391 The effect of protectin DX on inflammation and fibroproliferation in primary lung fibroblasts

Objective: To explore effects of protectin DX (PDX) on the expression of cyclooxygenase-2 (COX-2), fibroproliferation, collagen production and myofibroblast differentiation in mice primary lung fibroblast cells (LF). Methods: After the lung fibroblasts were isolated and purified, the acute inflammation model and the fibriproliferation model was respectively established with lipopolysaccharide (LPS) and with TGF-β1. Then primary lung fibroblast cells were incubated in DMEM medium containing LPS or TGF-β1 in the presence or absence of PDX. After incubation, cell viability was measured using cell titer. mRNA expression was measured by realtime-PCR. The cells were harvested, and COX-2 protein was analyzed by western-blot. Supernatants from lung fibroblast cells were collected and PGE2 protein expression was measured by ELISA. Results: It is a reasonable model of fibroproliferation in fibroblasts by administering 10 ng/mL TGF-β1 for 48 hours. PDX reduced LPS-induced expression of COX-2 and production of PGE2, while inhibiting TGF-β1-induced lung fibroblast cells proliferation in a concentration dependent manner. In addition, PDX reduced rat primary LF collagen production, α-smooth muscle actin (α-SMA) and fibronectin expression induced by TGF-β1. Conclusion: PDX inhibits LPS-induced expression of COX-2 and production of PGE2 in primary lung fibroblast cells, while decreasing TGF-β1-induced  fibroproliferation, collagen production and differentiation in primary lung fibroblast cells.

397 Anti-gastric cancer activity of novel IKKβ inhibitor in vitro

Objective: To investigate the anti-gastric cancer activity of novel IKKβ inhibitor EF24 analog H18 in vitro. Methods: Molecular docking is used to simulate the binding of H18 and IKKβ. MTT assay was used to detect the effect of H18 on the growth of gastric cancer cell line BGC-823. The clonogenicity assay was employed to investigate the role of H18 in the colony formation ability of BGC-823. Cells apoptosis was observed by DAPI staining. The inhibition of the NF-κB signaling pathway was verified by Western blot. Results: H18 binds to the allosteric site of IKKβ. The IC50 of H18 to BGC-823 cells was (1.7±0.3) μmol/L at 72 h. Compared with IKK inhibitor BMS-345541, H18 significantly inhibited cell proliferation, but had no effect on apoptosis. H18 can inhibit significantly NF-κB signaling pathway in gastric cancer cells. Conclusion: H18 is a promising IKKβ inhibitor with good anti-tumor activity in vitro.

401 Expression of LINC00152 in esophageal squamous cell carcinoma and its clinical significance

Objective: To explore the expression and clinical significance of long non-coding RNA (lncRNA) LINC00152 in esophageal squamous cell carcinoma. Methods: A total of 95 esophageal squamous cell carcinoma patients from the First Affiliated Hospital of Wenzhou Medical University during January 2008 to December 2013. LncRNA microarray gene chip technology was used to detect the expression level of lncRNA in the cancer tissue of esophageal squamous cell carcinoma and corresponding paracancerous normal tissues between metastatic group and non-metastatic group, in order to find the differential expression of lncRNAs. The differential expression of lncRNA in squamous cell carcinoma was verified by qRT-PCR. The correlation between lncRNA expression level and clinical features was analyzed. The ROC curve was drawn to evaluate the efficacy of LINC00152 as a predictor of esophageal squamous cell carcinoma metastasis. Correlation data of international gene chip lncRNA database of esophageal squamous cell carcinoma were retrieved to analyze the relationship between LINC00152 expression level and survival. Results: LncRNA microarray gene chip experiments showed that there were 10 lncRNAs in esophageal squamous cell carcinoma and paracancerous normal tissue with more than 5 times differential expression. The qRT-PCR validation of esophageal squamous cell carcinoma and paracancerous normal tissue found that the expression of 3 lncRNAs (MIR205HG, LINC00152, FOXD2-AS1) were statistically different between metastatic group and non-metastatic group. Statistical analysis revealed that the expression of LINC00152 was correlated with the lymph node metastasis of esophageal squamous cell carcinoma (P<0.01). ROC curve analysis showed that the AUC of ROC was 0.781(95%CI=0.512-0.824, P=0.032). International gene chip database data of esophageal cancer showed that the high expression of LINC00152 predicted a poor survival. Conclusion: LINC00152 may be involved in the metastasis process of esophageal squamous cell carcinoma and is a new biomarker for the prognosis of esophageal squamous cell carcinoma.

