Construction of hepatitis C virus CTL-Th epitopes chimeric DNA vaccine

Objective: To construct hepatitis C virus (HCV) CTL-Th epitopes chimeric DNA vaccine and explore its in vivo immune response. Methods: Based on previously identified four HCV-specific HLA-A*0201-restricted CTL (Cytotoxic T Lymphocyte) epitopes (NS4b_78, NS5a_367, C_181 and NS2_172), two HLA-A*1101-restricted CTL epitopes (NS3_609 and NS5b_251), one HLA-A*2402-restricted CTL epitopes (NS5b_382) and two Th epitopes (P73-15 and NS5A276-288), the gene encoding chimeric CTL epitopes and Th epitopes was synthesized and cloned into eukaryotic expressing vector pcDNATM3.1/myc-His(-) A. The recombinant plasmid was used to immunize HLA-A*0201, HLA-A*1101 and HLA-A*2402 transgenic mice, ELISPOT and CTL cytotoxicity assay were used to measure the frequencies of CTL epitope-specific T cells in splenocytes of mice and the cytotoxic activity of CTL against target cells. Results: The gene containing Kozak sequence and encoding Igκ chain signal sequence, seven CTL epitopes, two Th epitopes was successfully cloned into eukaryotic expressing vector. Recombinant plasmid immunization elicited epitope-specific IFN-γ-secreting T cells which could lyse CTL epitope-pulsed splenocytes. Conclusion: A Hepatitis C virus CTL-Th epitopes chimeric DNA vaccine which can induce epitope-specific CTL response is successfully constructed.

The role of changes of key enzymes of brain 5-hydroxytryptamine and 5-hydroxytryptamine bypass in rat model of postoperative fatigue syndrome

Objective: To explore the role of changes of key enzymes of brain 5-HT and 5-HT bypass in rat model of postoperative fatigue syndrome. Methods: 64 SD rats were randomly divided into model group and control group according to the weigh (which both including 1 d, 3 d, 7 d, 10 d group). The rats of model group would be resected the middle 70% small bowel, and control group only undergoing on-off abdominal surgery. The brains were removed on 1 d, 3 d, 7 d, 10 d postoperatively respectively, and we detected the concentration of 5-HT by high performance liquid chromatography and the mRNA expression of TPH1, TPH2, IDO by realtime fluores-cence quantitative PCR in hippocampus area. Results: Compared with control group, concentration of 5-HT of model group rats increased significantly on 1 d, 3 d postoperatively respectively (P<0.05). And in model group rats, concentration of 5-HT on 1 d, 3 d increased significantly respectively compared with 7 d, 10 d postoperatively (P<0.05), on 3 d to its highest (P<0.05), 7 d, 10 d gradually returned to normal. Compared with control group, mRNA expression of TPH1 in model group rats increased significantly on 3 d postoperatively (P<0.05). And in model group rats, mRNA expression of TPH1 on 3 d increased significantly compared with 1 d, 7 d, 10 d postoperatively (P<0.05). Compared with control group, mRNA expression of TPH2 in model group rats increased significantly on 1 d, 3 d postoperatively respectively (P<0.05). And in model group rats, mRNA expression of TPH2 on 1 d, 3 d increased significantly respectively compared with 7 d, 10 d postoperatively (P<0.05), on 3 d to its highest, 7 d, 10 d gradually returned to normal. Compared with control group, mRNA expression of IDO in model group rats increased significantly on 1d postoperatively (P<0.05). And in model group rats, mRNA expression of IDO on 1 d increased significantly compared with 3 d, 7 d, 10 d postoperatively (P<0.05). Conclusion: In rat model of postoperative fatigue syndrome, concentration of 5-HT increases significantly early postoperatively. Its mechanism refers to up-regulation of mRNA expression of TPH1, TPH2 of 5-HT rate-limiting enzyme.

