1 Prokaryotic expression of the Z domain of Staphylococcus protein A and preparation of its polyclonal antibody

Objective: To prepare the Z domain of Staphylococcus protein A(SpA-3Z) expressed in prokaryotic cell and its polyclonal antibody, and dentify the specific activity of the antibody. Methods: The tandem repeats (SpA-3Z) were formed by three SpA-Z domains which linked with flexible linker peptide (GS). After prokaryotic codon optimized, the whole tandem repeats gene sequences were synthesized and then cloned into an expression vector pET21a(+) and constructed the recombinant plasmid pET21a(+)/SpA-3Z. After sequencing, the recombinant plasmid was transformed to E.coli BL21 (DE3) and induced expressed by Isopropyl β-D-1-Thiogalactopyranoside (IPTG), the recombinant protein (SpA-3Z protein) was purified by Ni-NTA affinity chromatography, and identified by SDS-PAGE and Western Blot analysis. Then the rabbits were immunized with the SpA-3Z protein, the sera samples were collected before and after immunization, in order to assay the response and titer of the serum polyclonal antibody by ELISA, and the combining specificity to natural SpA protein by Western Blot analysis. Results: The pET21a(+)/SpA-3Z recombinant plasmid was successfully constructed, and the SpA-3Z recombinant protein was expressed in prokaryotic expression system and the purified SpA-3Z was prepared, the molecular weight of SpA-3Z protein was about 21 kDa, which conform to its theoretical molecular weight, indicated by SDS-PAGE and also confirmed by Western Blot analysis. The specific serum polyclonal antibody IgG was induced by immunization with SpA-3Z in the rabbits, and its titer was up to 1:40 000. The Western Blot analysis showed that the single band appeared in the molecular weight of about 42 kDa position (natural SpA protein), indicated that the rabbit serum polyclonal antibody could specifically recognize the natural SpA protein. Conclusion: The SpA-3Z recombinant protein shows strong immunogenicity and antigenicity. The rabbits serum polyclonal antibody IgG shows high titer and good specificity, and can be used in the further study of SpA-Z related biological and immunological function.

6 Effect of enteral nutrition containing slow-release starch and high-monounsaturated-fat on blood glucose and lipid metabolism in patients with impaired glucose tolerance

Objective: To investigate the effects of enteral nutrition containing slow-release starch and high-monounsaturated-fat on blood and lipid metabolism in patients with impaired glucose tolerance. Methods: Sixty-two patients with impaired glucose tolerance from out-patient clinic were randomized to experimental meal group (Abbott Glucerna SR, n=32) and common breakfast group (isocaloric normal oral diet, n=30), the fasting plasma glucose, 2-hour postprandial plasma glucose, fasting insulin, 2-hour postprandial insulin, serum lipid profile were examined before and after 4-week intervention, and the parameters were compared within groups and between groups. Results: The 2-hour postprandial plasma glucose, postprandial serum insulin, serum triglyceride, total cholesterol and low-density lipoprotein cholesterol significantly decreased in experimental meal group (P<0.05 or P<0.01), when compared with that of baseline; After 4-week intervention, these parameters were statistically lower than that in common breakfast group (P<0.01 or P<0.05). Conclusion: Dietary consumption of slow-release starch and high-monounsaturated fat can improve glucose metabolism and lipid control in patients with impaired glucose tolerance.

10 Effect of exogenous gonadotrophin on human fetal ovarian tissue after xenotransplantation

Objective: To investigate the effect of exogenous gonadotrophin on the follicular development of cryopreserved human fetal ovarian tissue after xenotransplantation. Methods: The cryopreserved fetal ovarian tissues were grafted under kidney capsules of the nude mice. Mice randomly assigned to three different groups (A, B, C), and were treated with PMSG by different ways. Grafts of each group were recovered in batches at 10-14 weeks, 18-22 weeks, 26-30 weeks after operation. Grafts were evaluated using conventional histology and detected proliferating core antigen (PCNA) using immunohistochemical method. Results: There was no significant difference among three groups of preantral follicles. At 26-30 weeks, the numbers of secondary follicles in group C were significantly less than that in group A and B (P<0.05). Antral follicles were discovered in each group. Conclusion: The follices of the cryopreserved human fetal ovarian tissue can develop to antral follicle without exogenous hormone after xenotransplantation, and follicles can be promoted by exogenous gonadotropin.

