) and excessive gestational weight gain layer, AFABP concentration in macrosomia cord blood was significantly lower than that of the control group (P<0.05); the leptin concentration in macrosomia cord blood was significantly higher than that of the control group in the male fetal layer, lower gestational age layer (gestational age≤39) and normal gestational weight gain layer(P<0.05). Multivariable logistic regression analysis showed that the lower AFABP concentration in umbilical cord blood could increase the risk of non-GDM macrosomia (OR=3.792, 95%CI: 1.266~11.359, P<0.05), the occurrence of non-GDM macrosomia was also influenced by fetal gender, gestational age and gestational weight gain. Conclusion: Lower AFABP concentration in umbilical cord blood may increase the risk of non-GDM macrosomia. Further studies are required to elucidate its molecular mechanism.

705 Preparation of anti-infective and bactericidal dual-functional multilayer membrane structure

Objective: To study an implant coated with a (HA/CHI-Van)5 multilayer membrane structure to prevent implant infection. Methods: The HA and CHI-Van solution were assembled  to prepare a (HA/CHI-Van)5 multilayer film structure, and the assembly process was detected by zeta potential. The antibacterial adhesion function of the multilayer film was evaluated by a live bacterial staining test. And in vitro antibacterial effect was also tested by jailbreak experiments and FE-SEM experiment. Most importantly, adequate animal experiments were designed besides X-ray, Micro-CT and bone tissue sections were to discuss in vivo experiments. Results: Each time a different monolayer was assembled, the zeta potential alternated, demonstrating the successful  assembly of the multilayer film. Live bacteria staining experiments showed that CHI in the multilayer membrane structure had a certain anti-bacterial adhesion effect. The jailbreak experiment and the FE-SEM experiment proved that the (HA/CHI-Van)5 multilayer membrane structure had a good antibacterial effect in vitro. The X-ray  score of (HA/CHI-Van)5 group was 0.7, while the score of blank group was 12.3. (HA/CHI-Van)5 group was superior to blank group in bone mineral density, trabecular bone number, relative bone volume, and bone tissue section, and was very close to sham operation group. Conclusion: By applying a (HA/CHI-Van)5 multilayer membrane structure in the implant surface, a certain anti-bacterial adhesion and good antibacterial effect can be obtained, and the implant infection can be prevented.

712 The protective effect of sodium propionate on the colon tissues in sepsis rats by inhibiting NLRP3

Objective: To explore the protective effect of sodium propionate (SP) on colon tissue of septic rats and study its relationship with NLRP3 inflammasomes. Methods: Animal models of sepsis were established by cecum ligation and puncture (CLP) and randomly divided as sham operation group, model group, model+SP group and sham operation+SP group. Rats of each group were sacrificed 24 hours after modeling, and colonic tissues were collected. HE staining was used to observe the changes of colonic structure in each group of rats and ELISA kit to detect IL-6, TGF-β and TNF-α levels in colon tissue. Western blot and immunohistochemical staining were used to detect the expression of claudin-1 and occludin and NLRP3 inflammasomes. The same method was used to establish a sepsis model, and the 72 h survival rate of each group of rats was recorded. Results: SP treatment could improve the survival rate and reduce pathological damage in the model group. The levels of IL-6, TGF-β and TNF-α in colon tissue of model+SP group were lower than those of the model group. Compared with the model group, the expression of claudin-1 and occludin in the colon tissue of the model+SP group increased. The expressions of NLRP3, caspase-1, IL-β and IL-18 in colon tissue of model+SP group were significantly lower than those of the model group. Conclusion: SP can improve the survival rate of sepsis rats. In addition, SP reduces colon injury by increasing the expression of tight junction proteins in the colon tissue of sepsis rats and inhibiting the activation of NLRP3 inflammatory inflammasomes.