The effect of Bruceine D on high-risk human papillomavirus 16 infected cell and its possible mechanism

Objective: To investigate the Bruceine D’s (BD) effect on human papillomavirus (HPV) type 16 infected cell in vitro. Methods: The Ect1/E6E7 was cultivated with BD (1, 5, 10, 15 and 30 μmol/L) for 24, 48 and 72 h, the anti-proliferation effect was measured with MTT colorimetric assay; The Ect1/E6E7 was cultivated with BD (1, 5 and 10 μmol/L) for 24, 48 and 72 h, the cell growth cycle change was measured with cell cycle staining solution, the morphologic changes of cell apoptosis stained with Hoechst 33258 in the Ect1/E6E7 cell line were observed under fluorescence microscope treated with BD at concentration of 5 μmol/L for 48 h. Flow cytometry of Annexin V-FITC/PI assay was used to evaluate the apoptosis rate of Ect1/E6E7 cell line treated with BD at different concentrations (1, 5 and 10 μmol/L) in vitro. Cervical cancer cells Caski which contain complete HPV 16 E6, E7 gene, as a positive control, by real-time quantitative PCR to detect the mRNA expression of HPV E6, E7 of those two cells treated with BD. Results: Immortalized human ectocervical Ect1/E6E7 cell line treated with BD showed obviously morphological changes, and with the prolongation of drug action and the increase of drug concentration, the morphological changes became more apparent. BD can inhibit the proliferation of Ect1/E6E7 cell in dose-dependent and time-dependent manner (P<0.05). Ect1/E6E7 cell growth cycle was stagnated in the G1 phase. Hochest fluorescence staining presented the Ect1/E6E7 cells nucleus appeared apoptotic morphological change, and apoptotic bodies can be observed. Flow cytometry showed the apoptosis rate were (6.25±0.76)%, (20.21±1.32)%, (8.61±1.59)%, respectively, after 48 hours cultured with 1, 5, 10 μmol/L of BD (P<0.05). Real-time PCR results showed that after cultivating with 1, 5, 10 μmol/L BD for 48 h, the expression of HPV16 E6, E7 mRNA of Ect1/E6E7 cell were decreased, but compared with the control group, the difference was not statistically significant (P>0.05). The expression of HPV16 E6, E7 mRNA of Caski cell were decreased, the difference was statistically significant (P<0.05) compared with the control group. Conclusion: BD can inhibit the proliferation of Ect1/E6E7 cells significantly, the possible mechanism may be associated with the expression decline of HPV16 E6, E7 mRNA, which promote cell apoptosis.

Prokaryotic expression and immunogenicity of major outer membrane protein 21-387 from Chlamydia trachomatis

Objective: To explore prokaryotic expression and immunogenicity of major outer membrane protein (MOMP21-387) of Chlamydia trachomatis serotype E. Methods: The gene encoding MOMP21-387 was amplified from genome DNA of C. trachomatis E by PCR analysis, and then cloned into pET21a (+) vector to construct recombinant plasmid pET21a (+)/MOMP21-387, and was expressed in the prokaryotic expression system. The MOMP21-387 fusion protein was purified and identified by SDS-PAGE and western blot analysis. The immunogenicity was further assessed by immunizing BABL/c mice, the reactivity of specific serum IgG in serum and genital tract mucosal IgA were tested by ELISA, and the specific cytotoxicity of spleen cells was detected with lactate dehydrogenase (LDH) method. Results: The MOMP21-387 fusion protein was successfully expressed in a prokaryotic expression system, and the specific positive bands at the relative molecular mass (Mr) of about 44 000 was confirmed by SDS-PAGE and western blot analysis; And the purified MOMP21-387 fusion protein was obtained with the Ni-NTA affinity chromatography method. The mice can be induced to produce the specific serum IgG and reproductive tract mucosal IgA by immunizing with the MOMP21-387 fusion protein, and the value of the specific serum IgG and mucosal IgAin immunized groups were significantly higher than that of the PBS control group (P<0.05), the antibodys detected by ELISA reached peak at the 6th week post-immunization. The LDH analysis showed that, the killing rate of spleen cells to the target cell in MOMP21-387 protein immunized groups was significantly higher than those in PBS control group, when the effector cells to target ratio reached to 10:1, 20:1, 40:1 and 80:1 (P<0.05). Conclusion: The MOMP21-387 fusion protein of Chlamydia trachomatis serovar E shows good immunogenicity, It may lay the foundation for the Chlamydia trachomatis ELISA detection and vaccine development based on MOMP21-387.

