Abstract:Objective: To evaluate the value of computed tomography perfusion imaging in distinguishing inflammatory and malignant nodules. Methods: CT perfusion scans were performed in patients with lung nodule received CT perfusion between September 2010 and February 2013. Patients with lung cancer and/or inflammatory nodule were confirmed by operation and proved by pathology. The parameters of CT perfusion, including blood volume (BV), blood flow (BF), permeability-surface area product (PS), mean transit time (MTT) were produced by perfusion software, and peak high (PH), time to peak (TTP), PHSPN/PHAA were measured. CTP parameters between above subgroups were analyzed using Mann-Whitney test. Results: Thirty-two cases were effective, including 26 cases of lung cancer and 6 cases of inflammatory nodule. Inflammatory nodules had higher BF, PH, PHSPN/PHAA than lung cancers (P<0.05). SPNS with BF larger than 78 mL • min-1 • 100 g-1 or PHSPN/PHAA larger than 8.5% were more likely to be inflammatory (sensitivity of 88.5%, specificity of 100%, PPV of 100%, NPV of 66.7% and accuracy of 90.6% for BF; sensitivity of 80.8%, specificity of 83.3%, PPV of 95.5%, NPV of 50.0% and accuracy of 81.3% for PHSPN/PHAA). Conclusion: The CT pulmonary perfusion imaging is of some differential role in distinguishing the inflammatory nodule and malignant nodule.