408 The method and optimized condition for enriching empty plasmids transfected specific macrophages by flow cytometry sorting

Objective: To establish and optimize the method for enriching PX458 empty plasmid transfected J774A.1 cells by BD FACSAria II flow cytometer in order to figure out the optimum condition for screening J774A.1 cells. Methods: PX458 empty plasmid transfected J774A.1 cell line was constructed and stained with 7-AAD. GFP-positive transfectants of J774A.1 cells were sorted by 100 μm nozzle and initial, 4-way purity, purity, yield, fine tune, single cell sort modes respectively. The sorting efficiency was compared in different instrument parameters. Results: Under the stable fluid flow condition, the purity of the cells obtained by each sort mode of 100 μm nozzle was over 80%, and the sort mode with highest efficiency is purity mode, reaching 95.7%±1.0%. Conclusion: When the PX458 empty plasmid transfected J774A.1 cell line was shorted by BD FACSAria II flow cytometer with 100 μm nozzle, the purity sort mode is preferred.

412 Establishment of a non-invasive detection assay for three primary mitochondrial mutations associated with Leber’s hereditary optic neuropathy

Objective: To establish a non-invasive detection assay for three Leber’s hereditary optic neuropathy (LHON) families with m.11778G>A, m.14484T>C and m.3460G>A mutations. Methods: Three patients (WZ1481, WZ1478, WZ1435) with m.11778G>A, m.14484T>C and m.3460G>A mutations who visited School of Ophthalmology and Optometry, Wenzhou Medical University were included in this study. Purity and concentration of genes from buccal cell samples via manual sampling and reagent kits were determined. The whole genomic DNA of samples carrying m.11778G>A, m.14484T>C and m.3460G>A mutation with LHON as well as wild type were used as templates, amplification of DNA fragments each containing the above three mutations by PCR. The PCR products were analyzed by DNA sequencing, Dot blot, and Southern blot. Results: There were no significant differences in DNA concentration between buccal cells and blood (P>0.05). However, the purity of buccal cell DNA extracted by manual sampling was lower than the buccal cell DNA and blood DNA extracted by reagent kits (P<0.05). The DNA yielding of buccal cell DNA was similar to that of blood DNA. The results of DNA sequencing, Dot blot and Southern blot were also highly consistent. Both groups were positive in the control group and negative in the mutant group. Conclusion: The results of DNA sequencing, Dot blot and Southern blot demonstrate that buccal cell DNA can be used for the screening and identification of three primary mitochondrial mutations of LHON. This study has established a convenient and non-invasive detection assay suitable for clinical determination in three primary mitochondrial mutations associated with LHON.

418 The effect of YiShenJianPi decoction on dyslipidemia in women with polycystic ovary syndrome undergoing frozen embryo transfer

Objective: To study the effectiveness of YiShenJianPi decoction on blood lipid in women with polycystic ovary syndrome, and to investigate the impact on the outcome of FET cycles. Methods: A prospective study was performed in 227 women with PCOS and dyslipidemia who underwent the failure of first FET cycle. Patients in group A (n=55) received lifestyle modification programs. Patients in group B (n=85) received lifestyle modification and metformin. Patients in group C (n=87) received lifestyle modification and YiShenJianPi decoction. BMI, WHR, T, TC, TG, LDL-C, HDL-C were compared before and after treatment in all patients. Clinical outcome of re-FET was valued, including embryo implantation rate,clinical pregnancy rate, early abortion rate and live birth delivery rate. Results: T, TC, and TG of 3 groups, LDL-C and HDL-C of group B, C, WHR of group C, were all significantly improved after treatment (P<0.05), and meanwhile compared to group A, T, TC, LDL-C declined obviously and HDL-C increased remarkably in group B, C (P<0.05). The embryo implantation rate of group C (43.98%) was significantly higher than that of group A (31.68%) and B (35.06%) (P<0.05). No significant differences were detected in the clinical pregnancy rate early abortion rate and live birth delivery rate among 3 groups (P>0.05). Conclusion: The three strategies mentioned above could obviously alter dyslipidemia and help patients produce ideal pregnancy outcome, with YiShenJianPi decoction in particular, as it could remarkably increase the embryo implantation rate of FET.

