Expression of Smad1/5/8 gene in the rat myocardium after the inhibition of ALK-3 gene and Pax-8 gene

Objective: To explore the effect of Smad1/5/8 by down-regulating selectively the level of expression of paired box gene8 (Pax-8) or bone morphogenetic protein receptor type IA (BMPR-IA, also named ALK-3) in rat myocytes by RNA interference. Methods: The primary cultured H9C2 (2-1) myocytes were divided into four groups: short interference RNA targeting Pax-8 (Pax-8 siRNA) group, short interference RNA targeting ALK-3 (ALK-3 siRNA) group, non-specific siRNA group as the negative control (NC siRNA), and the blank control group. The former three groups were treated with siRNA-liposome compounds consisting of siRNA (5.0 μL) and Lipofectamine 2000 (5.0 μL), and the blank control group was treated with equal volume of culture medium. qRT-PCR was used to analyze the level of expression of Pax-8 mRNA and ALK-3 mRNA. Western blot was used to analyze the level of expression of phosphorylation protein Smad1/5/8 and Caspase-3. Results: In comparison with NC siRNA group and blank control group, the level of expression of Pax-8 mRNA in Pax-8 siRNA group was downregulated 50% and 54% respectively (both P<0.05), the level of expression of ALK-3 mRNA in ALK-3 siRNA group was downregulated 49% and 53% respectively (both P<0.05), while no significant difference was found between the NC siRNA group and blank control group. After the transfection, the level of expression of p-Smad1/5/8 protein in ALK-3 siRNA group was significantly lower than the level of the blank group (P<0.05) and NC siRNA group (P<0.05), while there was no significantly difference between the blank group and NC siRNA group. The level of expression of Caspase-3 protein in the Pax-8 siRNA group and ALK-3 siRNA group was significantly higher than the level of the blank group (P<0.05) and NC siRNA group (P<0.05), while there was no significantly difference between the blank group and NC siRNA group. Conclusion: To down-regulate the level of expression of Pax-8 mRNA or ALK-3 mRNA may promote apoptosis protein Caspase-3 in myocardial cells. To down-regulate the level of expression ALK-3 mRNA can reduce Smad1/5/8 protein phosphorylation, suggesting Smad1/5/8 plays an important role after the gene causing interference in ALK-3 in increasing apoptotic protein Caspase-3.

Correlation between human cytomegalovirus infection and systemic lupus erythematosus

Objective: To analyze infective status of the human cytomegalovirus (HCMV) in systemic lupus erythematosus (SLE) patients and explore its relevance to clinical indicators of SLE. Methods: Peripheral blood leukocytes (PBLs) and serum samples were collected and separated from 60 patients with SLE and 111 healthy individuals. Highly sensitive and specific PCR method was used to investigate HCMV-UL55 gene and UL138 gene in PBLs, whereas Roche’s electrochemiluminescence detection kit was used to investigate the HCMV-specific serum IgG and IgM. The positive rates and correlation between UL55, UL138 genes detection in PBLs and sera IgG, IgM in SLE patients were explored in SLE patients and healthy individuals. The relationship between cellular HCMV infection in PBLs and common clinical indicators of SLE was further analyzed. Results: Agarose gel electrophoresis and sequencing analysis showed established PCR could be used to detect HCMV-UL55 gene and UL138 gene. HCMV IgG in serum and HCMV UL138 gene detection in PBLs were almost all positive, there was no significant difference between SLE patients and healthy individuals (P>0.05). On the contrary, the positive rate of HCMV serum IgM test was low; the positive rate of SLE patients (6.67%) was slightly higher than that of the healthy individuals (0.90%) (P=0.052); While the HCMV infection rate by detecting HCMV-UL55 gene in PBLs was significantly higher in patients with SLE than that in the healthy control (P<0.01). Analyzing the correlation among the four detection methods of HCMV infection, the HCMV-UL138 gene detection in PBLs had a good consistency with HCMV-specific serum IgG test. Analysis of HCMV infection in PBLs and clinical indicators of SLE patients, unlike HCMV-UL138 gene, a number of clinical indicators were significantly higher in the SLE patients with HCMV-UL55-positive PBLs than that in the SLE patients with HCMV-UL55-nagtive PBLs. Conclusion: There is a certain correlation between HCMV infection and SLE, but the results of different detection methods are significantly different.