14 Intervene of Hedgehog signal on expression of proliferating cell nuclear antigen in renal tubular epithelial cells

Objective: To explore and investigate the activation and inhibition of Hedgehog signal regulated on expression of proliferating cell nuclear antigen (PCNA) in renal tubular epithelial cells (NRK-52E). Methods: NRK-52E cells were randomly divided into control group (treated with solvent only), activated group (treated with Shh at the concentrations with 10 μg/L) and treatment group (treated with Shh at the concentration of 10 μg/L plus cyclopamine at the concentrations with 5 μmol/L). After cultured for 24 h, immunofluorescence staining was used to evaluate the expressions of Ptch1, Smo, Gli1, and PCNA. The expression levels of Ptch1 and Smo mRNA were quantified by real-time RT-PCR. Results: NRK-52E cells without drug treatment maintain slowed proliferation state, and HH signal was not activated, and PCNA was in low expression. Treatment with exogenous recombinant protein Shh for 24 h, the mRNA or protein expression levels of Smo and Gli1 were significantly increased, and the expression of Ptch1 was decreased, suggesting that Shh activates the HH signal (P<0.05). In this process, the expression of PCNA protein was significantly increased. NRK-52E cells showed proliferation, and morphologic change occurred. After cyclopamine intervention, the activity of HH signal was decreased (P<0.05), and PCNA expression was also inhibited. In addition, abnormal proliferation of NRK-52E cells was inhibited, and increased apoptosis and necrosis cells. Conclusion: HH signal is involved in the proliferation of renal tubular epithelial cell. Regulating the activation of HH signal influences PCNA expression and cell proliferation.

18 Analyses of protein losses via dialysate and its influencing factors in peritoneal dialysis patients

Objective: To explore the daily protein losses via dialysate and its influencing factors in continuous ambulatory peritoneal dialysis (CAPD) patients, to provide theoretical basis for clinical nutrition support. Methods: Seventy two non-diabetic nephropathy patients with uremia and treatment with CAPD for longer than 3 months were recruited during December 2012 to October 2013 from a general hospital. General information, laboratory testing data, and global assessment of nutritional information of the patients were collected. Dietary surveys were conducted for 7 consecutive days to obtain protein and energy intake in patients. According to the amount of protein losses after dialysis, patients were divided into two groups, low protein losses group (<5 g•d-1) and high protein losses group (≥5 g•d-1). Relevant indicators were compared between two groups, logistic regression analysis was applied to analyze the factors which affect the level of protein loss in patients. Results: The median of protein losses in CAPD dialysis patients was 5.0 (range: 1.7-21.1) g•d-1, which was about 13.0 percent of total daily protein intake (95%CI: 10.8%-15.2%). Patients in high protein losses group showed a lower serum albumin level (P<0.05 ), higher serum creatinine, and higher urea nitrogen levels (P<0.05). There was no statistically significant difference between two groups in prealbumin, hemoglobin, transferrin, SGA score, nPNA and anthropometric measures such as BMI, MAMC, and MAC (P>0.05). Long dialysis duration, high peritoneal transport, high blood urea nitrogen, and male were risk factors of high protein losses by dialysate. Conclusion: CAPD patients may lose substantial amount of protein by dialysis solution, the loss amount is related with duration of dialysis, peritoneal transport characteristics, blood urea nitrogen level, and gender. The protein losses over 5 g•d-1 by dialysate may lead to lower serum albumin level. In terms of patients whose protein loss amounts were higher than 5 g•d-1 by dialysate, it is necessary to improve their current DPI levels (0.84 ± 0.25) g•kg-1•d-1).