717 Prediction of the histological grades of liver fibrosis and cirrhosis by quantitative analysis of gadoxetic acid contrast-enhanced MRI

Objective: To evaluate and predict histological grades of liver fibrosis and cirrhosis by Gd-EOB-DTPA-enhanced MRI. Methods: Totally 55 patients were recruited in this study, of which 10 cases without liver cirrhosis were divided as the control group, 10 cases with liver fibrosis as the liver fibrosis group, 35 cases with liver cirrhosis as the liver cirrhosis group. Gd-EOB-DTPA-enhanced MRI was performed in all 55 patients before surgery. The signal absolute enhancement value (AEV) and contrast enhancement ratio (CER) was analyzed quantitatively in the unenhancement and hepatobiliary phase of background liver. Then we analyzed the correlation among the signal AEV, the CER and histologic grade of liver fibrosis and cirrhosis. Results: The analysis of the correlation between the signal AEV and the histological grades of different diseased hepatic tissue (r=-0.526, P<0.001) and the correlation between CER and the histological grades of different diseased hepatic tissue (r=-0.741, P<0.001). Between the two analyzed methods of signal intensity, we confirmed the more significant negative correlation between signal CER and the histological grades of different diseased hepatic tissue. The cut-off values calculated by receiver operating characteristic curve (ROC curve) were 0.76 between normal hepatic
tissue and liver fibrosis, 0.46 between liver fibrosis and liver cirrhosis, respectively. Conclusion: The more significant negative correlations between CER and the histological grades of different diseased hepatic tissue were confirmed by quantitative analysis of gadoxetic-acid contrast-enhanced MRI and the analysis of CER can evaluate and predict the histological grades of liver fibrosis and cirrhosis.

723 CTA analysis of Willis’ circle classification and its association with anterior communication artery aneurysms

Objective: To analyze the relationship between classification of Willis circle (CoW) and aneurysm of anterior communicating artery (ACoA) based on cranial computer tomography angiography (CTA). Methods: A total of 1 184 inpatients who underwent both digital subtraction angiography (DSA) and cerebral CTA in the First Affiliated Hospital of Wenzhou Medical University from September 2006 to January 2016 were retrospectively analyzed. Patients with ACoA aneurysms were included in the ACoA aneurysm group (n=630), and patients without intracranial aneurysms were included in the control group (n=554). CoW is divided into anterior circulation and posterior circulation, which are further divided into 10 anatomical subtypes: a, b, c, d, e, f, g, h, i, j. All patients in the two groups were counted the number of each type of anterior and posterior circulation according to anatomical classification. The incidence of ACoA aneurysm was compared among different anatomical types. Results: The incidence of subtype g of anterior circulation in the control group was significantly higher than that in the ACoA aneurysm group. The incidence of h-type anterior circulation in ACoA aneurysm group was significantly higher than that in the control group. The incidence of type a, c and d posterior circulation in the control group was significantly higher than that in the ACoA aneurysm group. The incidence of type g, h and i posterior circulation in ACoA aneurysm group was significantly higher than that in the control group. Conclusion: The occurrence of ACoA aneurysm is related to the anatomical variation of CoW.

728 The effect of tamoxifen on ER- GPER+ breast cancer cell EMT and its mechanism

Objective: To investigate the effect of TAM on EMT in ER- GPER+ breast cancer cells and to discuss its signaling mechanism. Methods: ER- GPER+ breast cancer MDA-MB-468 cells were researched object. Cells were treated by TAM, intracellular GPER activity were inhibited by GPER specific inhibitor G15, GPER protein expression was interfered by siRNA , cell invasion ability was detected by invasion chamber experiment and western blot experiment was used to detect ERα, GPER, Vimentin, E-cadherin and N-cadherin protein expression changes. Results: TAM (5 μmol/L) could significantly enhance the invasion ability of MDA-MB-468 cells (P<0.01), and G15 (10 μmol/L) pretreatment could significantly inhibit TAM-induced cell invasion (P<0.01). GPER-siRNA could significantly interfere with the expression of GPER protein in MDA-MB-468 cells (P<0.01), and interference with GPER protein expression could effectively inhibit TAM-induced cell invasion (P<0.01). TAM treatment significantly induced up-regulation of Vimentin and N-cadherin protein expression (P<0.01) and down-regulation of E-cadherin protein expression (P<0.01). G15 pretreatment or interference with GPER protein expression could inhibit the above induction of TAM (P<0.05). Conclusion: TAM can induce EMT in ER-GPER+ breast cancer MDA-MB-468 cells, which is related to the activation of GPER.