Establishment and its significance of the method for the detection of HPV16 early gene E6-related transcripts in cervical cancer

Objective: To establish the method for the detection of HPV16 early gene E6-related transcripts and analyze the HPV16 E6-associated transcription during the development of cervical cancer. Methods: Sixty- three HPV16 positive cervical tissues, including 8 LSIL (low-grade squamous intraepithelial lesion, 38 HSIL (high-grade squamous intraepithelial lesion) and 17 cervical cancer, were collected. The extracted RNAs were then reversed by a RT primer containing a conserved sequences at 5’ end. The cDNAs were then used as the templates to amplify the transcripts using the HPV16 E6 specific primers (P1) and the conserved sequence (P0). After separated by 2.5% agarose gel electrophoresis, the amplification products were detected by Southern blotting using an E6-specific probe to verify the HPV16 E6-related transcripts. Results: The method for the detection of HPV16 E6-related transcripts was established. Further analysis showed that HPV16 oncogenes transcription patterns were significantly different in the tissues of LSIL, HSIL, and cervical cancer. The types of HPV16 E6-associated transcripts increased during the development from LSIL to cervical cancer. Conclusion: The method for the detection of HPV16 E6-related transcripts is successfully established and the patterns of HPV16 E6-associated transcripts are closely associated with the malignant degree of cervical neoplasm and may be involved in the development of cervical cancer.

Effect of recombined granulocyte colony-stimulating factor on cerebral cortical proteins of ischemia-reperfusion injury rats

Objective: To research the relation between difference protein with neuron protective effect of G-CSF in ischemia-reperfusion rats brain by using proteomics. Methods: Twenty-four experimental Wistar rats were divide into 3 groups, control group, rats cured by G-CSF group (G-CSF group), ischemia-reperfusion injured rats group (I/R group). Ischemia-reperfusion injury rats model was generated by using Koizumi’s way in G group and I/R group, one nylon thread was used to block the rats middle cerebral artery and reperfused after 2 hours. G-CSF (50 μg/kg/d) was injected subcutaneously on rats’ backs for successive 5 days in G-CSF group. Rats were executed and decapitated after 14 days after reperfusion to get the brain respectively. Sodium chloride injection was used in I/R group and control group. cortex proteins were extracted in the 3 groups of rats. Then the maps of the proteins were established by DIGE (differential gel electrophoresis, DIGE). The altered protein spots were identified with MALDI-TOF-MS and database searching. Results: Compared with the contrl group, the I/R group gained 56 differential protein spots, 39 spots expressed lowly, and 17 spots high. Compared with I/R group, the G-SCF group gained 35 differential protein spots, 16 spots expressed lowly, and 19 spots high, identified 6 protein spots, including dihydropyrimidinase-associated protein 2, glial fibrillary acidic protein, endomucin, Rho GDP dissociation inhibitor, Rab GDP dissociation inhibitor and guanine-nucleotide-binding protein. Conclusion: G-CSF is involved in neuroprotection after brain ischemia, possibly by regulating the expression of various neural regeneration-associated proteins at the subacute stage.

Experimental research of the hNIS transfected SW480 cells mediated radioiodine therapy

Objective: To detect the killing effect of colon cancer cells (SW480) by iodine 131 (131I). Methods: Cell apoptosis was detected when taken in 131I in vitro test and perchlorate suppression experiments and AnnexinV-FITC/PI double dye flow cytometry instrument. Results: Stably-hNIS-gene-expressing cell line was about 8 times higher than the control group and blank plasmid group in 24 h iodine uptake capacity. Perchlorate inhibition tests showed that the iodine uptake of hNIS-SW480 was the hNIS gene’s function. AnnexinV-FITC/PI double dye flow cytometry showed apoptosis index in the transfection group was higher than that of the control group and blank plasmid group. There were significant differences that radioactive iodine SW480 cells transfected with better killing ability under 100 μCi 131I in 24 h. Conclusion: 131I has significant cell killing effect on SW480 cells transfected hNIS.