22 Study on the expression of plasma H2 relaxin level in patients with chronic heart failure

Objective: To explore the expression of endogenous H2 relaxin hormone in chronic heart failure patients, and the relations with NYHA classification, to study its effect on the prognosis of patients with chronic heart failure prediction. Methods: One hundred and forty patients were selected for reseach from July 2012 to September 2012, in the First Affiliated Hospital of Wenzhou Medical University, 109 cases for chronic heart failure group and 31 cases for control group. In the early morning of the first day after admission, patients in each group were exsanguinated in supine, resting status for some venous blood to detect, LDL-cholesterol, hemoglobin, fasting blood glucose, creatinine clearance rate, brain natriuretic peptide (BNP) and so on. ELISA was performed to analyze the plasma concentration of H2RLX, collagen I. In addition, echocardiography inspection was given to estimate left ventricular ejection fraction. Results: ①Plasma H2RLX level of chronic heart failure patients [0.604 (0.419-0.743)] pg/mL was significantly higher than that of control group [0.390 (0.355-0.425)] pg/mL. The difference was statistically significant (P<0.05). ②The tendency as classification of NYHA showed that the plasma level of H2RLX and collagen I both increased in all of heart failure (HF) patients as aggravation of cardiac dysfunction. The difference was statistically significant (P<0.05). ③Plasma level of H2RLX and type I collagen showed significant correlation, there was statistically significant (r=0.889, P<0.001). ④H2RLX levels in chronic heart failure patients could predict the cardiovascular event in one hundred and eighty days. Conclusion: The H2RLX levels elevate in patients with chronic heart failure, which may involve in ventricular fibrosis; H2RLX may help to make preliminary estimation of the severity of chronic heart failure and H2RLX plasma levels may predict the prognosis of cardiovascular events.

26 Antioxidant protection against PC12 cells apoptosis induced by H2O2 and the preliminary mechanism of acylated 4-hydroxy chalcones

Objective: To investigate the protective function of previously synthesized and screened chalcones B1 and B2 against PC12 cells apoptosis induced by H2O2, and their impact on Nrf2/ARE signaling pathway. Methods: A model that PC12 cells were damaged by H2O2 was established, and MTT assay was used to detect the antioxidant activity as well as cytotoxicity of compounds B1 and B2. Real-time PCR was used to determine mRNA expression of Nrf-2 regulated genes GCLC and HO-1. Hoechst staining was applied to test the inhibitory ability against H2O2-mediated PC12 cells apoptosis. Results: B1 and B2 showed no protection on PC12 cells induced by H2O2 in case of 1 hour incubation, while they represented potent antioxidant efficiency after incubation for 24 hours, and both were nontoxic at 10 μmol/L. B1 showed no obvious effect on the expression of GCLC and HO-1 which were Nrf2-regulated downstream genes. B2 significantly prevented the PC12 cells apoptosis caused by H2O2 with activating the expression of GCLC and HO-1. Conclusion: B1 and B2 show a significant antioxidant activity against H2O2-induced PC12 cells injury. The possible mechanism of B2 may be its activation of Nrf2/ARE signaling pathways while B1 may perform the antioxidant action via other route.

31 The relationship among left ventricular function of obese children and serum levels of matrix metalloproteinases and their inhibitors

Objective: Through the study of serum levels of matrix metalloproteinases (MMPs) and its inhibitors (TIMPs), to investigate obesity children at high risk of developing cardiovascular disease impaction on cardiac function. Methods: Blood lipid, systolic pressure, diastolic pressure, blood sugar levels were examined in 30 simple obesity children and 30 normal children; echocardiography (ECHO) test cardiac function; MMPs and TIMPs using enzyme-linked immunosorbent assay (ELISA) detection. Results: ①Blood lipids, systolic pressure, diastolic pressure, blood sugar level in obese children were significantly higher than that in control group (P<0.05); ②Serum levels of MMPs in obese children were significantly higher than that in the control group (P<0.05), while TIMPs levels were lower than the normal control group (P<0.05); ③The left ventricular function changed in obese children, LVEDD, LVESD, LVPWT, IVST levels were significantly higher than that in control group (P<0.05); ④The left ventricular function indicators of obese children were positively correlated with children, the levels of MMPs were positively correlated with the left ventricular function index MMP-8, MMP-9 levels, prompting obesity increases the risk factor for cardiovascular disease, MMPs might participate in the structure remodeling of the left ventricle in obese children, thus caused the change of left ventricular function.correlated. Conclusion: Serum levels of MMPs and TIMPs had changed in school-age obesity children, the levels of MMPs were positively correlated with the left ventricular function index, suggesting obesity increases the risk factor for cardiovascular disease, MMPs may participate in the structure remodeling of the left ventricle in obese children, thus caused the change of left ventricular function.