733 Application of different glomerular filtration rate assessment formulas in type 2 diabetic patients

Objective: To evaluate the different glomerular filtration rate assessment formulas in type 2 diabetic patients. Methods: The data of 322 patients with type 2 diabetes mellitus were analyzed retrospectively, the glomerular filtration rate calculated by 24-hour creatine clearance rate was rGFR, eGFR was calculated by C-G formula, simplified MDRD formula, and the improved MDRD China formula and CKD-EPI formula, to compare the accuracy, accuracy, deviation, ROCAUC of each eGFR, and hierarchical analysis at different ages and glycosylated hemoglobin. Results: The correlation coefficient between GFR estimated by CKD-EPI formula and rGFR was the highest, r=0.788. The P15 and P30 of CKD-EPI formula were the highest, which are 48.1% and 73.6%, respectively. Bland-Altman diagram showed minimum deviation between simplified MDRD formula and rGFR, CKD-EPI formula had the highest accuracy. The area under ROC curve of MDRDC formula and CKD-EPI formula in the diagnosis of chronic renal insufficiency was equal and the largest. In the group of age≥60 years old and the group of 18<age<60 years old, the correlation of CKD-EPI formula was the best, P30 and P50 were the highest, and the highest accuracy. CKD-EPI formula had the best P30 and P50, the highest accuracy in different glycosylated hemoglobin. Conclusion: The CKD-EPI formula was most suitable for the estimation of GFR in Chinese patients with type 2 diabetes mellitus at the same time under different ages and glycosylated hemoglobin.

738 The effect of new rehabilitation management model on motor symptoms of patients with Parkinson’s disease

Objective: To observe the effect of the new management model on the motor symptoms of early Parkinson’s disease patients. Methods: Totally 120 patients with Parkinson’s disease who met the criteria (Hoehn-Yahr classification criteria of Phase I and II) were randomly divided into the treatment group and the control group, with 60 cases in each. The control group was treated with pure western medicine (Madopar 125 mg, 3/Day, 1 h oral treatment before meals), while the treatment group adopted a new Parkinson’s disease rehabilitation management model on the basis of the control group (plus Parkinson’s disease rehabilitation exercise treatment, 30 min, 5/week each training; to recruit these patients into Parkinson’s disease rehabilitation club and involve them in club activities regularly). Both groups were treated for 12 weeks. All patients were assessed before and after treatment using the Unified Parkinson’s Disease Assessment Scale (UPDRS, III) and Berg Balance Scale. Results: The score of UPDRS III and Berg in the treatment group was significantly improved (P<0.01), and the score of UPDRS III in the control group was significantly different from that before treatment (P<0.01), with the score of Berg significantly improved (P<0.05). The UPDRS III score was better than that of the control group (P<0.05), and the Berg score was significantly different from that before treatment (P<0.01). The results of further comparison between the scores of UPDRSIII items before and after treatment showed that there was no statistical differences in verbal expression, facial expression, and rigidity score between the treatment groups before and after treatment (P>0.05). In the treatment group, however, resting tremor, hand movement or posture tremor, finger tapping test, hand movement, rotation movement, leg flexibility, standing, posture, gait, posture stability, and body movement scores were better than those of the pre-treatment with significant differences (P<0.05). Conclusion: The new management model of Parkinson’s disease rehabilitation can improve the PD patients’ motion control ability, balance ability and the posture stability.