Effect of Shenmai injection on changes of patients’ cognitive function after received cardiopulmonary bypass

Objective: To observe the changes of patient’s cognitive function after undergoing cardiac valve replacement with cardiopulmonary bypass (CPB) and the intervention effect of Shenmai injection (SMI). Methods: Forty United States Association of Anesthesiologists (ASA) II or III patients (New York Heart Association, NYHA II or III) of the Mini-Mental State Examination (MMSE, range 0~30) scores≥26 after undergoing cardiac valve replacement with CPB were randomly divided into SMI group (group S, n=20) and control group (group C, n=20). After anesthesia induction, the patients of group S were infused intravenously with SMI (0.6 mL/kg added to 250 mL normal saline), and the patients of group C were given with equivalent normal saline at the same time, both completed before CPB. Aortic clamped time, CPB time, duration of anesthesia were recorded. Heart rate (HR), mean arterial pressure (MAP), jugular venous oxygen saturation (SJvO2) and jugular venous oxygen partial pressure (PJvO2) of anesthesia induction (T0), 10 min after induction (T1), CPB re-warmed to 36 ℃(T2), and 1 h (T3), 6 h (T4) , 24 h (T5), 48 h (T6), 72 h (T7) after CPB were recorded. MMSE were performed three days after surgery and the incidence of POCD was recorded. Results: There was no statistical significance in aortic clamped time, duration of CPB, anesthesia time, and hemodynamics (P>0.05) between the two of groups. Compared with T0, SJvO2 and PJvO2 of group C from T1 to T2, T4 to T7 and group S at T1, T4 to T7 decreased (P<0.05, P<0.01). Compared with group C, SJvO2 and PJvO2 of group S at T2 and T3 increased (P<0.01). MMSE evaluation and the incidence of POCD of group S were better than those of group C three days after surgery (P<0.05, P<0.01). Conclusion: Cardiac valve replacement with CPB can lead to cognitive impairment in patients and Shenmai injection can improve cognitive function by increasing oxygen supply for brain tissue.

Effect of corneal incision size on tear function after phacoemulsification

Objective: To explore the effect of different clear corneal incision size on change in tear function after phacoemulsification cataract surgery. Methods: Sixty patients (60 eyes) recruited for cataract surgery at the Eye Hospital of Wenzhou Medical University from November 2012 to May 2013 were randomly divided into two groups, A group (30 eyes) underwent regular 3.0 mm clear corneal incision phacoemulsification, group B (30 eyes) underwent 1.8 mm clear corneal micro-incision phacoemulsification. Before the surgery and 1 day, 1 week, 1 month, and 3 months after surgery, all subjects underwent measurements, including dry eye symptom score (DES), lower tear meniscus area (LTMA) and total tear meniscus area (TTMA) by optical coherence tomography (OCT), tear film break-up time (tBUT) and Schirmer test (ST-II). Statistic analysis used repeated measurement data analysis of variance, t-test and Pearson’s correlation. Results: In general, comparing to group A, the postoperative DES in group B was significantly lower (F=26.96, P=0.001), LTMA and TTMA in group B were significantly higher (F=56.21, 39.36, respectively, both P<0.001), tBUT in group B was significantly higher (F=28.39, P=0.001), and ST-II in group B was significantly higher (F=17.44, P=0.01). On 1 day and 1 week after surgery, DES in group B was significantly lower than that in group A (t=1.89, 1.55, respectively, both P<0.05). On 1 week after surgery, LTMA, TTMA and ST-II in group B were significantly higher than that in group A (t=1.38, 1.49, 1.82, respectively, all P<0.05). On 1 week, 1 month and 3 months after surgery, tBUT in group B was significantly higher than that in group A (t=2.32, 2.09, 1.58, respectively, all P<0.05). No significant difference was found at the other time points between the two groups. Comparing to the preoperative levels, DES in both groups increased on 1 day and 1 week after surgery (both P<0.05). Only in group A, the objective tear functional parameters changed significantly, such as LTMA (on 1 day and 1 week after surgery, both P<0.01), TTMA (on 1 day, 1 week and 1 month after surgery, Post Hoc, all P<0.05), tBUT (on 1 week after surgery, P<0.05), ST-II on 1 week and 1month after surgery, (both P<0.05), while no significant change was found in group B (all P>0.05). Conclusion: 1.8 mm clear corneal micro-incision phacoemulsification surgery has significantly less effect on change in tear function in the early postoperative period comparing to the 3.0 mm incision control, which has advantages in improving postoperative tear function and comfort.