36 Comparison of Femoston and Progynova applied in the cycle of frozen-thawed embryo transfer

Objective: To compare the effect of Femoston and Progynova applied in the hormone replacement cycle for frozen-thawed blastocyst transfer. Methods: Two hundred and eight blastocyst FET cycles were analyzed retrospectively from July 2011 to December 2012. All cases were divided into two groups: 107 Femoston plus progesterone protocol group (group A), 173 Progynova plus progesterone protocol group (group B). Patient’s age, basic endocrine, endometrial thickness on the day of progesterone initiation, mean dosage and duration of estrogen per cycle, high quality embryo rate, mean number of embryo transferred per patient, implantation rate, clinical pregnancy rate, early abortion rate and ectopic pregnancy rate were compared between the two groups. Results: Before progesterone initiation, the duration of estrogen used in group A and group B was (9.80±2.81) d and (10.84±4.12) d and the dosage of estrogen used in group A and B was (15.55±10.42) mg and (58.80±24.57) mg respectively. The estradiol level on the day of progesterone initiation in group A and group B was (2 955.58± 1 983.94) pmol/L and (923.25±441.52) pmol/L, respectively. They all had significant differences (P<0.05). Clinical pregnancy rate were 57.94% (62/107) and 55.49% (96/173), respectively, and there was no statistically significant difference (P>0.05). Patient’s endometrial thickness on the day of progesterone initiation, high quality embryo rate, mean number of embryo transferred for per patient, implantation rate, clinical pregnancy rate, early abortion rate and ectopic pregnancy rate in these two groups had no significant different (P>0.05). Conclusion: In hormone replacement cycle for frozen-thawed embryo transfer, the use of Femoston can obtain the similar pregnancy rate compared with Progynova. So Femoston can be used as estrogen alone in blastocyst transplant recovery cycle.

40 Reliability and validity of simplified Chinese version of the Iowa infant feeding attitude scale among fathers

Objective: To test the reliability and validity of simplified Chinese version of the Iowa infant feeding attitude scale (IIFAS) among fathers. Methods: Totally 204 fathers were investigated with simplified Chinese version of the Iowa infant feeding attitude scale. Cronbach’s alpha and exploratory factor analysis were used to test the reliability and validity of the scale respectively. Results: The critical ratio of the items ranged from to 3.747 to 7.800. The correlation coefficients between each item with the total score ranged from 0.287 to 0.549. The Cronbach’s alpha was 0.672 and the Cronbach’s alpha of each factor ranged from 0.500 to 0.712. The construct validity was confirmed by factor analysis and 54.054% variance was explained by 4 factors, the factor loading value ranged from 0.436 to 0.885. The predictive validity was acceptable. Conclusion: The simplified Chinese version of the Iowa infant feeding attitude scale is a reliable and valid instrument. It can be used to measure the infant feeding attitude of fathers in China.

