摘要目的:考察第3天胚胎提前解冻培养至第5天移植的临床效果,探讨第3天冷冻胚胎的发育潜能及培养至第5天进行移植的可行性。方法:前瞻性分析第3天卵裂期冷冻胚胎较多的患者,将第3天胚胎解冻后直接移植(A组)与第3天胚胎提前解冻培养至第5天移植(B组)的周期进行比较。将解冻胚胎培养后移植前有胚胎发育成囊胚或早期囊胚的列为BI组;而将细胞数增加,但均无胚胎发育成囊胚的周期列为BII;培养后所移植胚胎细胞均未增加的列为BIII组。结果:与A组比较B组在胚胎种植率、流产率与临床妊娠率上差异无统计学意义(P>0.05);BI组(发育至囊胚)的临床妊娠率明显高于A组(66.67% vs. 37.5%,P<0.05),BIII组解冻后胚胎存活,但并没有进一步发育的周期数比例则高达8.62%(5/58)。结论:第3天冷冻胚胎提前解冻培养至第5天移植并不影响冷冻移植周期结局,反而有利于预测冷冻移植周期的临床效果。
Abstract:Objective: To examine the clinical transplantation outcomes of cleavage stage embryos thawed 2 days earlier for culture, in order to confirm the development potential of cryopreserved cleavage stage embryos. Methods: In this prospective clinical study, 122 cycles of ART treatment for infertility have been evaluated. Frozen embryos in blastocyst group thawed and then cultured in sequential media until blastocyst stage were compared with thawed embryos in cleavage stage group. Results: Clinical rates was significantly higher in blastocyst group (66.67%) compared with cleavage group (37.5%). No statistical differences were reported in implantation rates between the two groups. Conclusion: The results indicated that blastocyst formation after thawing of cleavage stage embryos is a good predictor for embryo viability and pregnancy outcome.
