775 Application of eye tracking technology in screening children with attention deficit hyperactivity disorder

Objective: To explore the differences with eye-tracking technology in visuospatial attention,response inhibition, and eye movement control between children with Attention Deficit Hyperactivity Disorder (ADHD) and typically developing children, aiming to provide a more real-time and objective screening method for ADHD. Methods: A total of 140 children were selected from the Department of Child Health, Wenzhou People’s Hospital, from January 2023 to January 2024. Among them, 40 children were diagnosed with ADHD,and 100 healthy children (control group) were recruited during the same period. Eye-tracking equipment was used to detect participants’ eye movement while performing fixation tasks, saccade tests, and anti-saccade tests,including metrics such as average fixation duration, fixation number, and duration before first fixation, with a comparison of the differences in eye movement data between the two groups. Logistic regression analysis was used to assess the association between eye movement data and ADHD diagnosis and ROC curve analysis to evaluate the screening efficacy of various eye movement parameters. Results: Multivariate logistic regression analysis showed that in the fixation task, the average saccade duration in the ADHD group was significantly associated with ADHD diagnosis (OR=0.18, 95%CI=0.05-0.68, P<0.05). In the correct anti-saccade test, the time before first fixation was significantly associated with ADHD diagnosis (OR=0.96, 95%CI=0.93-1.00, P<0.05). In the erroneous anti-saccade test, the average fixation duration (OR=0.01, 95%CI=0-0.31, P<0.05), total fixation duration (OR=0.56, 95%CI=0.37-0.85, P<0.05), maximum fixation duration (OR=0.06, 95%CI=0.01-0.43, P<0.05), and the number of fixations (OR=0.83, 95%CI=0.71-0.98, P<0.05) were all significantly associated with ADHD diagnosis. ROC curve analysis showed that the AUC value of average saccade duration during the fixation task for ADHD screening was 0.697, the AUC value of time to first fixation in the correct anti-saccade test was 0.756, and the AUC values of average fixation duration, total fixation duration, maximum fixation duration,and number of fixations in the erroneous anti-saccade test were 0.716, 0.748, 0.733, and 0.722, respectively, all demonstrating good screening efficacy. Conclusion: Eye-tracking technology can effectively reveal differences in visual attention and eye movement control between children with ADHD and typically developing children,providing a new real-time, objective assessment tool for ADHD screening.

783 Nutritional status fluctuation and influencing factors in patients with multiple myeloma during induction chemotherapy

Objective: To observe prospectively and analyze the fluctuation of nutritional status and its changing regularity in patients with multiple myeloma during induction chemotherapy and to explore the influencing factors of malnutrition. Methods: A total of 140 newly diagnosed patients with multiple myeloma in the Second Affiliated Hospital of Air Force Medical University from January 2019 to December 2022 were selected as the research subjects. Patient-generated subjective global assessment (PG-SGA) score, BMI and serum albumin (ALB) were used to evaluate nutritional status of patients at 5 time points before chemotherapy and during chemotherapy cycles 1-4. Repeated measures analysis of variance was used to analyze the fluctuation trend of nutritional status in patients with multiple myeloma during induction chemotherapy, and generalized estimating equation was used to analyze the potential influencing factors of malnutrition. Results: A total of 136 patients completed 5 surveys, and the incidence of malnutrition fluctuated between 37.50% and 56.62% during induction chemotherapy. The PG-SGA score of the patients decreased first and then increased (P<0.001). At the same time, BMI and ALB increased first and then decreased (P<0.001), but subgroup analysis showed that the ALB level of patients with efficacy ≥PR in the late stage of induction chemotherapy showed an upward trend (P<0.001). The PG-SGA malnutrition of multiple myeloma patients during the whole induction chemotherapy period was affected by age, initial malnutrition, hospital anxiety and depression scale (HADS score), infection after chemotherapy, efficacy and nutritional support (P<0.05); Malnutrition based on BMI was affected by age,Pittsburgh sleep quality index, HADS score, disease stage, initial malnutrition and nutritional support (P<0.05);The malnutrition under ALB standard was affected by age, efficacy, infection after chemotherapy, disease stage and nutritional support (P<0.05). Conclusion: There’s a trend of dynamic change during the phase of induction chemotherapy in patients with multiple myeloma. Elderly multiple myeloma patients in stage Ⅲ with sleep disorders, anxiety and depression, initial malnutrition, infection after chemotherapy, failure to achieve PR and no nutritional support are more prone to suffer from malnutrition, to whom more attention should be paid during induction chemotherapy.