45 Study of screw pilot channel placement in the locking plates using the safety dril

Objective: To make use of the safety drill to deal with the placement of the screw pilot in the femoral head through proximal femoral locking plate, test the safety properties of the new safety drill compared with conventional drill. Methods: Twenty femoral heads for hole-drilling research were taken from the femoral neck fracture patients while having the hip arthroplasty in the department of orthopaedic in the 1st affiliated hospital of Wenzhou Medical University. Numbered all these femoral heads, divided them randomly into 2 groups. The N group was carried out drilling test with the new safety drill, while the O group with the traditional drill. All femoral heads were fixed firmly into Fixture in the self-made box in the same manner. Group N was carried out drilling test by new drill. All the participants were asked to perform 4 holes through the prepared pilot apparatus of the locking plate in the same order, starting with new drill clockwisely twice and then anti-clockwisely twice. They were informed to drill as deeply as they could do, but not to penetrate the joint. The drilling depth was recorded in millimeters by depth sounder after all the holes was drilled. Then they were ask to penetrate all the hole and the whole drilling depth were obtained. The difference between the two records was define as TED (tip-to-edge distance) for evaluation. Group O was carried out drilling test with traditional drill. All the participants were asked to perform 4 holes in the same manner as mentioned above. But they were informed to penetrate all the joint and record the perception of dynamics using Visual analog scales (VAS) for evaluation. Results: Ten orthopaedic surgeons took part in the tests, Mean TED for Nc, Na, Oc, Oa were respectively 5.75, 2.95, 6.55 and 3.20 mm. Mean VAS for Nc, Na, Oc, Oa were respectively 5.90, 9.55, 2.85 and 6.75 score. The drill type (factor A) for TED had no significant difference (FA=3.2603, P=0.1263) while the drill direction (factor B) for TED was statistically different (FB=294.1222, P=0.0000). The factor A and factor B for VAS had both statistically significant difference (FA=158.0891, P=0.0000; FB=378.8507, P=0.0000). Conclusion: The safety drill with reverse drilling technique (Na group) offersd a better TED while showing a higher safe threshold for penetration, which means a greater stability between screws and lower incidence of UJP.

50 Application of arterial interventional chemotherapy and embolization in advanced-stage epithelial ovarian cancer

Objective: To explore the signicance of the arterial interventional chemotherapy and embolization in the treatment of advanced-stage epithelial ovarian cancer. Methods: From December 1, 1999 to May 31, 2012, 100 patients of epithelial ovarian cancer in advanced-stage were admitted and all of them had cytoreductive surgery in our hospital. According to whether or not to accept the arterial interventional chemotherapy and embolization, patients of advanced-stage epithelial ovarian cancer were divided into two groups before cytoreductive surgery. Those were the arterial interventional chemotherapy and embolization group (the treated group) and primary cytoreductive surgery group (the control group). There were 27 cases in the treated group which had 1-2 courses of the arterial interventional chemotherapy and embolization before operation. While the other 73 cases in the control group had initial cytoreductive surgery. The results were analyzed retrospectively, comparing the two groups of patients with clinical indicators, related prognostic factors and the cumulative survival rate. Results: Comparison of factors between the two groups showed that the treatment group with high grade, stage late accounts for the proportion to be high. However, the rates of optimal cytoreductive surgery in the treated group were higher than that of the control group. There was no statistically significant difference between the two groups (P>0.05). Multivariate survival analysis showed that pathological staging and operation satisfaction were independent prognostic factors affecting overall survival rates for patients (P=0.010 and P=0.011). The 3-year and 5-year cumulative survival rates were 64% and 48% in the treated group, while 62% and 40% in the control group. There was no statistically significant difference between the two groups (P>0.05). Coclusions: Patients of advanced-stage epithelial ovarian cancer who can not to achieve optimal cytoreductive surgery should receive preoperative arterial interventional chemotherapy and embolization. It may improve the success rate of the operation, but can not improve the overall cumulative survival rate.