Relationship between the sensitivity of neoadjuvant chemoradiotherapy and the expression of HIF-1α and COX-2 in rectal cancer

Objective: To investigate the relationship between the sensitivity of neoadjuvant chemoradiotherapy and the expression of HIF-1alpha and its downstream gene COX-2 in rectal cancer. Methods: Between April 2008 and March 2013, 45 patients with histologically proven rectal cancer who underwent neoadjuvant chemoradiotherapy followed by surgery were investigated. Immunohistochemical assay was performed in pre-chemoradiotherapy specimens to determine the expression of HIF-1alpha and COX-2 protein. All cases were evaluated with modified Rectal Cancer Regression Grade (mRCRG) and T- level downstaging following neoadjuvant chemoradiotherapy. Results: The percent of achieving mRCRG I was higher in group of low HIF-1alpha expression than that of high HIF-1alpha expression after neoadjuvant chemoradiotherapy (50.0% vs 16.0%, P<0.05). The percentage of achieving T-level downstaging was also the same (70.0% vs 32.0%, P<0.05). The expression of COX-2 did not correlate with mRCRG and T-level downstaging in these patients (both P>0.05). There was low positive correlation between the expression of HIF-1alpha and COX-2 in biopsy specimens of rectal cancer (r= 0.36, P<0.05). Conclusion: The expression of HIF-1alpha is associated with efficiency of neoadjuvant chemoradiation of rectal cancer. Therefore, it can be used as the sensitive predictor of neoadjuvant chemoradiation of rectal cancer.

Clinical application of low dose research for coronary CT angiography with 320-slice volume CT

Objective: To evaluate and compare the image quality and radiation dose of coronary computed tomography angiography (CCTA) received by prospective ECG-gated CTA mode with tube current modulated CFA mode on 320-slice volume CT. Methods: Ninety-seven patients with suspected coronary artery disease were randomly divided into two groups: 53 consecutive patients were performed CCTA with tube current modulated CFA mode (group A) and 44 patients with prospective electrocardiographic gating (group B). Evaluating coronary artery by subjective and objective judgements, the image quality of CCTA were subjectively evaluated using a 4-point scale, while objectively evaluated by comparing mean intraluminal attenuation (SI), noise (SD), signal-to-noise ratio (SNR), contrast-to-noise ratio (CNR) of the ascending aorta. The image quality of coronary artery between groups was compared by Mann-Whitney U test, and the radiation dose between groups was compared by t test. Results: The average radiation dose was (8.66±3.67) mSv in group A and (3.99±2.42) mSv in group B, with significant difference for A vs B (t=-7.503, P<0.05). Of 1187 coronary artery segments in group A and B, good image quality (score of 3 or 4) was achieved in 92.4% (599∕648) in group A, 92.9% (501/539) in group B, with no significant difference for A vs B (Z=-0.480, P=0.631). The values of SI, SD, SNR, CNR in group A were (433.4±82.5)HU, (22.1±6.2), (20.7±5.7), (16.6±5.0), and the values of SI, SD, SNR, CNR in group B were (436.1±94.7) HU, (21.5±4.9), (20.9±5.5), (16.9±4.7). There was no significant difference of the image quality between group A and B objectively (P>0.05). Conclusion: 54% radiation dose of CCTA is reduced in 320-slice volume CT scan by using prospective ECG-gated CTA mode, moreover, the diagnostic quality of the image is also ensured.

