摘要目的:采用McNamara分析法评价成人严重骨性Ⅲ类错𬌗畸形经正畸正颌联合治疗后的效果。方法:选取2012年1月至2015年1月华西口腔医院正畸科需行正畸正颌联合治疗的骨性III类错𬌗畸形患者21例,正颌手术术式均为下颌升支矢状劈开截骨术和上颌Le Fort I型截骨术,治疗前后拍摄头颅侧位X线片,采用McNamara分析法分析测量结果。结果:与治疗前比,治疗后上颌长度、上颌突度增大,下颌长度、下颌凸度、下切牙凸距及下面高减小,差异均有统计学意义(P <0.05)。结论:经正畸正颌联合治疗后,严重的骨性畸形在矢状向及垂直向均得到了有效矫正,患者骨面型也有所改善。
Abstract:Objective: To evaluate the effect of McNamara analysis for orthodontic and orthognathic combined surgical treatment in skeletal class III malocclusion on adult patients. Methods: Twenty-one adult patients with skeletal class III malocclusion treated by orthodontic and orthognathic combined surgical treatment were randomly selected. Sagittal split ramus osteotomy and Le Fort I osteotomy were used in all orthognathic surgical treatment. Lateral cephalogram before and after treatment was evaluated through McNamara analysis by t test. Results: Maxillary length and maxillary protrusion increased, but mandibular length, mandibular protrusion,mandibular incisor protrusion and anterior lower facial height decreased after the treatment (P<0.05). Conclusion:After orthodontic and orthognathic combined surgical treatment, serious skeletal class III malocclusion was corrected in sagittal and vertical directions, and patient’s facial form was improved.