742 The roles of baicalein in proliferation inhibition in gastric cancer cells and its molecular mechanisms

Objective: To investigate the function and molecular mechanisms of baicalein in inhibiting the proliferation of gastric cancer cells. Methods: CCK-8 assay was used to detect the baicalein at different concentrations and its effect on the proliferation of gastric cancer cells (MGC80-3, HGC-27, and BGC-823), and the effect of nicotinic acid receptor (GPR109A) overexpression on the viability of gastric cancer cells. Transwell migration and invasion assay was used to detect the effects of baicalein at different concentrations on the migration and invasion of gastric cancer cells in vitro. RNA interference assay was used to detect the effect of silencing GPR109A expression on the proliferation of gastric cancer cells inhibited by baicalein. Results: Baicalein significantly inhibited proliferation, migration and invasion of gastric cancer cells in vitro (P<0.05). Overexpression of GPR109A also markedly inhibited proliferation of gastric cancer cells (P<0.05). Moreover, the expression of GPR109A silenced by RNAi could block the inhibitory effect of baicalein on the proliferation of gastric cancer cells (P<0.05). Conclusion: Baicalein can significantly suppress the proliferation of gastric cancer cells in vitro. The inhibitory effect of baicalein may be attributed to the upregulation of GPR109A protein expression in gastric cancer cells.

748 Clinical application of dexmedetomidine combined with ropivacaine for programmed intermittent epidural bolus in labor analgesia

Objective: To discuss the clinical application effect of dexmedetomidine combined with ropivacaine for programmed intermittent epidural bolus (PIEB) in labor analgesia. Methods: Sixty primiparas  admitted to the First Affiliated Hospital of Wenzhou Medical University from Sep 2018 to August 2019 were randomly divided into two groups: fentanyl group (Fen group, n=30) received epidural 2 μg/mL fentanyl and 0.1% ropivacaine, whereas the other dexmedetomidine group (Dex group, n=30) received epidural 0.5 μg/mL dexmedetomidine and 0.1% ropivacaine. Both groups were given epidural analgesic solution of 8 mL every 60 minutes respectively. The heart rate (HR), fetal heart rate (FHR), and mean arterial pressure (MAP) were assessed at the beginning of analgesia (T0), 15 min (T1), 30 min (T2), 1 h (T3), 2 h (T4) after analgesia, beginning of the second stage (T5), and 1 h after delivery (T6). Pain visual analogue scale (VAS), Ramsay sedation score, and modified Bromage score were assessed. And the neonatal Apgar score of 5 min, anesthetics consumptions, first and second stage, side effects, and PCEA attempts numbers were also observed. Results: The HR, FHR, MAP, modified Bromage score, the first and second stage, and neonatal Apgar score of 5 min did not show significant difference between 2 groups (P>0.05). Compared with Fen group, The VAS of Dex group were extremely declined and the Ramsay sedation score were significantly increased at T1 and T2 (P<0.05). The anesthetics consumptions and PCEA attempts numbers of Dex group were definitely lower than Fen group (P<0.05). The side effect did not show significant difference between 2 groups (P>0.05). Conclusion: The dexmedetomidine combined with ropivacaine for pieb in labor analgesia reduces local anesthetics consumption and pain outbreak, and has better analgesic and sedative effect.

753 The correlation between MTHFR C677T gene polymorphism and abnormal liver function in pediatric acute lymphoblastic leukaemia after treatment with high-dose methotrexate

