Abstract Objective: To investigate the imaging manifestations of growing pain in the lower limbs of children and to better understand this disease. Methods: The imaging findings of 4 patients with growing pain in the lower limbs in children were retrospectively studied. Results: X-ray manifestations showed no abnormalities in both knees and middle-lower segments of tibiofibula. MRI manifestations of knee joint included symmetrical patchy, spotted T1WI low signal and FS-T2WI/PD high signal shadows were found in both sides of medullary cavity of femur, tibia metaphysis and epiphyseal. The cortex was intact and continuous, and no obvious abnormal signal was found in the soft tissue around the joint. The abnormal changes of both tibia and soft tissue of leg were symmetrical patches, dotted T1WI low signal and FS-T2WI high signal shadows with unclear boundary, which were found in the middle and lower tibial medullary cavity on both sides with intact cortex. The medial head of gastrocnemius and anteromedial fascia of tibia showed strip-like low signal on T1WI and high signal on FS-T2WI. The AV values of the transverse and sagittal positions from the two groups were statistically different (P=0.001). Conclusion: The imaging changes of growing pain in children’s lower limbs are characterized by normal X-ray film and symmetrical bilateral bone marrow edema on MR. Combined with clinical signs, certain imaging diagnosis can be made.
