The analysis of macular microvasculature alteration of myopic eyes with optical coherence tomography angiography

Objective: To explore the alterations of myopic macular retinal capillaries and its correlation with retinal thickness using optical coherence tomography angiography (OCTA). Methods: Ninety eyes of 45 people were included in this study, divided into low myopia and emmetropia group 30 eyes of 15 people (control group), moderate myopia group 30 eyes of 15 people, high myopia group 30 eyes of 15 people. Acquisition of macular micro-vascular density and retinal thickness, then obtained the superficial and deep vascular graph by automatic stratification. The densities of 1 mm circle (fovea) and 3 mm circle ring (parafovea) in the center of macular fovea were analyzed using the built-in software. Results: There was a significant positive correlation between foveal superficial/deep density and the axial length (r=0.51, 0.52, P<0.05). There was a significant negative correlation between parafoveal superficial density and axial length (r=-0.34, P=0.01), but no significant correlation between parafoveal deep density and axial length (r=-0.03, P=0.79). There was a significant positive correlation between foveal superficial/deep density and the inner five-layer thickness of fovea (r=0.85, 0.82, P<0.05). There was no significant correlation between parafoveal superficial/deep density and the inner five-layer thickness of parafovea (r=0.16, 0.07, P>0.05). Conclusion: The density of retinal blood vessels in macular fovea will increase with increasing axial length and retinal thickness, while the density of retinal blood vessels will decrease with increasing of axial length but not directly related with the change of retina thickness in retinal parafovea. Clinical application of blood flow imaging technology should be taken account into myopia on the macular micro vascular impact.

Effect of microRNA-143/145 on endometriosis in SD-rat models

Objective: To investigate the therapeutic effect of miR-143/145 on endometriosis cysts in SD rat models. Methods: The model of endometriosis in SD rats was established by autologous transplantation, and was identified at the histopathological level. Eighteen SD rats were randomly divided into blank control group (n=6), negative control group (n=6) and miR-143/145 mimic group (n=6), were given the corresponding treatment, respectively. Finally laparotomy, and the size of ectopic cysts were measured by vernier caliper, and the volume of ectopic cysts were compared among the three groups. The histopathological changes of the three groups were also compared. Results: The successful rate of modeling of endometriosis in SD rats was 84%. After treatment, the difference in the volume of ectopic cysts in each group: the volume of ectopic cysts in control group was (16.38±4.19) mm3, negative control group was (21.14±0.51) mm3, miR-143/145 mimic group was (5.91±1.91) mm3. There was no significant difference between the blank control group and the negative control group (P>0.05), however, the volume of ectopic cysts in the miR-143/145 mimic group was significantly reduced compared with negative control group (P<0.01). The histopathological changes: the histopathological morphology of ectopic cyst in blank control group and negative control group was similar, but in miR-143/145 mimic group the number of glands was decreased, the glandular atrophy, and the arrangement of stromal cell were messy compared with the mimic NC group. Conclusion: MiR-143/145 can atrophy ectopic cysts of endometriosis and achieve therapeutic effect.

Instilling water-soluble drugs through the exposed trachea in mice and designing the control group

Objective: To make water-soluble drugs instilled through the exposed trachea a stable and roughly equal distribution in the lungs of mice and to design the control group scientifically. Methods: Intervention measure, mouse position, instillment speed, minimum volume that requires and maximum volume that tolerates were all taken into consideration to make the distribution of the drug (Methlene Blue instead) in lung lobes in mice (weighting from 18 g to 30 g) stable and roughly equal. Then taking lipopolysaccharide (LPS) as example, the LPS group and normal saline (NS) group were treated with LPS (2 mg/mL) and normal saline (NS) respectively in the way above mentioned in mice weighting from 20 g to 24 g. The normal group was with no pretreatment. After modeling for 3 h, 8 h, 14 h, the level of monocyte chemoattractant protein-1 (MCP-1) in bronchoalveolar lavage fluid (BALF) detected by enzyme-linked immunoabsorbent assay (ELISA), the polymorphonuclear neutrophil percentage (PMN%) in BALF tested with Wright-Giemsa stain, histopathological changes assayed by HE staining, the expression of intercellular adhesion molecule 1 (ICAM-1) assayed by immunohistochemical staining and the lung wet weight/dry weight ratio (W/D ratio) in lung tissue would be tested. Results: Using the process developed above, the distribution of Methlene Blue in lung lobes was stable and roughly equal. Compared with the normal group, the level of MCP-1 in BALF in the LPS group increased at 3 h, 8 h, and as time went by, the PMN% in BALF increased and histopathological changes appeared obvious, and both the expression level of ICAM-1 and the W/D ratio increased (P<0.05) in LPS group. Compared with the normal group, only the level of MCP-1 in BALF and the W/D ratio increased significantly (P<0.05) in the NS group at 3 h, and all measurements had no difference at 8 h and 14 h. Conclusion: The process developed here to instill water-soluble drugs through exposed trachea is successful. The control group should be treated with NS for the same time if researching the lung changes within 8 h, or just the normal mice instead if at least 8 hours.

