Abstract Objective: To explore the correlation between lymphocytes and hemorrhagic transformation after thrombolysis and the prognosis of different stages. Methods: A total of 457 patients included in the First Affiliated Hospital of Wenzhou Medical University from January 2016 to December 2018 were treated with intravenous thrombolysis, and 324 patients were finally enrolled. Enrolled patients can be divided into hemorrhage transformation group and non-hemorrhage transformation group according to the CT imaging, good prognosis group and poor prognosis group according to the modified Rankin Scale (mRS). Baseline data of patients including lymphocyte count before thrombolysis were collected. The assessment of neurological dysfunction was performed using the National Institutes of Health Stroke Scale (NIHSS). The mRS was used to assess the clinical prognosis after 3 months and 12 months, respectively. Results: There was no significant difference in lymphocytes in the hemorrhagic transformation group (P>0.05). Lymphocytes [OR=0.640, 95%CI=(0.420, 0.975), P=0.038], NIHSS (OR=1.330, 95%CI=1.232-1.435, P<0.001), and diabetes (OR=2.203, 95%CI=1.199-4.047, P=0.011) were relevant factors in the 3-month prognosis. Lymphocytes count [OR=0.555, 95%CI=0.347-0.888, P=0.014], age (OR=1.045, 95%CI=1.015-1.075, P=0.003), NIHSS (OR=1.297, 95%CI=1.202-1.398, P<0.001), and diabetes (OR=2.769, 95%CI=1.455-5.272, P=0.002) were relevant factors in the 12-month prognosis study. Conclusion: Lymphocytes have no significant correlation with hemorrhagic transformation after thrombolysis. But High lymphocyte count is the protective factors for poor prognosis in patients with acute ischemic stroke after thrombolytic therapy both in the 3-month and 12-month.