791 Hirsutine mitigates high-fat-induced apoptosis in H9C2 cells by activating mitophagy

Objective: To explore the effect of Hirsutine on mitophagy and apoptosis in H9C2 cells induced by high-fat. Methods: With untreated H9C2 cells served as the control group (Control), H9C2 cells stimulated by palmitic acid were used as the high-fat group (PA), H9C2 cells treated with Hirsutine as the treatment group (PA+HS) and H9C2 cells treated with the Rho kinase inhibitor Y-27632 as the inhibitor group (PA+HS+Y-27632).CCK8 assay was used to evaluate cell viability; TUNEL staining was used to detect cell apoptosis; TMRE staining was used to detect changes in mitochondrial membrane potential (mPTP); Western blot was used to detect the expression of mitophagy proteins LC3I/II, Pink 1 and Parkin as well as apoptosis-related proteins Cleavedcaspase 3, caspase 3, Bax, and Bcl-2. Results: Compared with the control group, the viability of cells treated with 200, 400, and 800 μmol/L PA decreased (P<0.05); the TMRE fluorescence intensity was significantly weaker,while the expression of LC3-I/II, Pink , and Parkin was significantly lower (P<0.05); the TUNEL-positive cells increased significantly, and the expression of Cleaved-caspase 3/caspase 3 and Bax/Bcl-2 increased significantly (P<0.05) in the PA group. Compared with the PA group, the TMRE fluorescence intensity was significantly stronger; the expression of LC3-I/II, Pink , and Parkin was significantly higher (P<0.05); the TUNEL-positive cells decreased significantly, and the expression of Cleaved-caspase 3/caspase 3 and Bax/Bcl-2 decreased significantly (P<0.05) in the PA+HS group. Compared with the PA+HS group, the TMRE fluorescence intensity was weaker; the expression of LC3-I/II, Pink , and Parkin was significantly lower (P<0.05); the TUNELpositive cells increased significantly, and the expression of Cleaved-caspase 3/caspase 3 and Bax/Bcl-2 increased significantly (P<0.05) in the PA+HS+Y-27632 group. Conclusion: Hirsutine alleviates apoptosis of H9C2 cells induced by high-fat through the activation of mitophagy.

798 Evaluation of left ventricular systolic function reserve in patients with non-obstructive hypertrophic cardiomyopathy using myocardial work combined with exercise stress echocardiography

Objective: To quantitatively evaluate the changes of left ventricular myocardial work before and after Squatting-Rising loading test in patients with non-obstructive hypertrophic cardiomyopathy (NHCM) by using noninvasive left ventricular pressure-strain loop (LVPSL), providing reference for clinical evaluation of left ventricular contractile reserve function, and individualized exercise guidance. Methods: A prospective analysis was made of 37 non-obstructive hypertrophic cardiomyopathy patients diagnosed by echocardiography in the First Affiliated Hospital of Wenzhou Medical University from January 2022 to December 2023. LVGLS and myocardial work parameters were estimated before and after Squatting-Rising loading test, and the relationship between LVGLS and myocardial work parameters was analyzed. Results: During the afterload-elevation,systolic blood pressure increased by (18±13) mmHg (P<0.01), followed by a slight increase in LVEF from (65.09%±4.91%) to (66.01%±4.92%) (P>0.05). There was a significant increase in LVGLS, GCW and GWW following the loading test (P<0.05). However, there were no significant differences in GWI and GWE before and after loading test. Furthermore, LVGLS was positively correlated with GWI, GCW, and GWE (P<0.05), while negatively correlated with GWW (P<0.05) during rest. After loading text, LVGLS was positively correlated with GWI, GCW, and GWE (P<0.05), but not correlated with GWW. The repeatability of myocardial work parameters was better in both groups. Conclusion: This study demonstrated that a modest increase in afterload can result in slight increase in LVGLS and significant increase in GCW, LVPSL combined with exercise stress echocardiography provides a more sensitive assessment on the measurement of myocardial reserve in NHCM patients.