The structure and properties of singlet and triplet states of benzophenone in different environments

Objective: To study the structural and spectroscopic properties of singlet and triplet states of benzophenone in vacuum phase and solvent environments (cyclohexane, chloroform, ether, ethanol). Methods: The geometry configuration, electronic structure, ultraviolet and infrared spectroscopy of benzophenone were calculated by density functional theory (DFT) and time-dependent density functional (TD-DFT) methods with CAM-B3LYP function and 6-311++G(d, p) basis set. The solvent effects were dealt with the polarizable continuum model. Results: The singlet and triplet states of benzophenone were non-planar symmetry. The UV-vis spectra of singlet state showed two obvious absorption peaks. The maximum absorption wavelength underwent a red-shift with the increasing solvent polarity. There were three absorption peaks in triplet state and their intensities were weakened as compared with the singlet state. The C=O stretching frequency of infrared spectroscopy was shifted to the lower wave numbers in singlet state with increasing polarity while disappeared in singlet state. A new C-H bending mode and a combination of modes implying C-C and C-O stretching vibration were appeared in triplet state. Conclusion: The C=O stretching frequency and the maximum absorption wavelength are sensitive to the solvent polarity and show a red-shift with increasing solvent polarity. However, there is no striking difference between vacuum phase and solvent environments in IR absorption peaks of triplet state as well as geometry configuration.

The application of three-dimensional printing technique for the treatment of femoral intercondylar fractures

Objective: To investigate the clinical outcome of three-dimensional printing technique for the treatment of femoral intercondylar fractures. Methods: Forty eight patients with femoral intercondylar fractures between January 2015 and September 2015 were randomly divided into treatment group and control group. Mimics software was used to reconstruct fracture model and reduction in treatment group, and preoperative plan was designed based on 3D printing model. Control group’s patients underwent traditional treatment. The surgery duration, blood loss, fluoroscopy time and HSS score were collected. Results: The surgery duration in treatment group (61.25±14.28) min was shorter than that in the control group (79.14±18.43) min, blood loss in treatment group (81.74±23.50) mL was less than that in the control group (106.80±22.75) mL while fluoroscopy time also showed better performance (2.10±0.50 vs. 4.10±0.70), and there was significant difference between two groups (P<0.05). After one-year follow up, the knee function in treatment group was better than that in the control group (P<0.05). Conclusion: Three-dimensional printing technique for the treatment of femoral intercondylar fractures can save surgery duration, reduce blood loss to certain extent, and obtaine better clinical outcome compared to traditional treatment.

Correlation between FAT/CD36 expression in placenta and macrosomia

Objective: To explore the correlation between expression of FAT/CD36 in placenta and macrosomia. Methods: The cases were collected from June 2014 to June 2016 in the Second Affiliated Hospital & Yuying Children’s Hospital of Wenzhou Medical University, including 44 macrosomia newborns and 45 normal birth weight newborns. A self-designed questionnaire was used to collect related information of pregnant women and their neonates, real-time fluorescent quantitative PCR (qPCR) was used to detect FAT/CD36 mRNA expression in placenta. Western blot was used to detect FAT/CD36 protein expression. The influencing factors of macrosomia were analyzed by multivariate logistic regression model. Results: qPCR and Western blot showed FAT/CD36 expression of macrosomia was significantly higher than that of healthy controls (P<0.05). The result of multivariate logistic regression analysis showed that high expression of FAT/CD36 mRNA expression was a risk factor, and the occurrence of macrosomia was influenced by gestational age, gestational weight gain and newborns sex. Conclusion: The up-regulation of FAT/CD36 in placenta may be correlated with macrosomia.