Different activities of amplitude of low-frequency fluctuation among attention deficit hyperactivity disorder subtypes in brain rs-fMRI

Objective: To study the different activities of amplitude of low frequency fluctuation (ALFF) among attention deficit hyperactivity disorder (ADHD) subtypes and the correlation between ALFF and behaviors. Methods: Seventy-eight participants were checked up by resting state functional magnetic resonance scanner, including 43 ADHD patients (15 of ADHD-I and 28 of ADHD-C) and 35 non-ADHD controls. The amplitude of low frequency fluctuation (ALFF) approach was used to analyse the brain function in resting state, and Conners children’s behavior scale was used to assess the behavior actuality. Results: Compared with the normal controls, ADHD-I showed decreased ALFF in left inferior frontal gyrus, right precuneus, and etc; ADHD-C showed decreased ALFF in left middle frontal gyrus, right precuneus, bilateral limbic lobe cingulate gyrus, and etc; and compared with ADHD-I, ADHD-C showed decreased ALFF in right cuneus, bilateral temporal lobe, limbic lobe cingulate gyrus, and etc. There were many brain regions of frontal/temporal lobe and cerebellum were correlated with Conners Index of Hyperactivity. All of the differences were statistically significant (P<0.05). Conclusion: Amplitude of low frequency fluctuation (ALFF) values of many brain regions were different among ADHD subtypes in rs-fMRI, and most of which were correlated with Conners Index of Hyperactivity.

Silencing gene of heat shock protein 27 increases the sensitivity to 5-FU in colorectal cancer cells

Objective: To investigate the relationship between the HSP27 expression level and the sensitization to 5-FU in different kinds of colon cancer cells, and then explore the effect of HSP27 gene down-regulated by siRNA interference, and the sensitive of 5-FU. Methods: The HSP27 expression level in different kinds of colon cancer cells were detected by Western blot, and the inhibition rates of different concentration of 5-FU were determined by CCK-8 method, then the relationship between the relative level and IC50 of 5-FU was analyzed by Linear correlation. Three siRNA sequences targeted on special sequence of HSP27 gene were designed, All siRNAs were transfected by lipofectamine 2000. The transfection efficiency was observed by laser confocal microscope. The expression of HSP27 was tested with qRT-PCR and Western blot analysis. Different concentrations of 5-FU were added to the cells, which were transfected with siRNA-HSP27 48 h before. Then, the inhibition rate was determined by CCK-8 after 24 h. Results: The correlation between the level of HSP27 expression and IC50 were positive. Real-time PCR and Western blot revealed that two targeted HSP27-siRNA could effectively inhibit the mRNA and protein expression of HSP27. CCK-8 showed that the transfection of HSP27-siRNA can improve the sensitivity of SW480 to 5-FU and the IC50 was downregulated significantly. Conclusion: The correlation between the level of HSP27 expression of colon cancer cells and IC50 of 5-FU are positive. HSP27-siRNA leads to the efficient inhibition of HSP27 expression in SW480 cells, and increase the sensitization to 5-FU.

Predictive value of interleukin-6 and procalcitonin for lower extremity amputation in patients with diabetic foot ulcers

Objective: To determine clinical and laboratory factors that predict amputation surgery and to evaluate the predictive value of interleukin-6 (IL-6), and procalcitonin (PCT) in patients with diabetic foot ulcers (DFUs). Methods: Forty-five DFUs patients admitted to our hospital were consecutively enrolled. The patients’ demographics and wound characteristics were noted. IL-6 and PCT were measured at admission. Results: Twelve of the 45 patients (26.7%) eventually underwent lower extremity amputation. Compared to the non-amputation group, a previous history of amputation (P=0.003), the presence of gangrene (P=0.019), the Wagner grade (P=0.002), the lower extremity neuropathy (P=0.011), the IL-6 concentration (P=0.012), the white blood cell count (WBC) (P=0.033), and the erythrocyte sedimentation rate (ESR) (P=0.040) were significantly high in the amputation group. However, the PCT concentration was not significantly different. Conclusion: PCT failed to predict patients who will eventually require amputation and IL-6 may have predictive value for lower extremity amputation in patients with DFUs. Further studies are needed to confirm its predictive value in patients with DFUs.