55 CT diagnosis of pulmonary hamartoma without typical calcified signs

Objective: To summarize the CT features of pulmonary hamartoma without typical calcified, in order to improve the clinical diagnostic accuracy. Methods: Retrospective analysis with 11 cases of pulmonary hamartoma confirmed by pathologically from June 2008 to October 2013. Ten patients underwent dynamic contrast-enhanced CT scans, 1 patient underwent ordinary routine CT scan. Results: A total of 11 cases of which 7 cases occurred in right lung, 3 cases in left lung, 1 case in the trachea. the tumor size range from 0.7 cm × 0.7 cm to 3 cm × 2.5 cm, with an average diameter of 1.8 cm. Fat density could be detected in 3 cases, 6 cases showed the soft tissue density. Calcification density couldn’t be measured in all lesions. Six cases of nodules showed the lobulation sign, but all of the tumors showed no spiculation sign and no pleural indentation sign and no vascular convergence sign. After dynamic enhancement, 9 cases enhanced CT values were less than 20 HU, 1 case enhanced CT values between (30~40) HU. Conclusion: The pulmonary hamartoma without typical calcified shows the benign tumor characteristics. The lobulation sign is relatively common, so it is easily be misdiagnosed as lung cancer, in thin slice CT scan, the small fat particles can make a definite diagnosis.

58 Analysis of the related factors on the diagnostic changes of somatoform disorder

Objective: To investigate the development and changes in the diagnosis of patients with somatoform disorders and diagnosis changes with related factors. Methods: Using a retrospective case-control study, a new diagnosis on somatoform disorder patients in the year of 2002 were renewed diagnosis after ten years, totally 196 cases involved. The diagnosis of 80 cases of changes in patients as study group, maintain the diagnosis of 116 patients were the control group for Multivariate Logistic regression model for analysis of diagnosis of change. Results: 41% of cases had changed with related factors such as family psychotic, age of onset, trigger factors, frequent relapse. And significantly difference were found in the seven early symptoms of psychosis in all eight early psychiatric symptomsin for the 2 group (P<0.01). Social withdrawal, role function obstacle, weird act, slow emotion, difficulties in thinking made impact on changes of somatoform disorder diagnosis (P<0.05). Conclusion: Patients with somatoform disorder with the development of time has a high rate of diagnostic change, the early symptoms of psychotic patients are more likely to develop into mental disease.

61 The effects of L-carnitine on patients with fatty liver

Objective: To investigate the effects of L-carnitine on regulating blood lipids, transaminase, hepatic steatosis and fatty of patients with fatty liver. Methods: Dietary intervention with self-control method was adopted in 32 patients with fatty liver living one community. All participants take plus two oral L-carnitine tablets twice a day for three months. Blood lipids, transaminase, hepatic steatosis and fatty prior to intervention were tested and compared to those of after intervention in all participants. Results: After 3 months’ dietary intervention, ALT, AST, TBIL, TG, TC, LDL of patients were significantly decreased in contrast to those of prior intervention (P<0.01), while HDL was significantly increased (P<0.01). Body weight, body fat index, waistline, abdominal circumference, hips, waist-hip ratio of all participants were decreasing (P<0.01), respectively, meanwhile the subcutaneous fat thickness (arm, back, abdomen) also significantly decreased in contrast to that of prior intervention (P<0.01). Moreover, liver/spleen ratio was significantly increased after intervention (P<0.01), and no side effects was observed in all participants during the trial. Conclusion: As a dietary supplement, L-carnitine can improve the blood lipids, hepatic steatosis, reduce the patient’s transaminase and weight. The dietary intervention strategy is convenient and feasible for patients with fatty liver, and also easy for long-term use.

65 Study on impairment of sensory integration ability and intelligence in post-hand-foot-mouth disease combined with encephalitis

Objective: To study the impairment of brain function in post-hand-foot-mouth disease combined with encephalitis. Methods: Thirty-five post-hand-foot-mouth disease combined with encephalitis and 71 healthy controls were compared on various sensory integration ability and intelligence. Results: The prevalence of sensory integrative dysfunction in encephalitis group was higher than that in the control group (57.1% vs 32.4%, P<0.05). Significant differences were found between groups on measures of the total development quotient (91.5±10.0 vs 96.7±6.8, P<0.05). Conclusion: Impairment of sensory integration ability and intelligence is found in post- hand-foot-mouth disease combined with encephalitis infants. It is important to cause the attention of doctors and parents about this dysfunction.