Expression of histone deacetylase 6 in diffuse large B-cell lymphoma and its clinical significance

Objective: To explore the histone deacetylase 6 (HDAC6) expression in diffuse large B-cell lymphoma (DLBCL) and to analyze its relationship with the clinicopathological characteristics and survival. Methods: The clinical data from 93 DLBCL patients and 15 patients with reactive hyperplasia of lymph node who were treated in the Second Affiliated Hospital of Wenzhou Medical University from December 2007 to September 2015 was retrospectively analyzed. Cases were subjected to immunohistochemical staining for HDAC6 protein expression in the DLBCL tissue. SPSS was used to analyze the relationship between HDAC6 expression and various clinical factors, and the relationship between HDAC6 and survival. Results: High expression rates of HDAC6 protein in 93 DLBCL tissues were 69.9%, which was significantly higher than 20% in reactive hyperplasia of lymph node (P<0.05). In DLBLC, the ratio of IPI (0-2) in high expression group was significantly higher than that in low expression group but the ratio of increased LDH and β2-microglobulin in high expression group was significantly lower than that in low expression group (P<0.05). On Kaplan-Meier analysis, the 5-year overall survival and the 5-year progression free survival in high expression group was significantly higher than that in the low expression one. Conclusion: HDAC6 is highly expressed in DLBCL patients, patients with highly expressed HDAC6 represents low IPI score and light tumor burden. The highly expressed HDAC6 as a favorable prognostic factor has more prognostic value for DLBCL.

The influential factors of real time tissue elastography quantitative analysis on the diagnosis of hepatic fibrosis

Objective: To identify the factors which could influence the liver fibrosis index (LFI) based on real-time tissue elastography for liver fibrosis staging. Methods: Two hundred and sixty-two consecutive patients with chronic hepatitis B were prospectively recruited between January 2015 and December 2016 in the First Affiliated Hospital of Wenzhou Medical University. Real-time tissue elastography was performed and the LFI was calculated for the patients. The relationships between the LFI and clinical or morphological parameters were analyzed using pearson correlation analysis. Thereafter, a stepwise multiple linear regression analysis was used to identify the variables to be independent associated with LFI. Results: Pearson correlation analysis showed that body mass index (BMI), platelet count, portal vein diameter, hepatic artery diameter, hepatic artery velocity, spleen thickness, spleen diameter, splenic index, splenoportal index, splenic artery diameter, splenic vein diameter, and splenic vein velocity were significantly associated with LFI (P<0.05). BMI, platelet count, splenic vein diameter, and splenic vein velocity were identified as the independent variables associated with LFI by stepwise multiple linear regression analysis (P<0.01). Conclusion: BMI, platelet count, splenic vein diameter, and splenic vein velocity are identified as the independent variables associated with LFI. Clinical data and laboratory data, especially the ultrasonic parameters may improve the performance of LFI in staging liver fibrosis in patients with chronic hepatitis B.

The effects of Evodiamine on the proliferation and apoptosis of pancreatic cancer SW1990 cells in vivo and in vitro

Objective: To investigate the effects of Evodiamine on the proliferation and apoptosis of pancreatic cancer SW1990 cells. Methods: Pancreatic cancer SW1990 cells were mixed with different concentrations of Evodiamine (0, 5, 10, 20, 40, 80 μmol/L) after 12, 24, 48 hours respectively. The SW1990 cells’ proliferation was detected by cell counting Kit-8 (CCK-8). The SW1990 cells’ cycle was detected with flow cytometry test. Nude mouse models were established with orthotopic transplantation tumor, which were injected with different concentrations of Evodiamine for 2 weeks. The orthotopic transplantation tumor’s weight was measured after sacrificed. The expression of Active Caspase-3 and Active Caspase-8 in tumor tissues was detected by Western blot and immunohistochemistry. Results: Evodiamine inhibited the proliferation effect of SW1990 cells by concentration and time dependence. The quantity of pancreatic cancer SW1990 cells in G2/M phase significantly increased. The expressions of Active Caspase-3 and Active Caspase-8 protein significantly increased. With increasing of Evodiamine concentrations, the inhibiting effect on the nude mouse’s growth of pancreas transplantation tumor was more remarkable and the expressions of Active Caspase-3 and Active Caspase-8 increased much more obviously in the tumor tissues. Conclusion: Evodiamine can significantly inhibit human pancreatic cancer SW1990 cells’ proliferation with concentration and time dependence. Its mechanism may retard the growth of pancreatic cancer cells in G2/M phase. In the meantime, Evodiamine can activate Caspase-8 and Caspase-3, then promote the apoptosis of pancreatic cancer cells.