Effects of lipopolysaccharide on proliferation and apoptosis in human umbilical vein endothelial progenitor cells

Objective: To investigate the effect of proliferation, apoptosis and cell cycle of lipopolysaccharide (LPS) on human umbilical vein endothelial progenitor cells. Methods: mononuclear cells were isolated from human umbilical cord blood. Mononuclearcells (MNCs) were isolated from human umbilical cord blood in vitro by Ficoll density gradient centrifugation. EPCs were characterized as adherent cells with double positive to DiI-acLDL uptake and lectin binding by direct fluorescent staining under a laser scanning confocal microscope. There were five groups. The control group and four LPS concentration groups: 2.5, 5.0, 10.0, 20.0 mmol/L. MTT was used to detect cell apoptosis and cell cycle. Results: ①10.0 mmol/L LPS promotes proliferation of EPCs, while 20.0 mmol/L LPS inhibits the proliferation of endothelial progenitor cells (P<0.05). ②20.0 mmol/L LPS promotes apoptosis of EPCs, compared with the control, the difference was significant (P<0.05). ③LPS at the concentration of 10.0 mmol/L reduces cells at G0/G1 phase and increases S and G2/M phase cells; 20.0 mmol/L LPS induces EPCs blockade at S phase, G2/M phase cells decreased, compared with the control, the difference was significant (P<0.05). Conclusion: 10.0 mmol/L LPS promotes the proliferation of EPCs. 20.0 mmol/L LPS inhibits the proliferation and promotes apoptosis of EPCs.

1H NMR-based metabonomic study on cerebellum of type 2 diabetes mellitus model

Objective: To investigate the diabetes-related metabolic changes in the cerebellum of db/db mice. Methods: Specimens of the cerebellum were dissected from the 17-week db/db mice and the age-matched controls. The cerebellum tissue samples were extracted and analyzed by 1H NMR-based metabonomics. Results: Compared to the controls, the db/db mice exhibited markedly different metabolic profiles, which included significant decrease in the levels of glycine (P<0.01), aspartate (P<0.01), glutamate (P<0.01), GABA (P<0.01), N-acetyl-aspartate (P<0.01) and alanine (P<0.05) and increase in the levels of glutamine (P<0.01), taurine (P<0.05) and lactate (P<0.01). Conclusion: These metabolic changes suggest that glucose metabolism and amino acid metabolism in the cerebellum of db/db mice are disordered, which provides clues for further understanding the pathogenesis of diabetic encephalopathy.

Genetic polymorphism of nine short tandem repeat loci in han ethnic population in Wenzhou

Objective: To analyze the alleles and genotype frequencies of nine short tandem repeats loci, those are D18S1364, D12S391, D13S325, D6S1043, D2S1172, D11S2368, D22-GATA198B05, D8S1132, D7S3048, of Han Ethnic Population in Wenzhou. Methods: extract DNA respectively from anticoagulant of 355 unrelated individual samples of Han Ethnic population living in Wenzhou. Deal the nine STR loci of the samples with multiple PCR amplification by STR_Typer_10_v1 kit. Analyze the PCR products by 310 genetic analyzer and GeneMapper ID 3.2v software of AB company. Deal the results with statistical analysis on the allele frequency and common forensic parameters by PowerState V12.xls software. Results: 15, 12, 9, 17, 15, 13, 11, 10 and 12 alleles were observed respectively from the nine STR loci. The distribution of genotype frequencies matched the Hardy-Weinberg equilibrium, P was more than 0.05. The heterozygote was more than 0.7831. The polymorphism information content was more than 0.7666. The combined power of discrimination was more than 0.9266. Conclusion: All of the nine STR loci of Han Ethnic Population in Wenzhou in this study have high genetic polymorphism, and it can be used as good genetic markers. The data obtained in this study can be used in individual identification, paternity test and population genetics investigation of Han Ethnic population living in Wenzhou.