Influence of estrogen level on the coagulation and fibrinolysis in patients with hysteromyoma and its clinical significance

Objective: To explore the influence of endogenous estrogen level on the coagulation and fibrinolysis in patients with hysteromyoma and its clinical significance. Methods: Serum Estradiol (E2), prothrombin time (PT), activated partial thromboplastin time (APTT), fibrinogen (FIB), D-Dimer (D-D), activity of factor VIII (FVIII:C) and antigen of Von Willebrand Factor (VWF:Ag) were measured by one-stage recalcification clotting time assay, immunoturbidimetry assay and chromogenic substrate method in 142 patients with hysteromyoma and 80 healty controls. The patients with hysteromyoma were grouped by the quartile method with their estrogen levels and comparative analysis. Results: Compared with the controls, the PT, APTT and FIB of the patients with hysteromyoma were obviously lower, but the D-Dimer was higher (respectively, P<0.05), and there were no significant differences in serum E2, FVIII:C and vWF:Ag (respectively, P>0.05). Grouped by the endogenous estrogen level, the PT and APTT were shorter in the Q4 group of the patients with hysteromyoma than that in the other groups, and the D-Dimer was obviously higher (respectively, P<0.05), but there were no significant differences in the other parameters (respectively, P>0.05). Conclusion: The coagulation function and the secondary fibrinolytic function in the patients of hysteromyoma can relatively enhance, though there is no significant differences at the whole estradiol level in the patients of hysteromyoma, there is relatively enhancement in the coagulation and fibrinolysis in the patients of the higher estradiol level.

The clinical value of prophylactic antibiotics application under the guidance of procalcitonion in intestinal obstruction

Objective: To explore the clinical value of prophylactic antibiotics application under the guidance of procalcitonion in intestinal obstruction. Methods: The results of prophylactic antibiotics application were observed and analyzed in 80 cases of non-surgical treatment of intestinal obstruction. Procalcitonion and blood routine were tested in all cases in 1, 3, 5 day after admission such as cut point for research. Forty patients (experimental group) stopped using antibiotics under the normal level of procalcitonion (<0.5 ng/mL). The rest of 40 patients (control group) stopped using antibiotics on the basis of traditional standard. Results: There was no significant difference between two groups in cure time and infection complications (P>0.05); there was very significant difference between two groups in average duration of antibiotics application, the experimental group was significantly shorter than that of the control group (P<0.01). Conclusion: Prophylactic antibiotics application under the guidance of procalcitonion is superior to the traditional standard in intestinal obstruction patients.

Detection and correlation study of IL-37, IL-1β and IL-18 in gout patients

Objective: To determine levels of interleukin-37 (IL-37), IL-1β and IL-18 in serum of patients with gout at difference stages, and to explore the role of IL-37 in the pathogenesis of gout. Methods: The levels of IL-37, IL-1β and IL-18 in 20 patients with gout, 20 patients with hyperuricemia and 20 healthy controls were detected wtih ELISA, the changes and correlations in different groups were analyzed. Results: Gout group in acute attack stage IL-1β and IL-18 levels was significantly higher than those in complete remission period (P<0.05), while the IL-37 level was only slightly higher than that of complete remission period (P>0.05). IL-37, IL-1β and IL-18 in gout group at acute attack stage were significantly higher than that in the hyperuricemia group and healthy control group (P<0.05). The higher expressions of IL-37 in complete remission period were detected and compared with the hyperuricemia group and healthy control group, whereas there was no difference in IL-1β and IL-18 levels (P>0.05). The level of serum IL-37 in gout group at acute attack stage, was positively correlated with ESR, CRP, IL-1β and IL-18 (P<0.01). Conclusion: IL-37 as an inhibiting inflammation factor probably takes part in the process of inflammatory respense of gout.