Paeoniflorin up-regulates expression of Sirt1 in spinal dorsal horn and dorsal root ganglia in chronic constriction injury rat

Objective: To observe the effect of paeoniflorin on pain behavior and the expression of Sirt1 in spinal dorsal horn and dorsal root ganglion in rats with chronic constriction injury. Methods: Forty-eight male SD rats were randomly divided into sham operation group, negative contract group (SC group), CCI group and paeoniflorin group. Heat shrinkable foot reflex latency (TWL) and mechanical paw withdrawal reflex threshold (MWT) were detected for rats in each group at 1 day before operation and postoperative 3, 7 and 14 days. Rats were sacrificed at 3, 7 and 14 days after surgery then L4-5 segment of spinal cord and dorsal root section organization was taken, and real-time PCR was used for expression of Sirt1, Western blot was used in the same time to detect the change of Sirt1 protein level. Results: At 3, 7 and 14 days after surgery, TWL and MWT of CCI group decreased significantly compared with sham group (P<0.05), and achieved the minimum value at the third day. Real-time PCR showed that the expression of Sirt1 of CCI group of spinal dorsal horn and dorsal root ganglia were significantly decreased at the 7th day and 14th day after surgery compared with sham group (P<0.05); the expression of Sirt1 of paeoniflorin group was upregulated compared with SC group (P<0.05); the results of Western blot also showed that the level of Sirt1 protein in the dorsal horn and dorsal root ganglion were gradually increased with time, and there was difference between the Sirt1 group and the SC group at 14th day. Conclusion: Paeoniflorin can alleviate by pain neuropathic disease caused, and the up regulation of the expression of Sirt1 protein in dorsal horn and dorsal root ganglion of the spinal cord is a possible mechanism.

Comparison of different ovary stimulation protocols in vitro maturation of polycystic ovary syndrome patients

Objective: To compare the effectiveness of two clinical stimulated protocols in vitro maturation (IVM) cycles of polycystic ovary syndrome (PCOS) patients. Methods: Laboratory and clinical results were analyzed retrospectively for PCOS patients in IVM from January 2005 to December 2015 in the First Affiliated Hospital of Wenzhou Medical University. The overall 164 cycles were divided into two groups according to the stimulation protocol used, group A, IVM followed standard luteal long protocol (116 cycles) and group B, mild stimulated IVM (48 cycles). Basal characteristic, laboratory and clinical outcomes were used to assess the influence of different stimulated IVM in PCOS patients. Results: In the group A, the total dose of gonadotropin (1 008.83±579.68) IU was statistically higher than that in group B (540.52±329.16) IU (P<0.05). Numbers of oocytes retrieved of group A was significantly lower than that in group B (P<0.05). The group A also showed lower miscarriage rates compared to the other group (8.11% vs. 20.00%, P<0.05). The differences of embryo implantation rate (23.18% vs. 24.37%), clinical pregnant rate (45.95% vs. 53.33%) and live birth rate (32.43% vs. 28.89%) between the two groups were not statistically significant (P>0.05). Conclusion: PCOS patients using IVF convert in IVM cycles or gonadotropin priming IVM received the similar clinical pregnant rate compared with traditional IVF. However, the lower miscarriage rate of IVM converted from conventional stimulated IVF may lead to more clinical application.

The correlation between value of serum vitamin D and outcomes following in vitro fertilization

Objective: To investigate the effect of vitamin D deficiency on the in vitro fertilization (IVF) outcomes and validate the predictive value of serum vitamin D levels on pregnancy outcomes following IVF. Methods: This prospective study randomly chose 321 cases undergoing IVF with long protocols from January to June 2016, in which 307 patients finished ovarian sitimulation cycles. The patients were allocated to one of the two groups according to their serum 25-OH vitamin D (basal VD) concentrations on menstrual cycle 2 to 5 days before IVF. VD deficiency Group (n=173) with less than 50 nmol/L, the control (n=134) with more than or equal to 50 nmol/L. In 196 cycles undergoing fresh embryos transfer, they were divided into pregnant group (n=107) and non-pregnant group (n=89). The levels of 25-OH vitamin D and the results of ovarian stimulation and clinical outcomes were compared in groups. Results: Basal serum 25-OH VD constration was significantly lower in VD deficiency group than that in control (38.2±7.8 nmol/L vs 63.8±11.1 nmol/L, respectively, P<0.01). Expect for age, there were no statistical differences of body mass index (BMI), basal follicle stimulation hormone levels, the peak values of estradiol number of oocytes, the rates of fertilization, good-quality embryos, implantation and clinical pregnancy in two groups (P>0.05). Concentration of plasma 25-OHVD in pregnant women was similar to that of non-pregnant women (P>0.05). Compared with non-pregnant patients, the rates of ovum maturation, fertilization and good-quality embryos were significantly higher in pregnant patients (P<0.01). Conclusion: The results obtained suggest that basal serum levels of vitamin D are not correlated with clinical outcomes of infertile women after IVF, and currently, the concentration of vitamin D could not be a valuable predictor of IVF success ratio.