Effect of dexmedetomidine on serum visfatin and high-sensitive C-reactive protein in patients with severe craniocerebral trauma

Objective: To explore the effect of dexmedetomidine on serumvisfatin and high-sensitive C-reactive protein in patients with severe craniocerebral trauma. Methods: Seventy-two cases of severe craniocerebral trauma patients received emergency operation from August 2011 to August 2013 in our hospital were selected as research subjects, the patients were randomly divided into treatment group (n=36) and control group (n=36), patients in treatment group received intravenous injection of dexmedetomidine 0.5 μg/(kg•h) from the time before anesthesia induction to postoperative 3 days, patients in control group were given equal volume of saline. Venous blood samples were collected to determine serumvisfatin, Hs-CRP level before anesthesia induction (T0), at the end of the surgery (T1), on postoperative 1 day (T2), 3 days (T3), 5 days (T4) and 7 days (T5) respectively. Glasgow outcome scale (GOS) score was recorded after surgery for six months. Results: ①Compared with T0, serumvisfatin and Hs-CRP of two groups were significantly higher at T2-T4 (P<0.05), both indexes of treatment group at T5 were similar with those at T0, while both indexes of control group at T5 were significantly higher than those at T0 (P<0.05). Compared with control group, serumvisfatin and Hs-CRP of treatment group were significantly lower at T2-T5 (P<0.05). ②Spearman correlation analysis showed that the serum visfatin level was positively correlated with Hs-CRP (r=0.462, P=0.000), and also positively correlated with WBC count (r=0.221, P=0.028). ③Multi-factor logistic regression analysis showed that serum visfatin (OR=1.733, P<0.05) and Hs-CRP (OR=3.184, P<0.05) were positively correlated with poor prognosis (GOS score after six months<3 points). ④Rate of good prognosis in treatment group (88.9%) was significantly higher than that in the control group (66.7%) in term of GOS score after six months (x2=5.143, P=0.233). Conclusion: Serum visfatin is closely related to inflammatory response after brain injury and it companied with Hs-CRP are high risk factors for poor prognosis in patients with severe craniocerebral injury. Dexmedetomidine can obviously reduce serum visfatin and Hs-CRP levels and thus inhibit inflammation response so as to improve the prognosis of patients.

Efficacy of dexmedetomidine on mechanical ventilation after uvulopalatopharyngoplasty

Objective: To compare the effect of sedation between dexmedetomidine and midazolam for severe obstructive sleep apnea hypopnea syndrome (OSAHS) after uvulopalatopharyngoplasty (UPPP). Methods: Sixty-four ASA I~II patients of severe OSAHS scheduled for UPPP were randomly allocated into group D (dexmedetomidine, n=32) and group M (midazolam, n=32). The patients of the two groups were sedated with dexmedetomidine or midazolam respectively in the ICU after surgery and Ramsay scores of 3-4 were maintained. Sufentanil were added for analgesia. The supposed hypertension or tachycardia were treated with extra medication. The administration of sedative was stopped after 20 hours stay in ICU and the patients were sent back to ward when the Aldrete score reached 10. Results: The anticipated depth of sedation were acquired in 63 patients and there was a significant difference (P<0.05) demonstrated by 24 patients in group D while 8 patients in group M received no extra medication for hypertension or tachycardia. The level of stress hormone, the mean arterial pressure and heart rate were significantly lower in group D than group M at 2 h (T1), 10 h (T2), 20 h (T3) after entering ICU and before extubation (T4) respectively. The time intervals between medication suspend and departure from ICU were significantly shorter while the degree of satisfaction for the process of sedation were higher in group D than that in group M. Conclusion: Dexmedetomidine has better features of sedation than midazolam for the patients of severe OSAHS with mechanical ventilation after UPPP in ICU.