Objective: To investigate the correlation between MTHFR C677T gene polymorphism and abnormal liver function in pediatric acute lymphoblastic leukemia (ALL) after treatment with high-dose methotrexate (HD-MTX). Methods: The clinical data of 66 children with ALL treated with HD-MTX in the Second Affiliated Hospital & Yuying Children’s Hospital of Wenzhou Medical University from October 2017 to March 2020 were analyzed retrospectively. According to the genotype of MTHFR C677T, the children were divided into wild group (CC) and mutant group (CT+TT) to compare differences in incidence of toxicity, liver function indexes PA, ALT, AST, AST/ALT, TP, ALB, LDH, GGT, TBIL, CHE and blood drug concentrations at 48 h and 72 h after HD-MTX treatment. Results: Compared with the wild group, MTHFR C677T mutation group had a higher incidence of abnormal liver function (P<0.05), in which the proportion of children with abnormal levels of AST, ALT and CHE increased significantly (P<0.05), but there was no significant difference in other liver function indexes (P>0.05). Further analysis of the difference in HD-MTX concentrations between different genotypes showed that the mutant type, compared with the wild type, had higher blood drug concentrations at 48 h and 72 h (P<0.05). Conclusion: The proportion with abnormal levels of serum AST, ALT and CHE is higher for children with MTHFR C677T mutation type after treatment with HD-MTX, and detection of MTHFR C677T genotype has clinical application value in predicting abnormal liver function after HD-MTX treatment.

758 Investigation of TCM constitution of HLA-B27-associated acute anterior uveitis

Objective: To investigate and summarize the characteristics and distribution patterns of Chinese Traditional Medicine (TCM) constitution in quiescent stage of acute anterior uveitis patients with HLA-B27 positive, so as to provide more theoretical information for clinical prevention and treatment. Methods: There were 322 patients of acute anterior uveitis in quiescent stage included in this study. The participants were divided into HLA-B27(+) group and HLA-B27(-) group according to HLA-B27 gene test results, and their TCM constitution characteristics and distribution were analyzed. Results: Of all 332 patients, there were 295 cases of single constitution type and 37 cases of combined constitution with 75 physical types, which meant a total of 332 patients with 370 constitution types in this study. Of the 185 cases of HLA-B27(+) patients, the constitution type of single physical, mild physical, pathological physical and complex physical were 164, 98, 66 and 21 cases, respectively. The complex physical type contained 43 different kinds of physical types. In contrast, of the 147 cases of HLA-B27(-) patients, the constitution type of single physical, mild physical, pathological physical and complex physical were 131, 97, 34 and 16 cases, respectively. The complex physical type contained 32 different kinds of physical types. Therefore, HLA-B27(+) group had a higher proportion of pathological constitution and damp heat constitution, which showed statistical significance between them (P<0.05). Conclusion: HLA-B27-associated acute anterior uveitis is a disease with similar physical susceptibility, but in quiescent stage mild physical constitution
is dominated. Compared with HLA-B27(-) patients with acute uveitis, the pathological constitution overall proportion of patients with HLA-B27(+) patients with acute uveitis was higher, and there were more patients with damp-heat constitution.

During the outbreak of COVID-19, the First Affiliated Hospital of Wenzhou Medical University has constructed the nursing emergency organization system and set up the quality management group. By adopting multi-channel solutions and omni-trainings, the hospital tried to scientifically supervise the system to build three-dimensional safety network. What’s more, people-oriented management measures motivate nurses to get highly involved and establish the awareness of crisis to protect themselves, effectively enhance their capacity of treat and cure.

765 The effect of intensive self-efficacy nursing on self-efficacy and post-traumatic growth of postoperative patients with breast cancer

Objective: To observe the effect of intensive self-efficacy nursing interventions on self-efficacy level and post-traumatic growth level of postoperative patients with breast cancer. Methods: According to the sodium discharge standard, 81 postoperative patients with breast cancer were chosen. Then they were randomly divided into two groups, 40 patients in the observation group and 41 patients in the control group. The control group was treated with the routine nursing of the thyroid and breast-surgery while according to the characteristics of the disease and the theory of self-efficacy. Two months after the intervention, the differences in self-efficacy level and post-traumatic growth level between the two groups were compared. Results: Before the interventions, there was no statistical difference in self-efficacy level and post-traumatic growth level between the observation group and the control group (P>0.05). However, after intensive interventions, the observation group was significantly higher in the self-efficacy level and post-traumatic growth level than those of the control group (P<0.05). Conclusion: The clinical nursing mode of intensive self-efficacy can effectively improve the patients’ self-efficacy level and post-traumatic growth level.