806 Application of AI-assisted diagnostic system in cervical liquid-based thin-layer cytology

Objective: To verify with clinical data the application value of AI-assisted diagnostic system in cervical liquid-based thin-layer cytology examinations and to explore the feasibility of in-depth development of AI technology in the field of pathological medicine. Methods: A retrospective analysis was conducted on 1 500 cases of cervical liquid-based thin-layer cytology smears collected from February to November 2023 at the First Affiliated Hospital of Wenzhou Medical University. The dataset comprised 843 membrane preparations and 657 sedimentary preparations. The sensitivity, specificity, and accuracy of cytopathologists alone, the AI-assisted diagnostic system alone, and cytopathologists utilizing the AI-assisted diagnostic system were evaluated, alongside a comparison of reading time. Results: Cytopathologists utilizing the AI-assisted diagnostic system demonstrated a sensitivity of 92.83%, specificity of 98.56%, and accuracy of 97.33%, compared with the AI-assisted diagnostic systems alone (with a sensitivity of 81.93%, specificity of 92.71%, and accuracy of 90.40%) and the cytopathologists alone (with a sensitivity of 82.24%, specificity of 97.63%, and accuracy of 94.33%), showing significant difference (P<0.05). Compared with the AI-assisted diagnostic system alone and the cytopathologists alone, cytopathologists utilizing the AI-assisted diagnostic system demonstrated significant improvement in sensitivity, specificity, and higher accuracy in both membrane-based and sedimentation-based slide preparation methods,with statistical significance (P<0.05). In addition, the average time for AI-assisted physicians to read images was 58 s/image,which was significantly lower than 70 s/image for cytopathologists alone. Conclusion: Cytopathologists leveraging AI-assisted diagnostic system benefit from heightened sensitivity, specificity, and accuracy. The adoption of AIassisted diagnostic system enhances screening efficiency and alleviates the workload of cytopathologists.

811 Clinical features and genotype analysis of two pedigrees with hereditary protein C deficiency

Objective: To investigate the clinical features and gene mutations in two families with hereditary protein C (PC) deficiency, and to explore the relationship between PC deficiency and vascular thromboembolism.Methods: Peripheral blood and clinical data were collected from two probands and their family members (eleven people across three generations), and the PC activity, protein S activity and antithrombin activity were detected to confirm the phenotypic diagnosis. PCR was used to amplify and directly sequence the PROC gene of the probands.Software analysis was conducted to assess the conservativeness and pathogenicity of the mutated loci. Protein models were constructed to analyze the spatial structure before and after the mutation. Results: The clinical manifestations of proband 1 were pulmonary embolism and deep venous thrombosis of the lower extremities, and proband 2 had DVT. Genetic analysis revealed that the proband 1 had c.541T>G heterozygous missense mutation and c.577-579delAAG heterozygous deletion mutation and the proband 2 had a c.659G>A heterozygous missense mutation. Conservativeness and pathogenicity analysis confirmed that most of these amino acid residues were conserved, and all the mutations were found to be pathogenic. Protein models showed that most of these mutations caused changes in the hydrogen bonds between amino acids and side-chain groups or produced truncated proteins.Conclusion: The c.541T>G heterozygous missense mutation, c.577-579delAAG heterozygous deletion mutation and c.659G>A heterozygous missense mutation were related, respectively, to the decrease of PC level in these two probands, and these mutations may also be one of the reasons for the occurrence of VTE.

817 Application of gasless laparoendoscopic single site surgery in giant ovarian cysts

Objective: To explore the feasibility and application value of gasless laparoendoscopic single site surgery for giant ovarian cysts. Methods: Clinical data of 51 patients who underwent surgery for giant ovarian cysts from Huangyan Hospital Affiliated to Wenzhou Medical University between January 2020 and December 2023 were retrospectively analyzed, from which 26 cases of gasless laparoendoscopic single site surgery for ovarian cysts (the study group) and 25 cases of traditional laparoscopic surgery for ovarian cysts (the control group) were enrolled in this study. A comparative analysis of the perioperative surgical outcomes, postoperative pain scores and complications was conducted on the two groups. Results: There was no statistically significant difference in hospitalization costs, postoperative length of stay and perioperative complications (P>0.05). In comparison with the control group, the study group exhibited superior perioperative efficacy, as evidenced by reduced operation time, first anal exhaust time and first off bed time as well as less intraoperative bleeding,significantly lower rate of intracapsular fluid leakage, and higher degree of satisfaction with the incision cosmetic (all P<0.05). Conclusion: With the advantages of short operation time, minimal bleeding, low rate of intracapsular fluid leakage, quick recovery, good cost-effectiveness, and enhanced aesthetic appearance, gasless laparoendoscopic single site surgery is a safe and effective procedure for treating giant ovarian cysts.