Salvage of failed nail-plate system internal fixation for peritrochanteric hip fractures

Objective: To explore the methods and efficacy of salvage of failed nail-plate system internal fixation for peritrochanteric hip fractures. Methods: The clinical data of failed intertrochanteric area fracture and nonunion after surgery in 28 cases admitted to the Second Affiliated Hospital of Wenzhou Medical University from January 2007 to January 2013 were retrospectively analyzed. Results: The mean operation duration was 170 min (110-280 min). Follow-up time was 7-79 months (mean 29 months). Healing of internal fixation of fracture was 20 cases, the healing rate being 95.2% (20/21). Postoperative hip pain improved significantly, only 3 cases were mild pain, 1 case moderate pain. Ability to walk after surgery: 23 cases of normal walking, 4 cases needed to help with the cane to walk, 1 case was unable to walk. Postoperative Harris hip average score was 83 points (33-100 points) in 28 cases, of whom excellent was in 16 cases, good in 5 cases, medium in 6 cases and poor in 1, excellent rate being 75.0%. Measurements of neck shaft angle was 125° (100°-140°) in X-ray film. Conclusion: Treatment method usually includes the replacement of the prosthesis and re-fixation, after intertrochanteric fracture fixation screw-plate system is failure. The same internal fixation plus graft treatment is used again for young patients and some ability, good bone and good hip status in elderly patients. Selection of kinds of internal fixation bases on types of fixation and bone defects. PFN, DCS and DLK are better choices. Joint replacement is adopted for lower activities and poor bone, head and neck bone defect absorption or hip damage in elderly patients, which are able to obtain satisfactory clinical results.

Comparison of the anesthetic efficacy of different anesthesia in endoscopic sinus surgery

Objective: To compare the effect of total intravenous anesthesia with sevoflurane inhalation anesthesia in endoscopic sinus surgery. Methods: Forty patients of ASA I and II undergone selective endoscopic sinus surgery were randomly divided into two groups: group T (remifentanyl 5~10 μg/kg/h and propofol 4~8 mg/kg/h maintenance anesthesia), group S (sevoflurane 0.8~1.0 MAC maintenance anesthesia), and induced by intravenous in both groups. The fluctuation of the BIS values should be controlled between 45 and 60 during the surgery. Hemodynanmic parameters including MAP, HR, and SpO2 were recorded continuously, especially before induction (T0), after induction before intubation (T1), 1 minute after intubation (T2), 3 minutes after intubation (T3), 1 minute after extubation (T4), and 3 minutes after extubation (T5). Operation time, recovery time, extubation time, the case of using hypotensive drugs during perioperation and the incidence of side effects after postoperation such as dysphoria, PONV, severe sedation were also recorded. Results: Compared with group S, the MAP at T3 and the SpO2 at T5 were declined in group T (P<0.05). Compared with T0, the MAP at T1~T3 were lower and at T4 was risen in group S (P<0.05), the HR at T4 was accelerated in group S (P<0.05). The MAP at T1~T3 was lower in group T (P<0.05), and the HR at T3 was slower in group T (P<0.05), the SpO2 at T1~T3 was risen and T5 was lower in group T (P<0.05). Compared with S group, the recovery time in group T was shorter (P<0.05), the incidence of emergence agitation in group T was lower (P<0.05), and the Ramsay sedation score in group T was higher (P<0.05), the cases of patients need controlled hypotension were less (P<0.05). Conclusion: The anesthesia effectiveness of propofol/remifentanyl intravenous anesthesia is more suitable and safer for endoscopic sinus surgery than sevoflurane inhaled maintenance anesthesia, the MAP is easier to adjust for the satisfaction of requirement of operation, revival time is faster, but incidence of agitation is loss.

HLA-B*5801 carrying station and the correlation of allergic reactions caused by allopurinol

Objective: To investigate the HLA-B*5801 carrying station and it’s correlation of allergic reactions caused by allopurinol of our hospitalized patients. Methods: The patients’ data from inpatient department were collected, then their HLA-B*5801 was analyzed and detected. Results: There were 100 cases determined the HLA-B*58 allele, and the HLA-B*5801 allele positive cases were 23, including 8 cases with allergic reaction. HLA-B*5801 negative one did not happen allergic reaction. Conclusion: Our patients carried high rate of HLA-B*5801, it will be more safe to detect the gene in time before use allopurinol. But the association between HLA-B*5801 and allergic reaction caused by allopurinol remains further research.