423 Clinical efficacy of apatinib combined with transcatheter arterial chemoembolization in the treatment of hepatocellular carcinoma and its effect on tumor angiogenesis

Objective: To explore the value of apatinib combined with transcatheter arterial chemoembolization (TACE) in the treatment of unresectable advanced hepatocellular carcinoma (HCC). Methods: A retrospective study of 59 patients with HCC who were admitted to Lishui Central Hospital from March 2017 to January 2019. All patients underwent TACE and were divided into apatinib combined with TACE group (combined group, 28 cases) and TACE group (31 cases) according to the presence or absence of apatinib. After 3 months of treatment, the tumor responses of the two groups were observed and compared, and the changes in serum vascular endothelial growth factor (VEGF) and caspase-8 before and after treatment were compared, so as to clarify the anti-liver cancer efficacy of apatinib combined with TACE. Results: The total efficacy rate was 71.43% in the combined group, significantly higher than 45.16% in the TACE group (P<0.05). The VEGF levels before treatment in the combination group and the TACE group were (122.99±13.10) pg/mL and (120.36±13.86) pg/mL, respectively. There was no significant difference between the two groups (P>0.05). After treatment, they were (91.49±8.65) pg/mL and (116.89±10.95) pg/mL which were lower than before treatment (P<0.01), and the levels in the combination group was significantly lower than that in the TACE group (P<0.01). The levels of caspase-8 before treatment in the combination group and the TACE group were (83.68±9.21) pg/mL and (82.87±12.01) pg/mL, respectively. There was no significant difference between the two groups (P>0.05). After treatment, they were (105.55±7.25) pg/mL and (86.57±7.52) pg/mL, which were higher than before treatment (P<0.01), and the levels in the combination group was significantly higher than that in the TACE group (P<0.01). Conclusion: Apatinib combined with TACE can effectively inhibit tumor angiogenesisand to improve disease control in the treatment of unresectable advanced hepatocellular carcinoma.

427 Factors influencing the sedative effect of propofol during target controlled infusion
Objective: To analyze the relevant factors affecting the sedative efficacy of propofol in target-controlled infusion. Methods: A total of 100 patients scheduled for elective operation undergoing general anesthesia from March to June 2018 in the Second Affiliated Hospital of Wenzhou Medical University were included in this study. Age, gender, height, weight and body mass index (BMI), systolic blood pressure, diastolic blood pressure and blood biochemical indexes of the patients were recorded. Patients were received propofol in target-controlled infusion, and the initial plasma target concentration was set at 1.2 μg/mL. After the target concentration was steadily maintained for 30 s, the gradient was increased by 0.3 μg/mL every 30 s until the patients lost consciousness. The target plasma concentration of propofol at this time was recorded. Spearman rank correlation and rank sum test were used to analyze the correlation between each variable and the plasma target concentration, and the screened variables were included in the stepwise multiple linear regression model, finally determining the independent influencing factors. LOESS model was used to analyze the association between independent influencing factors and plasma target concentration, then the trend chart was fitted. Results: Correlation analysis showed that age, BMI, systolic blood pressure, blood glucose, urea nitrogen, urea nitrogen/creatinine and history of hypertension were significantly associated with the target plasma concentration (P<0.05). Stepwise multiple linear regression analysis showed that age, BMI, and systolic blood pressure were independent factors in the efficacy of propofol sedation (P<0.05). According to the trend chart fitted by LOESS model, the plasma target concentration decreased as the increase of age and BMI. The plasma target concentration was first increased and then decreased as the increase of systolic blood pressure. Conclusion: Age, BMI and systolic blood pressure are significant factors affecting the efficacy of propofol sedation.

432 The phenotypic and gene mutation analysis of a family with hereditary plasminogen deficiency

Objective: To detect the phenotype and gene mutation of a family with hereditary plasminogen (PLG) deficiency and to investigate the molecular mechanism of its pathogenesis. Methods: Prothrombin time (PT), activated partial thromboplastin time (APTT), thrombin time (TT), fibrinogen (FIB), D-dimer (DD), fibrinogen degradation product (FDP), plasminogen activity (PLG:A), plasminogen antigen (PLG:Ag), antithrombin activity (AT:A), protein C activity (PC:A) and protein S activity (PS:A) were tested in the diagnosis of the proband and his family members (3 generations and 4 persons). Direct DNA sequencing was used to analyze all exons and exon-intron boundaries of the PLG gene, as well as the 5’ and 3’ non-translation regions and the corresponding mutation site regions of family members, confirmed by reverse sequencing. ClustalX-2.1-win software was used to analyze the conserved amino acid sequence of homologous species. PolyPhen-2, PROVEAN, SIFT and Mutation Taster were used to forecast the possible impact of mutant amino acids on protein function. Finally, the protein model and amino acid interaction analysis was performed on the mutation sites using Swiss-Pdb Viewer software. Results: The PLG:A of the proband and his grandfather and father were reduced to the half, while the PLG:Ag was normal, the results being 45%, 95%; 64%, 94% and 64%, 104% respectively. Gene sequencing revealed that the PLG gene of the proband had c.1858G>A heterozygous missense in exon 15, resulting in p.Ala601Thr. Its grandfather and father had the same genetic mutation. Ala601 was highly conserved among its 11 homologous species. The four online bioinformatics software have consistent predictions, all of which were harmful mutations that affect the PLG function, leading to the corresponding diseases. Conclusion: The proband’s PLG gene exon 15 has heterozygous missense mutation of c.1858G>A(p.Ala601Thr) . And the three generations of the grandparents are Ala601Thr heterozygotes, with the mutation in accordance with Mendelian inheritance law and with the family plasminogen activity reduced.