823 Effects of different anesthesia depth on postoperative brain injury indicators and inflammatory mediators in elderly patients after general anesthesia

Objective: To investigate the effects of maintaining different depths of anesthesia within the recommended range of bispectral index on postoperative brain injury indices and inflammatory factors in elderly patients undergoing general anesthesia. Methods: A total of 40 elderly patients who underwent elective laparoscopic gastrointestinal tumor surgery at the First Affiliated Hospital of Wenzhou Medical University in 2021 were included in this study, who were randomly divided as the BIS40 group and the BIS55 group according to the random number table method, with 20 cases in each group. Both groups underwent intravenous induction,and inhalation anesthesia was used to maintain the appropriate depth of anesthesia, in which the intraoperative bispectral index (BIS) was maintained at 40-45 for patients in BIS40 group, and the BIS was maintained at 55-60 for patients in BIS55 group. The hemodynamic indices of the patients were recorded at the immediate time point of pre-induction (T0), completion of induction (T1), immediately after establishment of pneumoperitoneum (T2),specimen collection (T3), irrigation (T4), completion of surgery (T5), extubation (T6) and return to the ward (T7). Venous blood was collected preoperatively, 2 h, 6 h and 24 h postoperatively for the determination of serum hemoglobin (NGB), hypoxia-inducible factor (HIF-α), neuron-specific enolase (NSE), S100-β, IL-6 and TNF-α,respectively. Results: Compared with BIS55 group, patients in BIS40 group had greater dosage of sevoflurane,lower BIS value, and prolonged time of extubation (P<0.05). The serum TNF-α and IL-6 levels of patients in BIS40 group were lower than those in BIS55 group at 2 h after surgery (P<0.05), and the differences in serum NGB, NSE, S100-β, and HIF-α levels at the remaining time points were not statistically significant (P>0.05).Conclusion: Different depths of anesthesia within the recommended range of bispectral indices do not affect the index of postoperative brain injury after general anesthesia in elderly patients, but the level of inflammatory factors is lower in the early postoperative period in patients with BIS between 40-45.

828 Pericapsular nerve group block in combination with the supine coronal approach to anterior quadratus lumborum block for hip fracture surgery in elderly patients

Objective: To evaluate the perioperative analgesic effect of pericapsular nerve group (PENG) block in combination with the supine coronal approach to anterior quadratus lumborum block (SCAQLB) for hip fracture surgery in elderly patients. Methods: A total of 80 elderly patients undergoing hip fracture surgery in Cangnan Hospital Affiliated to Wenzhou Medical University from January 2023 to December 2023 were selected,who were randomly divided into 2 groups (n=40 each) using a random number table: PENG in combination withSCAQLB (group PS) and fascia iliaca compartment block (FICB) group (group F). All patients underwent nerve block for analgesia guided under ultrasound before changes of position for combined spinal-epidural anesthesia.The cumulative consumption of sufentanil and the time to first compression within 24 hours after surgery through patient-controlled intravenous analgesia (PCIA) were recorded. The visual analogue scale (VAS) scores at rest and during movement were recorded when patients entering the operation room (T0), 30 min after nerve block (T1),6 h (T2), 12 h (T3) and 24 h (T4) after surgery. Ease of spinal anesthesia positioning, rate of rescue analgesia at the time of the position changing, patient’s satisfaction score and complications such as quadriceps weakness were also recorded. Results: The cumulative consumption of sufentanil in group PS was lower than that in group F (P<0.001). Time to first PCIA request was prolonged significantly in group PS than that in group F (P<0.001).VAS scores at rest and during movement in group PS were lower than those in group F at T1 to T4 (P<0.05).The satisfaction rate of patients was significantly higher in group PS than in group F (P<0.05). The rate of rescue analgesia at the time of the position changing was lower in group PS than in group F (P<0.05). The satisfaction rate of anesthesiologists with body position in group PS was higher than in group F (P<0.05). No case of quadriceps weakness occurred in group PS, whereas 9 cases occurred in group F. Conclusion: PENG in combination with SCAQLB is superior to FICB in that it can provide good analgesic effect on the positioning pain before spinal anesthesia and postoperative pain in elderly patients undergoing hip fracture surgery, without weakening the quadriceps muscle strength but improving patient’s satisfaction.