Influence of telmisartan on the impaired glucose regulation in patients with hypertention

Objective: To explore the influence of telmisartan on the impaired glucose regulation (IGR) of patients with hypertention. Methods: Ninety patients with IGR and hypertension who were selected in our hospital were randomly divided into two groups. The patients in study group (45 cases) were treated by taking telmisartan 40~80 mg per day for 12 weeks and the patients in control group (45 cases) were required to take irbesartan 75~150 mg per day for 12 weeks. Their blood pressure (BP) and the insulin sensitivity including fasting plasma glucose (FPG), 2 hours postprandial glucose (2hPG), glycosylated hemoglobin, fasting insulin (FINS) and insulin resistance index (IRI) were observed and analysed after 12 weeks. Results: Systolic blood pressure (SBP) and diastolic blood pressure (DBP) decreased significantly in both groups after treatment (P<0.05). Compared with the control group, FBG, 2hPG, HbA1c, FINS and IRI in the study group decreased after telmisartan treatment (P<0.05). Conclusion: Telmisartan can effectively improve glucose metabolism in the patients with IGR and hypertension.

Objective: To assess the feasibility, safety and effectiveness of Novasure impedance to control endometrial ablation system in the treatment of abnormal uterine bleeding. Methods: From June 2011 to June 2013, 50 women with abnormal uterine bleeding who received Novasure system for treatment were retrospectively analyzed. The clinical effect and complications were followed up in months 1, 3, 6, 12, 24. Results: The procedures were completed successfully in all patients. The work time of the radio frequency was (80.57±4.38) s (ranged from 57~128 s). The average duration of the operation including anesthesia was (15±4.6) min. The blood lost was less than 10 mL during the operation. The average hospitalization time after operation was (40±7.2) h. All the patients were followed-up for 24 months. The effective rates in 1, 3, 6, 12 and 24 months were 100% (50/50), 90% (45/50), 86% (43/50), 92% (46/50) and 96% (48/50) respectively. The rate of amenorrhea was 46% (23/50), 50% (25/50), 48% (24/50), 54% (27/50) and 76% (38/50) respectively. The degrees of satisfaction were 100% (50/50), 90% (45/50), 90% (45/50), 92% (46/50) and 96% (48/50). No severe perioperative or postoperative complications occurred. Conclusion: Novasure impedance controlling endometrial ablation system which is worth promoting in clinical is safety and effective in the treatment of abnormal uterine bleeding.

Observation on the effect of non-nutrient sucking to relieve infant pain during lacrimal irrigation procedure

Objective: To observe the effect of non-nutrient sucking on relieving infant pain during lacrimal irrigation procedure. Methods: One hundred full-term infants requiring lacrimal duct flushing from postnatal 60 days to 90 days were randomly given non-nutrient sucking intervention or routing treatment, the neonatal pain rating scale between two groups was compared, heart rate and blood oxygen level were detected after 5 min. Results: Score of the conventional treatment group was 4.17 +/- 1.26, and of non-nutrient sucking group was 3.08 +/- 1.32 in the lacrimal duct flushing for 1 min, the difference between two groups was statistically significant (P<0.01), there was no statistically significant difference of lacrimal duct flushing for 5 minutes between two groups (P>0.05), the difference of heart rate between two groups after the lacrimal duct flushing for 5 minutes had statistical significances, but the blood oxygen level between the two groups had no statistical differences (P>0.05). Conclusion: Giving the non-nutrient sucking can alleviate pain in infants during lacrimal duct flushing operation, the non-nutrient sucking method is easy to use, and has few side effects, so it can be used in many ophthalmological operations such as lacrimal duct flushing to relieve pain in infant.