437 The value of contrast-enhanced magnetic resonance T1-weighted imaging in the diagnosis and classification of complex anal fistula

Objective: To explore the diagnostic value of contrast-enhanced magnetic resonance T1-weighted imaging in complex anal fistula. Methods: A total of 43 patients with complex anal fistula which confirmed by surgery from the Third Affiliated Hospital of Wenzhou Medical University and the Pingyang People’s Hospital from February 2015 to January 2018 were included in the study. They were underwent T1-weighted imaging (T1WI), T2-weighted imaging with fat saturation (T2WI FS) and T1-weighted imaging with fat saturation (T1WI FS) contrast-enhanced sequences. The display of the three sequences on internal fistula, main canal and branch tube of anal fistula were compared with surgical findings. Results: Among the 43 patients, 67 internal openings, 63 fistulas and 47 fistula branches were found in surgeries. There were 10 internal openings (14.93%), 34 fistulas (53.97%) and 10 fistula branches (21.28%) found by T1WI. There were 52 internal openings (77.61%), 55 fistulas (87.30%) and 40 fistula branches (85.11%) found by T2WI FS. There were 61 internal openings (91.04%), 60 fistulas (95.24%) and 42 fistula branches (89.36%) found by T1WI FS contrast-enhanced scan. Compared with T2WI FS, TIWI FS contrast-enhanced scan had a higher accuracy in showing the internal openings (P<0.05), and there was no statistical difference between the accuracy of showing the fistula and the fistula branch (P>0.05). The accuracy of T1WI FS contrast-enhanced scan was higher than that of T1WI (P<0.05), and the difference was statistically significant. T1WI FS contrast-enhanced scan was highly consistent with surgical results in Parks classification (Kappa=0.926), which was obviously higher than T2WI FS and T1WI sequences (Kappa=0.786, Kappa=0.467). Conclusion: T1WI FS contrast-enhanced sequence can be an important supplement to conventional T2WI FS and T1WI sequences of anal fistula MR examination, especially in the preoperative localization of the internal openings and the classification assessment of complex anal fistula.

442 The effectiveness of 755 nm picosecond laser flat lens combined with Focus lens array in the treatment of melasma

Objective: To observe the clinical efficacy and adverse effects of 755 nm picoseconds laser flat lens combined with focus lens array in the treatment of melasma. Methods: Totally 36 outpatients with melasma were treated with 755 nm picosecond laser flat lens combined with focus lens array for 6 treatment sessions at 4-week intervals. The patients were first treated with 755 nm picosecond laser flat lens. The treatment parameters: 0.4 J/cm2 of fluence, 8 mm of spot size and 5 Hz of frequency. Then they were treated with 755 nm picosecond laser focus lens array with the same parameters. Patients were all followed up for 12 weeks. Melasma area and severity index (MASI) scores were calculated after treatment, and related adverse reactions and patient satisfaction survey were recorded at the same time with statistical analysis followed. Results: The mean MASI scores before treatment, after 6 treatments and after 12-week follow up were (11.17±5.29), (4.63±3.05) and (5.11±3.23) respectively. The decreasing ration of the MASI score was (64±7)% after 6 treatments and (61±6)% after 12-week follow up. The mean MASI scores after 6 treatments and 12-week follow up were both lower than before treatment, and the difference was statistically significant (P<0.05). The mean MASI score after 12-week follow up was slightly higher than that after 6 treatments. The difference was not statistically significant (P>0.05). The decreasing ration of the MASI score in Fitzpatrick type III patients was (68±8)%, higher than that in Fitzpatrick type IV patients (56±6)%, and the difference was statistically significant (P<0.05). Patient satisfaction rate was 94.4% after 6 treatments and 88.9% after 12-week follow up respectively. Skintexture was finer and firmer, fine lines and coarse pores were obviously improved in most patients. No adverse reactions occurred in all patients. Conclusion: The combination ofpicosecond 755 nm laser flat lens with focus lens array is effective and safe with lower recurrence rate and higher patient satisfaction in the treatment of melasma and thus worthy of promotion.