833 Construction of a cloud-based remote monitoring system and its clinical application

Objective: To build a remote monitoring system based on cloud platform development that is suitable for multiple application scenarios such as intensive care and anesthesia monitoring, and can simultaneously monitor the conditions of multiple patients in adjacent units within any monitoring unit. Methods:With the aid of modern audio-video transmission technology and digital processing technology, combined with information technology, a remote monitoring system has been developed targeting the uniqueness and requirements of various application scenarios. A retrospective analysis was conducted on the clinical data of 648 patients randomly selected from bed numbers 002, 013, 040, 044 and 045 in the intensive care unit of the First Affiliated Hospital of Wenzhou Medical University from 2020 to 2023, including 437 males and 211 females.The patient data from 2020 were used as the control group without remote monitoring, while the patient data from 2021 to 2023 were divided by year into three observation groups of the remote monitoring system. Statistics were collected on three sorts of catheter-associated infection indicators among patients, including the planned and actual collection times of vital sign data in nursing records, and the response speed of real-time machine monitoring. Data analysis was performed using the R (version 4.2.1) tool. Results: Analysis of the three sorts of catheter-related infection indicators showed that compared with the control group, there was a statistically significant difference (P<0.001) between the three groups of ventilators and intravascular catheters. The infection rate significantly decreased in 2021, 2022, and 2023. The timeliness of data collection in nursing records improved significantly, with statistical difference (P<0.001). The average response time of the key interface of the system was as fast as within 0.2 seconds. Conclusion: The remote monitoring system based on the cloud platform runs well, effectively saves medical resources, improves medical efficiency and reduces medical costs, thus providing evidence for further promotion and application.

839 Clinical significance of blood coagulation function and lupus anticoagulant detection in patients withankylosing spondylitis

Objective: To investigate the clinical significance of coagulation function and lupus anticoagulant(LAC) detection in patients with ankylosing spondylitis (AS). Methods: A total of 60 patients who were diagnosed with AS in Wenzhou Hospital of Integrated Chinese and Western Medicine from July 2020 to June 2023 were selected as the experimental group, and 60 healthy people during the same period were selected as the control group. The levels of routine coagulation function [prothrombin time (PT), activated partial thromboplastin time (APTT), fibrinogen (FIB), thrombin time (TT), D-dimer (D-D)] and LAC were detected in the two groups. Mann-Whitney U test was used to compare the differences in the above indicators, and ROC curve was drawn to analyze the auxiliary diagnostic efficiency of each indicator. Results: There was significant difference in the levels of PT, APTT, FIB, DD and LAC between AS patients and healthy control group (P<0.05). The area under ROC curve of LAC, FIB and D-D was 0.97, 0.93 and 0.84, respectively. The Youden index was 0.90, 0.78 and 0.58, respectively.The sensitivity was 96.70%, 85.00% and 71.70%, respectively. Conclusion: Patients with AS have the risk of thrombosis, and the detection of coagulation function and LAC is of certain value in the auxiliary diagnosis.

843 Application of an integrated nursing device in exudation management after ascites puncture in patients with malignant tumor

Objective: To explore the application effect of a new seepage-proof nursing device in the fluid management after ascites puncture in tumor patients. Methods: From January 2023 to February 2024, 80 patients of malignant tumor with ascites admitted to our hospital were divided as the experimental group and the control group by random number table method, with 40 cases in each. The control group received conventional puncture catheter fixation and ascites drainage while the experimental group was treated with the integrated puncture point seepage-proof nursing device. The comprehensive indexes of puncture point leakage, infection, puncture pain score and dressing change times were observed in the two groups. Results: On the day after puncture, there was no significant difference in the amount of leakage around the puncture point between 2 groups (P>0.05). On the 3rd and 7th day after puncture, the amount of fluid in the experimental group was less than that in the control group, and the amount of fluid in the control group gradually increased with time (P<0.05), while there was no significant difference in the experimental group (P>0.05). On the 7th day after puncture, the incidence of infection, the incidence of skin inflammation around the puncture point and the number of dressing changes within 10 d in the experimental group were lower than those in the control group, with statistical significance (P<0.05). From the day after puncture, day 3 to day 7, the pain score at the puncture site in the control group gradually increased, with statistical difference (P<0.05), while the change of pain score in the experimental group showed no statistical significance (P>0.05). The NRS score of the puncture site in the experimental group on the 7th day after puncture was higher than that on the 3rd day and the day of puncture (P<0.05). Conclusion: The application of this new anti-leakage device can effectively reduce the occurrence of adverse reactions related to drainage after ascites puncture in tumor patients, such as seepage, infection, dermatitis and puncture point pain.

855 Advances in the study of pancreatic fat deposition

In recent years, the prevalence of obesity has increased year by year,which has become a serious global public health problem. Previous studies have shown that the phenomenon of pancreatic fat deposition is more common in patients with obesity. Current studies found an association between pancreatic fat deposition and obesity-related diseases, such as non-alcoholic fatty liver disease, metabolic syndrome, Type 2 diabetes and cardiovascular diseases. However, its related mechanism has not been fully clarified. Therefore, further research is needed to identify the clinical consequences, risk factors and pathogenesis of this pathological condition, and to discover effective interventions. This review summarizes the diagnostic methods, related mechanisms, risk factors, and clinical consequences of fatty pancreas. In addition, potential therapeutic approaches to address pancreatic fat deposition are discussed.