The clinical application of rapid rehabilitation combining with laparoscopic surgery in colorectal cancer

Objective: To explore the feasibility and advantage of rapid rehabilitation combining with laparoscopic surgery in colorectal cancer. Methods: Sixty-nine cases of colorectal cancer from 2013 to 2015 in Department of Anorectal Surgery of the Second Affiliated Hospital of Wenzhou Medical University were selected as observation objects. They were divided into four groups, group A: laparoscopy combined with rapid rehabilitation group, group B: open surgery combined with rapid rehabilitation group, group C: laparoscopy combined with conventional rehabilitation group, group D: open surgery combined with traditional rehabilitation group. Pairwise comparisons were made among the groups, and four groups of patients were compared with each other in the postoperative recovery time of intestinal peristalsis, the time of the first postoperative exhaust, postoperative pain score, postoperative activity time, total hospitalization time and C-reactive protein concentration in perioperative periodetc. Results: The postoperative recovery time, postoperative activity time and the total number of days of hospitalization of laparoscopy combined with rapid rehabilitation group patients were significantly shorter than that of the other three groups (P<0.05); postoperative pain score of group A was better than that of the other three groups (P<0.05); CRP of four groups declined in different degrees in the five days, and the blood CRP concentration of combination of laparoscopic fast recovery group was significantly higher than that of the other three groups (P<0.05); There was no significant difference among the four groups in postoperative anal exhaust time (P>0.05). Conclusion: The mode of laparoscopy combined with rapid rehabilitation can shorten patients’ peristalsis recovery time, early activity time of the patient and the total number of days of hospitalization, can relieve pain after surgery. What’s more, certain advantages can be found in patient recovery after surgery, in reducing surgical trauma to the patient’s stress response. It is worthy of promotion for its good clinical benefits.

Influencing factors of postoperative cognitive dysfunction in elderly patients undergoing sevoflurane anesthesia

Objective: To investigate the effect of sevoflurane anesthesia on postoperative cognitive dysfunction in elderly patients and the relationship with Tau protein in cerebrospinal fluid. Methods: Sixty-five ASA I-III patients aged over 65 years undergoing eletive plasma resection of prostate were enrolled. All patients were received spinal anesthesia at L3-4 interspace and inserted with spinocath catheter. Cerebrospinal fluid and peripheral venous blood were sampled respectively before induction and at 1 h after operation. The expression levels of Tau5 and p262Tau protein were determined by the enzyme-linked immunosorbent assay (ELISA). Anesthesia was maintained with sevoflurane. Patients were divided into the POCD group and the non-POCD group by the MMSE scores and Z scores which were performed 1 d before and 7 d after operation. The Tau5 and p262Tau protein concentrations of the two groups were taken into comparison. Results: Twenty-two of the elders developed POCD (39%). There were no significant differences in general conditions and operative indicators between the two groups. The expression levels of Tau5 and p262Tau protein in cerebrospinal fluid had no significant differences between two groups before induction (P>0.05). Compared with non-POCD group, the p262Tau protein concentration was significantly increased in POCD group after operation (P<0.05). The expression of p262Tau protein was up-regulated in POCD group in 1 h after operation compared to preoperation (P<0.05). Conclusion: In incidence of POCD is associated with the increase of Tau protein phosphorylation ser262Tau in cerebrospinal fluid in elderly patients during sevoflurane anestheisa.

Expression of survivin and PTEN in benign and malignant gastric ulcer

Objective: To study the expression of survivin and the 10th chromosome homologous gene lacking protein phosphatase-tension (PTEN) in benign and malignant gastric ulcer. Methods: One hundred gastric ulcer tissues and one hundred gastric cancer tissues were collected from January 2015 to March 2016 in the Third Affiliated Hospital of Wenzhou Medical University. Expressive difference of PTEN and survivin between gastric ulcer tissues and gastric cancer tissues were measured by immunohistiochemistry. Results: The positive expression rate of survivin in gastric cancer tissues was higher than that in the gastric ulcer tissues, but the positive expression rate of PTEN in gastric cancer tissues was lower than that in the gastric ulcer tissues, the difference was both statistically significant (P<0.01). Conclusion: The expression of survivin in gastric cancer is positive, and the expression of PTEN is negative.