447 Meta-analysis of metformin in combination with GLP-1 agonist on metabolic syndrome in women with polycystic ovaries

Objective: Meta-analysis was performed to assess the efficacy of metformin in combination with GLP-1 agonist on the Metabolic syndrome of women with polycystic ovary syndrome (PCOS). Methods: The randomized controlled trials (RCT) were retrieved from Pubmed, Cochrane library, EMbase, CNKI, WanFang and VIP database and references listed in all studies. RCTs meeting inclusive studies were included the data extracted, the quality was evaluated and cross-checked by two reviews independently according to Cochrane Handbook for Systematic Reviews of Interventions and then Meta-analysis was conducted using Stata 12.0 software. Results: A total of 4 studies were included, of which 61 were in test group (metformin combined with GLP-1 agonist) and 60 were in control group (GLP-1 agonist). The results of Meta-analysis showed that test group had a significant difference in waist circumference change compared with control group (SMD=0.39, 95%CI: 0.03-0.75, P=0.035), the rest in BMI (SMD=-0.13, 95%CI: -0.48-0.23, P=0.492), body mass (SMD=-0.19, 95%CI: -0.54-0.17, P=0.311), insulin resistance index (SMD=-0.68, 95%CI: -2.23-0.86, P=0.386), fasting insulin level (SMD=0.19, 95%CI: -0.28-0.66, P=0.426), sex hormone binding globulin (SMD=0.72, 95%CI: -0.55-2.00, P=0.266), serum total testosterone (SMD=-0.70, 95%CI: -1.98-0.57, P=0.279), there were no significant difference. Conclusion: Metformin combined with GLP-1 agonist is superior to GLP-1 agonist alone in improving the waist circumference of patients with PCOS. In contrast, metformin does not increase the therapeutic effect of GLP-1 agonists in BMI, body mass, insulin resistance index, fasting insulin level, sex hormone binding globulin, serum total testosterone.

452 Application of pyloric chisel in transumbilical single-site laparoscopic pyloromyotomy

Objective: To investigate the value of pyloric chisel in transumbilical single-site laparoscopic pyloromyotomy. Methods: Pyloric incision knife was applied in 28 cases of congenital hypertrophic pyloric stenosis from Feb. 2011 to Mar. 2016 (knife group) and 30 children were treated by pyloric chisel from Jun. 2016 to Jan. 2018 (chisel group). The operative time and complications were compared between the two groups. Results: The operative time was shorter in the pyloric chisel group than the knife group [(21.2±3.9) min vs. (36.8±7.3) min, t=-10.048, P<0.001]. The complications were fewer in the pyloric chisel group than the knife group (χ2=4.041, P=0.044). There were 2 cases of mucosal perforations that had to perform abdominal surgery in the knife group. Five cases of serous tearing were found in the knife group but only one in the pyloric chisel group. All cases were followed up for 1 year and all children were normally developed. Conclusion: Compared wtih pyloric incision knife, pyloric chisel is more efficient and safer in the surgery of transumbilical single-site laparoscopic pyloromyotomy.

Objective: To analyze the path of psychological nursing intervention in the treatment, rehabilitation and growth of patients with mental disorders, and to reflect on the issues of value neutrality and empathy in the intervention process, so as to provide experience for psychological nursing services to intervene in the rehabilitation of mental disorders. Methods: By reflecting the intervention and supervision process of 1 patient with mental disorders, this paper summarized the advantages of introspection, the emotional involvement of the nursing workers and the neutrality of values. Results: Communication and cooperation with psychiatrists were more conducive to the treatment and rehabilitation of patients. Focusing on the present and the advantage perspective helped the patient to discover more possibilities and hopes. The application of painting skills and systematic family view could help us further understand the patient. When involved in the dilemma of value neutrality and role identity deviation, we have to sort it out and timely supervision is needed. Conclusion: In the process of intervention, we should maintain value neutrality to avoid emotional involvement, which is more conducive to the growth of patients and the goal achievement of good family relationship. Team work and supervision are beneficial to the development and service of social workers.

461 Discussion on the ethical review of human gene editing research: reflections on the gene edited baby born in China

This paper analyzes the legal and ethical issues in the event of gene editing baby and reflects from the following four aspects: rigid regulation, lagging ethical norms, independence and supervision. In order to improve the ethical review of human gene editing research, the paper proposes from perspectives of strengthening the ethical legislation, access and approval mechanism of gene editing research, ethical regulations of gene editing technology, independent ethical review system and global supervision.
